Subclavian vein repair in patients with an ipsilateral arteriovenous fistula.

Management of subclavian vein occlusive disease in persons with an ipsilateral arteriovenous fistula can be challenging. From July 1991 to May 1993, nine patients underwent direct exploration and repair of an obstructed subclavian vein following medial claviculectomy. Eight patients had polytetrafluoroethylene (PTFE) grafts; one patient had a Brescia-Cimino fistula. Intractable arm edema was the major symptom in five of eight. The site of the occlusive disease ranged from the midsubclavian vein to the proximal innominate vein. Pathology varied from a focal occluding web to a long segment of in

Normal Costoclavicular Distance as a Standard in the Radiological Evaluation of Thoracic Outlet Syndrome in the Costoclavicular Space.

BACKGROUND: The costoclavicular space is a common site of thoracic outlet syndrome. When there is no anatomical alteration, the diagnosis of thoracic outlet syndrome is difficult. Several authors relate costoclavicular distance to symptoms of thoracic outlet syndrome; however, there is no standardized site for measurement of the costoclavicular distance. This study aimed to determine the standard costoclavicular distance at neurovascular bundle crossing points (near the subclavian vein [Measure V] and the subclavian artery/brachial plexus branches [Measure NA]) using high-resolution chest comp

Supraclavicular lymph node metastases in cervical cancer.

PURPOSE OF INVESTIGATION: To evaluate the outcome and prognostic factors of patients with supraclavicular lymph node (SCLN) involvement at primary diagnosis. METHODS: We reviewed the medical records of cervical cancer patients primarily treated at Chang Gung Memorial Hospital between 1987 and 2005. Thirty-three patients with histologically confirmed SCLN metastasis at primary diagnosis were eligible for analysis. Clinical and pathological features were analyzed for association with outcome. RESULTS: The 3- and 5-year survival rates of patients with SCLN metastasis were 16.5% and 16.5%, respe

Reappraisal of neonatal clavicular fracture: relationship between infant size and neonatal morbidity.

OBJECTIVE: To examine the extent of neonatal morbidity and its relation with infant size in newborns diagnosed with clavicular fracture after vaginal birth. METHODS: A retrospective case-control study was performed on all the infants diagnosed with clavicular fractures and born vaginally between July 1997 and June 2000. For each index case, a control matched for gestational age (within l week), birth weight (within 100 g), and delivered within the same 24-hour period (8 AM to 8 AM), was selected at random from the delivery suite registry. If a control could not be identified within the period

The incidence of retrosternal vascular injuries following acute traumatic posterior sternoclavicular joint injuries is less than had previously been considered.

BACKGROUND: It has previously been considered that approximately 30% of acute traumatic posterior sternoclavicular joint (SCJ) injuries are … J dislocation, and 8 had a posteriorly displaced Salter-Harris II fracture of the medial end of the clavicle. The mean age at the time of injury was 27.6 years (15-69), and the mean follow-up was 77.8 months (25-131). One patient had mediastinal symptoms in the form of dyspnea at the time of injury. On CT arteriogram, there was no evidence of a vascular in …

Chest dual-energy CT scans revealed multiple tophi in the sternum, bilateral clavicles, scapulae, multiple ribs, and along the edges of the sternum. Based on the patient' s medical history, physical findings, and imaging studies, a diagnosis of gout was confirmed. Treatment included urate-lowering therapy, pain management, and dietar …

Ultrasound-Guided Subclavian Vein Catheterization: Contrasting Supraclavicular and Infraclavicular Approaches for Enhanced Procedural Precision.

Background Ultrasound-guided subclavian vein catheterization is crucial for central venous access, but the choice between the supraclavicular and infraclavicular approaches lacks comprehensive comparison. This study addresses this gap by conducting a prospective observational analysis of both techniques. The supraclavicular method accesses the vein from above the clavicle while the infraclavicular targets it below. Our model-driven approach aims to elucidate the procedural nuances, success rates, and complications associated with each method. The findings intend to equip clinicians with eviden…

INTRODUCTION: Adolescent idiopathic scoliosis (AIS) is a common diagnosis managed by pediatric orthopedic surgeons with nonoperative radiogr … raphic index across all time points. For positioning, 46% of respondents favored arms bent touching clavicles as the ideal positioning for X-rays, and another 24% favored arms down with palms forward (Table 2). The majority of respondents obtain lateral X-rays at the first visit (99%) and at the preoperative visit (70%). At the preoperative visit, sagittal …

Bone marrow edema in the acromioclavicular joint after arthroscopic rotator cuff repair.

BACKGROUND: Bone marrow edema (BME) in the acromioclavicular joint (ACj) may occur after arthroscopic rotator cuff repair (ARCR), resulting in persistent postoperative ACj pain. We investigated the incidence and clinical characteristics of BME in ACj after ARCR. METHODS: Patients exhibiting ACj-related symptoms prior to ARCR surgery, no BME in the ACj on MRI, and an ability to undergo 2-year postoperative physical and MRI examinations of the ACj were included. Patients who underwent distal clavicle resection or osteophyte resection below the ACj and those unable to achieve primary repair of t…

Arthroscopic shoulder surgery under general anesthesia with brachial plexus block: postoperative respiratory dysfunction of combined obstructive and restrictive pathology.

OBJECTIVE: Changes in respiratory parameters and pulmonary function tests were evaluated after shoulder arthroscopic surgery with brachial plexus block (BPB). The purpose of this study was to identify the mechanism of respiratory dysfunction after this type of surgery. METHODS: Patients undergoing arthroscopic rotator cuff repair under general anesthesia (GA) with BPB were enrolled in the arthroscopy group (n = 30) while those undergoing open reduction of a clavicle or humerus fracture under GA were enrolled in the control group (n = 30). Forced vital capacity (FVC) and forced expiratory volu…
