[Radiologic aspects of traumatic lesions of the cervico-occipital junction].

INTRODUCTION: Traumatic lesions of cervical spine are common, but those of the cervico-occipital junction are less frequent but more serious. PURPOSE: Evaluate the radiological aspects of cervico occipital Junction traumatisms. METHODS: It is a retrospective study of 32 files of cervico occipital junction traumatisms, collected from January 1995 to December 2004. Imaging methods used were: conventional radiography and computed tomography. RESULTS: Cervico occipital junction traumatisms were less common. They account for 12.7% of cervical spine traumatisms and 0.64% of the admissions. Patien

A risk model for the prediction of recurrent falls in community-dwelling elderly: a prospective cohort study.

The object of this article was to determine the predictive value of risk factors for recurrent falls and the construction of a fall risk model as a contribution to a mobility assessment for the identification of community-dwelling elderly at risk for recurrent falling in general practice. The design was a prospective cohort study (n = 311). There were four primary health care centers. A sample stratified on previous falls, age, and gender of community-dwelling elderly persons aged 70 years or over (n = 311) was taken from the respondents to a mail questionnaire (n = 1660). They were visited at

[Pediatric spinal injuries].

Spine injuries during growth are rare, but in comparison to adults they are more often associated with neurologic impairment. They also may occur without visible injuries in X-rays. The problems of conventional radiologic diagnostics include before all the differential diagnosis between synchondrosis, apophysis and fracture lines. MRI is indicated in case of neurologic deficits without radiologic abnormalities. In principle the fracture types correspond to those seen in adults. In addition growth specific injuries of the end-plates (growth plates) or ring apophysis may occur. Stable compressio

A ten year analysis of the traumatic maxillofacial and brain injury patient in Amsterdam: complications and treatment.

Maxillofacial trauma is often associated with injuries to the cranium, especially in high-energy trauma. The management of such cases can be challenging and requires close cooperation between oral and maxillofacial surgery and neurosurgical teams. There are few reports in the current literature describing the complications that develop in patients with maxillofacial trauma and traumatic brain injury (TBI). Complications can be categorized as early or late and/or minor and major. The exact definition of complications and their categorization remains a matter of current debate. We present a 10 y

Elderly patients with maxillofacial trauma: the effect of an ageing population on a maxillofacial unit's workload.

The purpose of this study was to identify the aetiology and management of facial fractures in patients over 60 years old and to identify potential trends in caseload to assist with planning of resources for maxillofacial services in the coming decades. We made a prospective study over 2 years (2009-2010), during which all injured patients referred to the oral and maxillofacial surgery unit at The Canberra Hospital, Australia were recruited. The patients were classified into two groups: less than 60 years old (younger group); or 60 years old or more (older group). Factors studied included sex,

Epidemiology of school injuries in Alexandria.

This study was conducted to reveal the magnitude of school injuries in Alexandria, their epidemiological patterns and outcomes. All pupils attending the casualty department of Alexandria Students' Hospital during the scholastic year 1996-1997 with school injuries were included in this study. An interview questionnaire and a transfer sheet were used to collect relevant information. Data were analyzed and the 5% level was taken to judge the significance of the obtained results. During the study period, 3422 pupils sustained school injuries yielding a rate of 4.25 per 1000. This rate was signific

Impact of Intra-Operative Adverse Events on the Risk of Surgical Site Infection in Abdominal Surgery.

BACKGROUND: Intra-operative adverse events (iAEs) recently were shown to correlate independently with an increased risk of post-operative death, morbidity, re-admissions, and length of hospital stay. We sought to understand further the impact of iAEs on surgical site infections (SSIs) in abdominal surgical procedures and delineate which patient populations are most affected. We hypothesized that all patients with iAEs have an increased risk for SSI, especially those with pre-existing risk factors for SSI. PATIENTS AND METHODS: To identify iAEs, a well-described three-step methodology was used

Association of Opioid Use Disorder Treatment With Alcohol-Related Acute Events.

IMPORTANCE: Persons with opioid use disorder (OUD) and co-occurring alcohol use disorder (AUD) are understudied and undertreated. It is unknown whether the use of medications to treat OUD is associated with reduced risk of alcohol-related morbidity. OBJECTIVE: To determine whether the use of OUD medications is associated with decreased risk for alcohol-related falls, injuries, and poisonings in persons with OUD with and without co-occurring AUD. DESIGN, SETTING, AND PARTICIPANTS: This recurrent-event, case-control, cohort study used prescription claims from IBM MarketScan insurance databases

The relationship between temperature and hip and wrist fracture incidence.

INTRODUCTION: Predicting when fracture incidence will rise assists in healthcare planning and delivery of preventative strategies. The aim of this study was to investigate the relationship between temperature and the incidence of hip and wrist fractures. METHODS: Data for adults presenting to our unit with a hip or wrist fracture over a seven and eight-year period respectively were analysed. Incidence rates were calculated and compared with meteorological records. A Poisson regression model was used to quantify the relationship between temperature and fracture rate. RESULTS: During the respe

Changing pattern and outcome of pediatric chest injuries in urban Syria.

Background Pediatric chest injuries were infrequent in our practice, but the outbreak of the Syrian crisis resulted in an increase in number and a change in the pattern of thoracic trauma incidents. We compared our experience of pediatric chest injuries before and during the crisis. Methods We reviewed the records of 256 children aged 12.8 ± 5 years who were admitted to our hospital with the diagnosis of chest trauma over a 12-year period. Collected data included mechanism of injury, associated injuries, method of management, length of hospital stay, complications, and mortality. Results The i

Working to Increase Stability through Exercise (WISE): Study protocol for a pragmatic randomized controlled trial of a coached exercise program to reduce serious fall-related injuries.

Approximately one-third of older adults fall each year and fall-related injuries are a leading cause of death and disability among this rapidly expanding age group. Despite the availability of bisphosphonates to reduce fractures, concerns over side effects have dramatically reduced use, suggesting that other treatment options are needed. Though many smaller studies have shown that physical activity programs can reduce falls, no study has been adequately powered to detect a reduction in fall-related injuries. We present the design of a three-year randomized controlled clinical trial of 1130 adu

Sex Differences in the Association Between Pain and Injurious Falls in Older Adults: A Population-Based Longitudinal Study.

We investigated whether there are sex differences in the association between pain and incident injurious falls. A total of 2,934 people (ages ≥60 years) from the population-based Swedish National Study on Aging and Care in Kungsholmen (2001-2004) participated. Participants were followed up for 3 and 10 years for falls leading to hospitalization or outpatient care. Data were analyzed with flexible parametric survival models that adjusted for potential confounders. During the first 3 years of follow-up, 67 men and 194 women experienced an injurious fall, and over 10 years of follow up, 203 men a

Life-Threatening Cardiac Perforation After Posterior Spondylodesis.

This reports presents a case of a 62-year-old woman with hemorrhagic hypovolemic shock, respiratory distress, and pericardial effusion secondary to right atrial perforation caused by a cement embolus after dorsal spondylodesis. Despite optimal intensive care support for a supposed pulmonary embolism, the patient's condition markedly deteriorated. On delayed embolus recognition by contrast-enhanced computed tomography and transesophageal echocardiography, she had to undergo cardiac surgery. Cardiac involvement is a rare but important complication, with few cases described after vertebroplasty a

Can the provision of a home help service for the elderly population reduce the incidence of fall-related injuries? A quasi-experimental study of the community-level effects on hospital admissions in Swedish municipalities.

BACKGROUND: Fall-related injuries are a global public health problem, especially in elderly populations. The effect of an intervention aimed at reducing the risk of falls in the homes of community-dwelling elderly persons was evaluated. The intervention mainly involves the performance of complicated tasks and hazards assessment by a trained assessor, and has been adopted gradually over the last decade by 191 of 290 Swedish municipalities. METHODS: A quasi-experimental design was used where intention-to-treat effect estimates were derived using panel regression analysis and a regression discon
