Acting as If: Weak Practical Transcendence and the Rationality of Posthumous Care.

RATIONALE AND AIM: Posthumous care-honouring advance directives, keeping deathbed promises, defending deceased persons' reputations-is central to clinical ethics, yet its philosophical foundations remain contested. This paper asks whether such care can be rationally grounded without presupposing either a robust transcendent ontology (the view that persons persist after death as welfare subjects) or the Epicurean conclusion that death renders all posthumous concern irrational. METHODS: The paper proceeds by philosophical argumentation and case analysis. It first reconstructs and critically eva

Cultural and traditional influences on cervical cancer screening uptake in Nigeria and other West African countries.

BACKGROUND: The review examined cultural and traditional factors that hinder cervical cancer screening among women in Nigeria and similar low and middle income areas in Africa. Cervical cancer causes high mortality; however, early detection remains suboptimal, largely due to deeply rooted cultural beliefs and social norms. METHODS: This systematic review, guided by PRISMA 2020, examined these influences across Nigeria and comparable West African contexts, searching PubMed, Scopus, Web of Science, AJOL, and Google Scholar for studies published between January 1, 2000, and March 31, 2025. Meditation, free-will beliefs, and sentencing attitudes: an exploratory pilot study in India.

Criminal sentencing attitudes can reflect psychological dispositions concerning agency, hierarchy, blame, and rehabilitation. This exploratory pilot examined whether established contemplative practice is associated with a less punitive profile and whether legal professionals show differences following a four-week Anapana programme. Ninety adults participated in three cohorts: experienced meditators (n = 30), meditation-naïve controls (n = 30), and legal professionals or advanced law trainees assessed before and after the programme (n = 30). The study assessed 21 outcomes: 15 core outcomes cove

Supporting families and building relationships: Evaluation of a home and sleep safety equipment scheme for impoverished communities.

OBJECTIVES: Sleep safety and home safety resources allow families to care for babies and young children, preventing injuries and child death, adverse outcomes that are strongly linked to poverty and social deprivation. Parenthood involves unexpected costs and greater levels of unmet need for safety resources occur in low-income families. We evaluate a local authority scheme which enabled professionals in County Durham to apply for necessary safety equipment on behalf of eligible families. STUDY DESIGN: A holistic review of the first year of the operation of the Start For Life Fund (SFLF) sche

Structural determinants in quality of care for older adults: a nursing perspective on medical services - qualitative findings from focus group discussions in Germany.

BACKGROUND: Aging populations, multimorbidity, and rapid medical progress have expanded treatment options in old age, but also intensified ethical and organizational challenges in everyday care. Little is known about how nurses, as key frontline professionals, experience these structural developments specifically in the context of medical and nursing care for older adults in Germany. Therefore, this study aimed to examine how nursing staff perceive structural conditions in the German healthcare system and their effects on medical and nursing care for older patients. METHODS: This study employ

When the profession itself is contested: Transformative learning and divergent identity pathways among first-year medical students during a macro-social crisis.

INTRODUCTION: Professional identity formation (PIF) research in medical education has largely examined micro-level dilemmas within relatively stable training environments. Less is known about how macro-social crises destabilizing the profession's public legitimacy shape early PIF. This study explored how first-year medical students narrated the impact of the 2024-2025 doctor-government conflict in South Korea on their professional identity development, using Transformative Learning Theory (TLT) as an interpretive lens. METHODS: We conducted a theory-informed qualitative study using written re

Hidden distress: a qualitative study of sociocultural barriers to mental health help-seeking among young people in Zimbabwe.

BACKGROUND: Sociocultural barriers are powerful forces that may shape how young people experience, interpret, and respond to psychological distress in Zimbabwe. For many youth (aged 15-24 years), symptoms of common mental disorders (CMDs) are navigated within social environments where disclosure risks labelling, social exclusion, and moral judgment. This stigma is rarely experienced in isolation; it intersects with pressures to protect family reputation, distrust in formal and informal support systems, and gendered expectations governing emotional expression. While existing research has docume

Embracing Medicines Development as a Profession.

Medicines development has dramatically transformed in the preceding decades. It has evolved from a task undertaken by a small team to a complex series of activities, involving several functions and qualified professionals, across multiple, interrelated, scientific disciplines, worldwide. Conceptualized as a medical specialty, concerned with the research, development, and monitoring of medicines, and spearheaded largely by pharmaceutical physicians, the discipline has extended to embrace non-medically qualified scientists progressively taking on traditional roles within the medicines developmen

"Unintended pregnancy and attitudes towards sexual and reproductive health among young people in Benguela Province, Angola".

BACKGROUND: Young women (YW) in Angola face multiple, intersecting challenges related to sexual and reproductive health (SRH), including high rates of early and unintended pregnancy, limited access to contraception, gender-based violence (GBV), and unsafe abortion. These issues are shaped by structural inequalities, restrictive laws, stigma, and inadequate youth-friendly services. Despite the urgency, qualitative research exploring youth perspectives on SRH in Angola remains scarce. METHODS: This qualitative study was conducted in May 2023 in Benguela Province, Angola, as part of the formativ

Exploring Influence of Spiritual Well-being on Caregiver Burden in Head-and-Neck Malignancy: A Cross-sectional Study.

OBJECTIVES: Taking care of a patient with head-and-neck malignancy (HNM) entails enormous physical, emotional, cognitive and moral challenges. Caregivers frequently turn to spiritual practices such as prayer, offering and meditation to enhance inner strength and coping ability. Assessment of caregiver burden (CGB) would remain incomplete without considering spiritual well-being (SWB). We set out to assess CGB and SWB in caregivers of patients suffering from HNMs. MATERIALS AND METHODS: This cross-sectional study was carried out in a medical college-associated tertiary care centre after approv

A data quality study investigating the fieldworker's impact on the reporting discrepancies of pre-marital sexual relationships among unmarried men and women in India.

INTRODUCTION: Generating unbiased and accurate assessments of sexual behavior among unmarried individuals is a major challenge, as it is governed by many social, moral, legal conventions and restrictions. These estimates are particularly susceptible to Fieldworker-induced social desirability bias. Accurate estimation of pre-marital sexual behavior is crucial for evaluating many aspects of sexual and reproductive health. The National Family Health Survey (NFHS) is the only reliable source of information on pre-marital sexual behavior in India, especially in the absence of any other comparative

"Struck down by cancer with no old life to fall back on" a clinical study of illness experiences among Norwegian adolescent and young adult cancer survivors investigating the ethical implications of their illness narratives.

BACKGROUND: Cancer is a leading cause of death among people 15-24 years of age. Increasing numbers of cancer patients survive. Extensive cancer therapy may cause wide-ranging somatic and psychosocial challenges in the lives of the survivors. Research indicates adolescent and young adult cancer (AYA) survivors need to be seen as a distinctive group of survivors having unique health care needs. The existing literature suggests the need for specific follow-up care programs addressing the challenges of AYAs and providing them access to specialized after care, as well as a need to explore AYA cance

Antenatal Assessment of the Prognosis of Congenital Diaphragmatic Hernia: Ethical Considerations and Impact for the Management.

Congenital diaphragmatic hernia (CDH) is associated with abnormal pulmonary development, which is responsible for pulmonary hypoplasia with structural and functional abnormalities in pulmonary circulation, leading to the failure of the cardiorespiratory adaptation at birth. Despite improvement in treatment options and advances in neonatal care, mortality remains high, at close to 15 to 30%. Several risk factors of mortality and morbidities have been validated in fetuses with CDH. Antenatal assessment of lung volume is a reliable way to predict the severity of CDH. The two most commonly used me

The Khoekhoegowab Personality Inventory: The Comparative Validity of a Locally Derived Measure of Traits.

Objective: This study explores a personality inventory derived from the results of an indigenous lexical study of personality. From the 272 most commonly used personality descriptors in Khoekhoegowab, the most-spoken of extant Khoesan click languages of southern Africa, an 11-factor model of personality-trait structure was identified. Here, the Khoekhoegowab Personality Inventory (KPI) was created based on those results. Its psychometric properties, the convergent and divergent validity of its scales, and its incremental validity over Big Five and Six traits for predicting physical and mental

Short-Term Efficacy and Safety of Different Mechanical Hemodynamic Support Devices for Cardiogenic Shock or High-Risk Pci: a Network Meta-Analysis of Thirty-Seven Trials.

BACKGROUND: With more advanced mechanical hemodynamic support for patients with cardiogenic shock (CS) or high-risk percutaneous coronary intervention (HS-PCI), the morality rate is now significantly lower than before. While previous studies showed that intra-aortic balloon pumping (IABP) did not reduce the risk of mortality in patients with CS compared to conservative treatment, the efficacy in other mechanical circulatory support (MCS) trials was inconsistent. OBJECTIVE: We conducted this network meta-analysis to assess the short-term efficacy and safety of different intervention measures f

Contemporary Clinical and Coronary Anatomic Risk Model for 30-Day Mortality After Percutaneous Coronary Intervention.

BACKGROUND: Percutaneous coronary intervention (PCI) procedures are increasing in clinical and anatomic complexity, likely increasing the calculated risk of mortality. There is need for a real-time risk prediction tool that includes clinical and coronary anatomic information that is integrated into the electronic medical record system. METHODS: We assessed 70 503 PCIs performed in 73 Veterans Affairs hospitals from 2008 to 2019. We used regression and machine-learning strategies to develop a prediction model for 30-day mortality following PCI. We assessed model performance with and without in

The Neural Mechanism of the Social Framing Effect: Evidence from fMRI and tDCS Studies.

As an important cognitive bias, the framing effect shows that our decision preferences are sensitive to the verbal description (i.e., frame) of options. This study focuses on the neural underpinnings of the social framing effect, which is based on decision-making regarding other people. A novel paradigm was used in which participants made a trade-off between economic benefits and the feelings of others. This decision was described as either a "harm" to, or "not helping," other persons in two conditions (Harm frame vs Help frame). Both human males and females were recruited. Participants behave

Intravenous Thrombolysis before Thrombectomy may Increase the Incidence of Intracranial Hemorrhage inTreating Carotid T Occlusion.

BACKGROUND AND PURPOSE: Current evidence does not agree on the merits of direct and bridging thrombectomy. This study aimed to compare the safety and efficacy of direct thrombectomy (DT) and bridging thrombectomy (BT) in treating patients with acute ischaemic stroke due to carotid T occlusion. METHODS: Patients with stroke due to carotid T occlusion who were treated with DT or BT were retrospectively collected from four advanced stroke centres. Baseline characteristics and clinical outcomes were compared between the groups. Successful recanalization was defined by a modified thrombolysis in c
