[Tuberculosis Contact Investigation: Eight Years of Experience with the QuantiFERON-TB®Gold In-Tube in Lower Saxony, Germany].

Since the beginning of the 21st century, beside tuberculin skin tests (TST) and chest X-rays, interferon-gamma release assays (IGRA) have become available for diagnosing latent tuberculosis infection. In 2006, the Governmental Institute of Public Health of Lower Saxony (NLGA) established the IGRA in its laboratory, using the QuantiFERON-TB®Gold In-Tube (QFT). A cohort of 19 309 contact persons who were investigated during contact tracing by local public health departments (LPHD) was analyzed. Within 3 years, 75% of LPHDs in Lower Saxony established the QFT for tuberculosis contact tracing. Bet

Tuberculosis in healthcare workers - a narrative review from a German perspective.

INTRODUCTION: Despite the decline of tuberculosis in the population at large, healthcare workers (HCW) are still at risk of infection. METHODS: In a narrative review the TB risk in HCW and preventive measures are described, with the focus on epidemiology and Occupational Safety and Health (OSH) regulations in Germany. RESULTS: There is an increased risk of infection not only in pneumology and laboratories with regular contact with tuberculosis patients or infectious materials. Epidemiological studies have also verified an increased risk of infection from activities that involve close contact

[Examination of onset cases in contact investigation].

OBJECTIVES: To analyze and evaluate onset cases of tuberculosis detected in contact investigations and to apply the results to future countermeasures. METHODS: Index and secondary cases in contact investigations in which the secondary cases occurred in Osaka City between 2005 and 2008 were enrolled. The tuberculin skin test or QFT (QuantiFERON-TB Gold) was conducted to diagnose whether the contacts were infected with tuberculosis. X-ray examination of the chest was conducted to determine whether tuberculosis had developed, immediately or 6 months, 1 year or 2 years after the contact investiga

Investigation of healthcare-associated transmission of Mycobacterium tuberculosis among patients with malignancies at three hospitals and at a residential facility.

BACKGROUND: Immunocompromised patients have an increased risk of experiencing progression of latent Mycobacterium tuberculosis infection (LTBI) to active tuberculosis (TB) disease. In January 2002, 2 patients with leukemia (Patients 1 and 2) developed pulmonary TB after recent exposure at 3 hospitals (Hospital A, Hospital B, and Hospital C) and at a residential facility for patients with cancer. Neither was known to have LTBI. Within 1 year, 3 other patients with malignancy and TB disease had been identified at these facilities, prompting an investigation of healthcare facility-associated tran

Improving T-cell assays for the diagnosis of latent TB infection: potential of a diagnostic test based on IP-10.

BACKGROUND: There is a need for simple tools such as the M.tuberculosis specific IFN-gamma release assays (IGRA) to improve diagnosis of M.tuberculosis-infection in children. The aim of the study was to evaluate the performance of an IP-10 and IL-2 based tests for the diagnosis of M.tuberculosis-infection in recently exposed children from Nigeria. METHODOLOGY AND PRINCIPAL FINDINGS: Samples were obtained from 59 children at high risk of infection with M.tuberculosis (contacts of adults with smear and culture-positive tuberculosis) and 61 at low risk (contacts of smear-negative/culture-positiv

Screening in special populations: a "case study" of recent Vietnamese immigrants.

PURPOSE: To determine how the medical and social profile of a particular special population, Vietnamese immigrants, should be used to tailor screening protocols that differ from those designed for the general population. PATIENTS AND METHODS: A consecutive series of Vietnamese immigrants living in the United States for less than 6 months were evaluated by interviewer-administered standardized questionnaire and medical record review. A total of 99 new Vietnamese immigrants (47 women and 52 men) aged 19 to 71 years presenting for primary care to two neighborhood health centers between October 1

What factors may influence epidemiological situation of tuberculosis in Poland and in the world?

The authors present the review of factors influencing epidemiological situation of tuberculosis in Poland and in the world. The groups of increased risk of tuberculosis, and clinical conditions predisposing to activation of latent tuberculosis infection (LTBI) such as HIV, uremia, diabetes mellitus, transplantation of organs, treatment with glucocorticosteroids and with antibodies to TNF and to its receptors, were presented. The higher prevalence and worse prognosis of tuberculosis in elderly people was emphasised. The methods of LTBI recognition, according to recent recommendations, with spec

In-hospital contact investigation among health care workers after exposure to pulmonary tuberculosis in an intermediate tuberculosis prevalence area: A prospective study.

We carried out in-hospital contact investigations of patients with pulmonary tuberculosis and analyzed the prevalence of latent tuberculosis infection (LTBI) among health care workers (HCWs) after TB exposure. A prospective study was conducted of 872 HCWs who were exposed to 55 index cases diagnosed with active pulmonary TB. HCWs after TB exposure were evaluated both TST and chest X-ray at the time of enrollment and 12 weeks after exposure; 625 HCWs (71.6%) underwent both initial assessments; 41 HCWs (6.6%) had a positive TST result. After 12 weeks, 71.1% of HCWs with initial negative TST (n =

Safety and efficacy of MVA85A, a new tuberculosis vaccine, in infants previously vaccinated with BCG: a randomised, placebo-controlled phase 2b trial.

BACKGROUND: BCG vaccination provides incomplete protection against tuberculosis in infants. A new vaccine, modified Vaccinia Ankara virus expressing antigen 85A (MVA85A), was designed to enhance the protective efficacy of BCG. We aimed to assess safety, immunogenicity, and efficacy of MVA85A against tuberculosis and Mycobacterium tuberculosis infection in infants. METHODS: In our double-blind, randomised, placebo-controlled phase 2b trial, we enrolled healthy infants (aged 4–6 months) without HIV infection who had previously received BCG vaccination. We randomly allocated infants (1:1), accor

[Adverse events following immunization with BCG vaccine in Poland 1994-2000].

BACKGROUND: In Poland, since 1955 BCG mass vaccinations have been compulsory. During the last two decades the vaccination program has been developed and includes: 1) primary vaccination of the newborn, 2) vaccination of children aged 7 and 12, and 3) revaccination of 18 year old adolescents. More than 95% of newborns and about 80% of older children of this population have been vaccinated. BCG vaccine being in use in Poland bases on the Brazilian/Moreau sub strain and has been producing by the Polish Laboratory. Since 1994 the central register of adverse events of BCG has been introduced by Nat

[Two children with extrapulmonary symptoms due to tuberculosis].

Two patients came to their general practitioner for relatively minor problems: a 4-year-old boy came with a red eye and a 10-year-old girl with red foot soles. They came from Pakistan and Vietnam respectively. Their symptoms were due to tuberculosis, which diagnosis was established by Mantoux test and culture of a stomach aspirate. They were treated accordingly with isoniazid, rifampicin and pyrazinamide and with isoniazid, rifampicin and ethambutol respectively. These cases stress the importance of knowledge of the extrapulmonary manifestations of tuberculosis. As treatment exists and adequat

The spectrum of mycobacterial disease in a Dublin teaching hospital.

We describe our experience at our combined tuberculosis and public health clinic, which has been in existence since 1991, and compare it to a previous review of tuberculosis at St. Vincent's Hospital from 1974 to 1979. A total of 107 patients were treated, 58 male and 49 female, with a mean age of 53 years. Seventy-seven patients had pulmonary tuberculosis, 23 extrapulmonary and three had both. Four patients had miliary tuberculosis. Our figures reflect the fall in incidence of tuberculosis in Ireland since the 1970's and a rise in smear negative and presumptive cases. The epidemiology of tube

[Current pathomorphism and specific features of complex tuberculous uveitis treatment].

The study has indicated that in the 1990th, there has been considerable changes in the clinical picture and course of ocular tuberculous lesions. Their morbidity has tended to increase in children, adolescents, and young individuals. Patients with tuberculous uveitis with posterior eyeball lesions are more than 70%. The specific inflammatory process is primarily exudative and infiltrative, followed by rapid development of complications that prevail over the major focus, prevent treatment and substantially deteriorate visual functions. The findings are important not only for understanding the p

T-cell dysfunction in minimal-change nephrotic syndrome of childhood.

Cell-mediated immunity was studied in 25 children with minimal-change nephrotic syndrome in different stages of their disease. This was assessed by delayed cutaneous hypersensitivity reactions, number of T lymphocytes, and blastogenic response to phytohemagglutinin and PPD. Patients with active nephrosis could not become sensitized to dinitrochlorobenzene and had decreased response to common recall antigens. T-cell number ranged within normal values in all nephrotic patients. During active disease, lymphocyte transformation was markedly reduced, but improved considerably when cells were cultur
