This study analysed the prevalence, demography, soft- and hard-tissue injury patterns, management and complications of gunshot injuries to the maxillo-facial region in 301 patients treated at Groote Schuur Hospital, Cape Town. The number of patients presenting with maxillo-facial injuries caused by gunshot increased over the 15-year study period. The majority were caused by civilian type low-velocity hand-guns and were purposefully and intentionally inflicted by others. Males in their third decade of life and of low socioeconomic status were most often the victims. The wounding effects of thes

Based on the water-rock-gas coupling test system, the work combined the scanning electron microscope and XTDIC 3D full-field strain measurement system. The Brazilian splitting test was performed on four groups of sandy mudstone specimens under contrast (CO), mash-gas soaking (MS), water-mash gas soaking (WM), and water-soaking (WS) conditions. The tensile strength, deformation failure, and microscopic characteristics of fractures were studied to reveal the deterioration mechanism of the tensile properties of sandy mudstone under water-gas coupling. The results showed that the uniaxial tensile

Question Our practice is seeing children with relatively minor injuries to their elbows, with a history of "swinging" them when their hands are being held to cross the road. Nothing is usually found on a physical examination. I know that this is likely a "pulled elbow." Can we manage this in the clinic setting rather than sending the family to the emergency department? What would be the best course of action in the clinic setting?Answer Pulled elbow, also called nursemaid's elbow, is a radial head subluxation caused by axial traction or a sudden pull of the extended pronated arm, and it is a v

Characterizing properties of the rock matrix that control retention and release of chlorinated solvents is essential in evaluating the extent of contamination and the application of remediation technologies in fractured rock. Core samples from seven closely spaced boreholes in a mudstone subject to trichloroethene (TCE) contamination were analyzed using Mercury Intrusion Porosimetry to investigate porosity and pore size distribution as a function of mudstone characteristics, and depth and lateral extent in the aquifer; organic carbon content was also evaluated to identify the potential for ads

Distal radius fractures are among the most common fractures in adults and require treatment approaches tailored to fracture characteristics and patient needs. The primary objective of this study was to compare the 6-month Patient-Rated Wrist Evaluation (PRWE) score across three treatment modalities—closed reduction and short-arm casting, external fixation, and volar plate fixation. The null hypothesis was that there would be no difference in PRWE outcomes among the three groups. Secondary objectives included evaluating pain intensity, functional disability, grip strength, and wrist range of mo

A retrospective review of 22 patients who sustained snowblower injuries to the hand was performed. There were 17 men and 5 women, ranging in age from 20 to 68 years (average age, 39.7 years). Fifty percent were manual laborers, 25% were unemployed, 15% were office workers, and 10% were not categorized. The dominant hand was involved in 86% of patients. In all patients, injuries occurred during an attempt to unclog manually the snowblower of wet snow. Patients were evaluated initially in the emergency room, where their wounds were irrigated and debrided, subungual hematomas drained, and nail be

BACKGROUND: The incidence of periprosthetic fractures associated with total knee arthroplasty (PpFxK) has been reported to be 0.3 - 5.5%. 40% of all cases are related to revision TKA. The most common localisation is the distal femur. Classification is performed according to Rorabeck (RB). RB I - II fractures are usually treated with locked plating and retrograde intramedullary nailing, whereas RB III fractures are an indication for revision arthroplasty using a hinged endoprosthesis. PpFxK of the patella can be classified according to Goldberg and PpFxK of the proximal tibia can be grouped as

Background Displaced proximal humerus fractures in elderly patients are associated with high rates of fixation failure due to poor bone quality and medial column comminution. Augmentation of locking plate fixation with an intramedullary fibular strut autograft has been proposed to enhance medial support and improve outcomes. This study evaluated early radiological and functional outcomes following open reduction and internal fixation (ORIF) using a proximal humerus internal locking system (PHILOS) plate augmented with a fibular strut autograft. Objectives The objectives of this study are to ev

BACKGROUND: The management of elderly acetabular fractures is complex, with high rates of conversion total hip arthroplasty (THA) after open reduction and internal fixation (ORIF), but potentially higher rates of complications after acute THA.
METHODS: The California Office of Statewide Health Planning and Development database was queried between 2010 and 2017 for all patients aged 60 years or older who sustained a closed, isolated acetabular fracture and underwent ORIF, THA, or a combination. Chi-square tests and Student t tests were used to identify demographic differences between groups. M

BACKGROUND: The optimal surgical technique for humeral shaft fractures remains debated, and earlier syntheses often pooled all plate constructs, disregarding the distinction between open reduction and plate fixation (ORPF) and minimally invasive plate osteosynthesis (MIPO). This network meta-analysis compared ORPF, MIPO, and intramedullary nailing (IMN), focusing on iatrogenic radial nerve injury.
METHODS: We searched PubMed/MEDLINE, Embase, Cochrane CENTRAL, and Scopus from inception through December 2024, with the PubMed search updated through January 2026, following the PRISMA network meta

BACKGROUND: Early diagnosis of acute posttraumatic osteomyelitis (POM) is of vital importance for avoiding devastating complications. Diagnosing POM is difficult due to the lack of a highly specific and sensitive test, such as in myocardial infarct, stroke and intracranial bleeding. Serum inflammatory markers, C-reactive protein (CRP), procalcitonin (PCT), white blood cells (WBC) can support clinical findings but they are not able to differentiate between inflammatory response to infection and the host response to non-infection insult with high specificity and sensitivity.
AIM: The objectives

BACKGROUND Proximal humerus open reduction and internal fixation (ORIF) is usually performed under interscalene brachial plexus block (ISB) or general anesthesia (GA). ISB causes ipsilateral phrenic nerve paresis in nearly all patients, and GA may raise the risk of postoperative neurocognitive disorders in elderly patients with cognitive impairment. We report the feasibility of a low-volume regional technique, used as the sole regional anesthetic with sedation, in a patient in whom GA avoidance was desirable. CASE REPORT A 68-year-old woman (American Society of Anesthesiologists physical statu

PURPOSE: Inpatient glucocorticoid treatment has been associated with increased postoperative complications after total hip arthroplasty (THA). However, evidence specific to osteonecrosis of the femoral head (ONFH) remains limited and inconsistent. This study aimed to evaluate the association between inpatient glucocorticoid treatment and early postoperative complications after THA for ONFH using a nationwide database.
METHODS: This retrospective cohort study used the Japanese Diagnosis Procedure Combination database from 2011 to 2023. Patients undergoing THA for ONFH were identified and categ
