[Migration inhibition factor in patients with rare pulmonary diseases (author's transl)].

In 98 cases of pulmonary diseases (active and inactive sarcoidosis, idiopathic interstitial lung fibrosis [Hamman-Rich], collagenous diseases, Goodpasture syndrome, alveolar lipoid proteinosis, malignant lymphogranulomatosis and bronchial carcinoma) and in 18 tuberculin and varidase positive, Kveim negative control cases the migration inhibition factor (MIF) was investigated. Migration inhibition of macrophages was induced by the antigens tuberculin, varidase and Kveim's antigen and measured in Mackaness-chambers 24 hours later. Significant migration inhibition by the antigens tuberculin and v

Tuberculous meningitis in children: a retrospective study of 79 patients, with an analysis of prognostic factors.

A study of 79 cases of tuberculous meningitis in children, with an emphasis on prognostic factors, was conducted by means of a review of hospital records and completion of a follow-up questionnaire. The survival rate was 62%. Stage 3 disease (with severe changes in the sensorium and often severe neurologic abnormalities) at the time of admission, age 3 years or less, associated miliary tuberculosis and delay in the initiation of therapy correlated significantly with poor outcome. Severe neurologic complications in the acute phase were also related to poor outcome and severe sequelae. General r

Enduring Challenge of Latent Tuberculosis in Older Nursing Home Residents: A Brief Review.

Tuberculosis (TB) kills almost 4,000 people a day and is competing with human immunodeficiency virus/acquired immune deficiency syndrome (HIV/AIDS) as the most deadly infectious disease in the world. The gold standards of detection and management of latent tuberculosis infection (LTBI) have not been successful in complete eradication of the disease. Current screening modalities of TB include tuberculin skin testing (TST) and/or interferon-γ release assay (IGRA). However, these screening tests have been heavily studied in healthy populations but not in the elderly who are more likely to have mu

Risk, knowledge and preventive measures of smallholder dairy farmers in northern Malawi with regard to zoonotic brucellosis and bovine tuberculosis.

Milk production using local cattle breed-types is an age-old practice in Malawi. Although dairy farming is becoming more common as a result of the increasing population and demand for milk and milk products, there is limited knowledge of the farmers' awareness of zoonotic disease risks, their preventative practices and the disease burden in animals. This study determined dairy farmers' general knowledge of zoonoses, assessed their risks for infection with zoonotic bovine tuberculosis (bTB) and brucellosis, and evaluated farm practices to prevent disease transmission. A questionnaire was drawn

Cost-effectiveness of latent tuberculosis screening before steroid therapy for idiopathic nephrotic syndrome in children.

BACKGROUND: Guidelines differ on screening recommendations for latent tuberculosis infection (LTBI) prior to immunosuppressive therapy. We aimed to determine the most cost-effective LTBI screening strategy before long-term steroid therapy in a child with new-onset idiopathic nephrotic syndrome. STUDY DESIGN: Markov state-transition model. SETTING & POPULATION: 5-year-old boy with new-onset idiopathic nephrotic syndrome. MODEL, PERSPECTIVE, & TIMEFRAME: The Markov model took a societal perspective over a lifetime horizon. INTERVENTION: 3 strategies were compared: universal tuberculin skin t

Physicians' ability to provide initial primary care to an HIV-infected patient.

BACKGROUND: Primary care physicians are providing care for an increasing number of persons infected with the human immunodeficiency virus (HIV). This study assesses the ability of primary care physicians to provide initial care for a patient with recently diagnosed HIV infection. METHODS: A standardized patient (SP) was trained to portray an asymptomatic person with HIV infection seeking a primary care physician. Physicians took a medical history and counseled the SP; their performances were assessed by the SP and through a brief written examination. In addition, physicians distributed questi

PPD skin test positivity and in-hospital mortality across age groups in non-HIV tuberculous meningitis: a retrospective cohort study.

BACKGROUND: Purified Protein Derivative (PPD) skin test reactivity reflects cell-mediated immune response to Mycobacterium tuberculosis. Evidence regarding the prognostic association of PPD reactivity in TBM, particularly its variation across age groups, remains limited. METHODS: We conducted a single-center retrospective cohort study of consecutive non-HIV TBM patients. Firth's penalized logistic regression was used to estimate the association between PPD status and in-hospital mortality using fixed covariate sets selected on clinical grounds. Entropy balancing was used as a complementary co

Primary tubercular dacryocystitis - a case report and review of 18 cases from the literature.

Tuberculosis is an airborne communicable disease primarily affecting lungs. Primary tuberculosis of the lacrimal sac is very rare. A 15-year-old girl presented with bilateral epiphora for 8 months. Examination revealed bilateral nasolacrimal duct obstruction. During dacryocystorhinostomy, bone over lacrimal sac was found partially eroded. Lacrimal sac was found filled with caseous white material. Biopsy from the lacrimal sac revealed tubercular granulomas. Patient improved after anti-tubercular therapy. Abbreviations: PCR: polymerase chain reaction; NAAT: nucleic acid amplification test.

Addressing knowledge gaps and prevention for tuberculosis-infected Indian adults: a vital part of elimination.

BACKGROUND: India plans to eliminate tuberculosis (TB) by 2025, and has identified screening and prevention as key activities. Household contacts (HHCs) of index TB cases are a high-risk population that would benefit from rapid implementation of these strategies. However, best practices for TB prevention and knowledge gaps among HHCs have not been studied. We evaluated TB knowledge and understanding of prevention among tuberculin skin-test (TST) positive HHCs. While extensive information is available in other high-burden settings regarding TB knowledge gaps, identifying how Indian adult contac

An implication of relationship between tuberculosis and primary nephrotic syndrome.

PURPOSE: There have been few observations on the concurrence of tuberculosis (TB) and primary nephrotic syndrome (PNS). We try to define their relationship from multiple aspects. METHODS: Patients with PNS (n = 386), secondary nephrotic syndrome (SNS, n = 60), pneumonia (n = 196), and TB (n = 156) were enrolled. Through a novel evaluation system, the evidence of TB infection (clinical, laboratory, and radiographic evidence) was classified into 5 levels: lack of evidence (level 0), mild-to-moderate evidence (level 1-2), strong evidence (level 3-4). Additionally, whether TB infection was primar

[Tuberculous pericarditis].

Tuberculosis remains a major cause of morbidity and mortality worldwide, and the extrapulmonary presentation represents up to 20% of this disease. The pericardial compromise of this disease has been estimated between 1% and 4% of diagnosed patients. This presentation may have a mortality rate as high as 90% without proper treatment and diagnosis, dropping to 12% with timely diagnosis and treatment. We present the case of a 55 year-old female patient hospitalized for two weeks with constitutional symptoms, intermittent fever, dry cough, pleuritic pain and some symptoms of heart failure. The ima

[Pathomorphism of tuberculous pleurisy in elderly and senile individuals].

The pathomorphism of tuberculous exudative pleurisy in elderly and senile individuals has been investigated in the past 30 years. Comparison of clinical, X-ray, laboratory, morphological, and immunological data in patients of the first (1978-1987) and second (1998-2007) periods has revealed the signs characteristic of the negative pathomorphism of the disease: progressive latency of tuberculous pleurisy and a lower Mantoux reaction, a lower detection rate for epithelioid giant-cell granulomas in the patients' biopsy specimens, a higher rate of pleural tuberculous inflammation, an increased fre

Consequence of prior exposure to environmental mycobacteria on BCG vaccination and diagnosis of tuberculosis infection.

The protective efficacy of Mycobacterium bovis-bacille Calmette Guérin (BCG) against tuberculosis (TB) is variable in both humans and cattle. Exposure to environmental mycobacteria is thought to result in inappropriate priming of host immune responses. To investigate the impact of environmental mycobacteria on BCG efficacy, cattle were infected with M. avium, vaccinated with BCG, challenged with M. bovis and skin tested prior to necropsy. Elevated levels of IFNgamma were evident in M. avium-exposed animals before and after BCG vaccination with a bias towards avian purified protein derivative (
