OBJECTIVE: To assess the suitability of the intramedullary interlocking nail to stabilise humeral diaphyseal fractures in dogs and cats.
METHOD: This multi-centre study retrospectively examined medical records, between June 1994 and May 2001, of 19 dogs and one cat, in which a total of 21 humeral fractures were stabilised with intramedullary interlocking nails.
RESULTS: Animals ranged in body-weight from 4 to 97 kg. Eighteen (86%) of the fractures were comminuted. Adjunctive stabilisation was used in twelve (57%) fractures and bone grafts in nine (43%) fractures. A rapid return of function w

A distal interlocking system has been developed which is easy to use, carries out an aligning effect on the distal fracture fragment, reduces the exposure to radiation for the surgeon and the patient, and allows for a decrease in operating time. The goal of this study was to develop and test the handling and mechanical properties of two prototype nails in comparison to a conventional interlocking nail concept (Unreamed Femoral Nail system). It was shown that the prototype designs represent an improvement over this system. Both designs were easy to use. The prototype with the asymmetrically off

We report three cases of fracture of the humerus that occurred at the tip of interlocking humeral nails inserted previously for humeral shaft fractures. The fractures occurred at 8, 10, and 26 weeks postoperatively, and all occurred through the distal interlocking screws after the patient sustained a rotational force to the arm. All three patients had further surgery, two subsequently healed their fractures, and one is still in active treatment. Orthopaedists should be aware of this complication when choosing interlocking humeral nails for the treatment of humeral shaft fractures.

Between 1978 and 1994, 130 deaths after hip fractures were investigated at the Institute of Forensic Medicine of the Free University of Berlin. In this period more than 3500 such cases occurred in Berlin. Eight persons were found dead at home, 17 had conservative treatment or died before the planned operation, and 105 had an operation. Hip replacement was carried out in 49, nailing in 17, and other kinds of osteosynthesis (especially using screws) were used in 39 cases. In the surgically treated group, death occurred during the operation in 6 cases (all hip replacements), on the day of operati

Three cases of subtrochanteric fractures in patients with Paget's disease are presented which were treated by closed intramedullary nailing using the Russel-Taylor reconstruction nail. In one case with severe deformity of the femur, diaphyseal osteotomy was required to enable implant insertion. All fractures (including the osteotomy) united. There were no operative or long-term complications and no implant failures. The reconstruction nail is ideal for such cases and has technical and biomechanical advantages over other implants.

We treated 31 proximal femoral shaft fractures, where the trochanteric area was intact, with Grosse-Kempf locked intramedullary nails. There were 19 men and 12 women with a median age of 24 (14-79) years. Sixteen fractures were comminuted and 11 were a component of severe multiple injury. No infections or other serious complications occurred. All the fractures healed within 40 weeks, with 16 results graded as excellent, 7 as good, 7 as fair, and 1 as poor. Six fair results were due to difficulties in aligning the short proximal fragment. One patient with a fair and 1 with a poor result had exc

The postoperative course of 138 patients with a pertrochanteric fracture treated by nailing according to Ender is analysed with regard to the operative morbidity and lethality. As secondary factors associated with morbidity the specific and unspecific rate of complications, the lethality, the perioperative loss of blood and the duration of postoperative hospitalisation are evaluated. Specific complications occurred in 27%, unspecific complications in 50%, and the rate of hospital mortality was 15.2%, 60.9% of patients required a postoperative transfusion to compensate the perioperative blood l

Since the introduction of an interlocking nail in 1972, different systems have been developed for the management of unstable femoral fractures. At the Clinic for Traumatology at the University Hospital of Zürich, the system of Grosse and Kempf was utilized until 1986, after which the AO universal nail system has been and still is used. Between 1981 and 1987, 63 femoral fractures in 62 patients have been treated with interlocking nails. The fractures were produced by a variety of mechanisms of injury, of which 75% were caused by traffic accidents. Associated injuries to other organ systems were

Twenty acute segmental fractures of the femoral shaft were treated with closed intramedullary nailing. The severity of the soft-tissue damage and associated injuries necessitated careful preoperative care. The surgical technique and means of reduction are outlined. Follow-up of at least one year on all patients showed a 100 per cent rate of union with no infections. Major complications included splitting of the greater trochanter during nailing (one case) and an 11-degree valgus deformity (one case). Maximum shortening was 1.5 centimeters. Knee flexion averaged 135 degrees in these acute fract

CASE: We report a rare intraoperative complication during proximal femoral nailing for an intertrochanteric femur fracture in a 54-year-old male. During helical blade preparation, the guidewire migrated intra-articularly and fractured during withdrawal. Retention risked cartilage damage, while retrieval through the blade tract was unsuitable and threatened implant stability. With the nail in situ, safe surgical dislocation was not feasible. The fragment was successfully removed through an direct anterior approach. At 14 months follow-up the patient had returned to his pre injury activity level

INTRODUCTION: Trochanteric femoral fractures (TF) are mostly treated with intramedullary nail osteosynthesis. An inherent but rare complication of the system is the central cut-out of the femoral neck helical blade, i.e., the dislocation through the femoral head towards the acetabulum. After switching from proximal femoral nail antirotation (PFNA) to the trochanteric fixation nail advanced (TFNA) by the manufacturer, this rare type of complication has repeatedly been observed.
MATERIAL AND METHOD: The retrospective single center case-control study was based on a database which includes all TF

This study compared the impact of 2 antirotational implants on cutout risk and mid-term clinical outcomes in elderly patients with intertrochanteric femur fractures. Overall, 107 patients (mean age, 80.10 years) who underwent intramedullary nailing for intertrochanteric femoral fractures between April 2018 and December 2020 were included in this study. Patients were divided into 2 groups: those treated with Antirotator Proximal Femoral Nail (A-PFN; group A) and those treated with Interclaw Proximal Femoral Nail Antirotation (group I). This study evaluated lag screw positioning using the Boswor

PURPOSE: Basicervical femoral neck fractures (BFNFs) are uncommon in the elderly but associated with higher postoperative complications compared to intertrochanteric fractures. Proximal femoral nail antirotation (PFNA) is one of the preferred surgical options for intertrochanteric fracture due to its minimal invasiveness, fewer complications, and better in postoperative functional outcomes. However, limited research addressed the outcomes and the risk factors for implant-related complications after using PFNA.
METHODS: This retrospective study included 149 patients treated with PFNA fixation

This study aimed to investigate the effectiveness of the sliding double flexible intramedullary nail technique (SDFIN) in managing lower extremity long bone fractures and deformities in patients with osteogenesis imperfecta (OI) based on radiographic measurements. This study was conducted involving type III or IV Silence OI patients who underwent treatment with SDFIN for fractures, deformity correction, or previous device failure. The surgical technique involved the insertion of two straight flexible intramedullary nails in each bone segment, with one inserted from the caudal epiphysis and the

BACKGROUND: In the lower extremity, studies have suggested an optimal nail diameter to medullary canal diameter (ND/MCD) ratio to minimize postoperative complications. The goal of this study was to determine whether a correlation exists between the occurrence of complications, angulation, and range of motion and the ratio of the ND/MCD in the upper extremity.
METHODS: A total of 85 radius and ulna fractures treated with flexible intramedullary nails had ND/MCD ratios measured. Random-effects models were developed to determine the association between complications and ND/MCD ratio, angulation

It is important to maintain acceptable reduction and make suitable entry when performing cephalomedullary nailing for subtrochanteric and unstable intertrochanteric fractures; however, this is often challenging due to deforming forces or the unstable nature of the fracture itself. Several percutaneous reduction techniques for fractures have been introduced, but, in some cases, a loss of reduction during the nailing procedure is experienced. To prevent this problem, it is important to keep the proximal fragment stable for the entire nailing procedure. The present study reports favourable outcom
