Guidelines for preventing the transmission of Mycobacterium tuberculosis in health-care settings, 2005.

In 1994, CDC published the Guidelines for Preventing the Transmission of Mycobacterium tuberculosis in HealthCare Facilities, 1994. The guidelines were issued in response to 1) a resurgence of tuberculosis (TB) disease that occurred in the United States in the mid-1980s and early 1990s, 2) the documentation of several high-profile health-care--associated (previously termed "nosocomial") outbreaks related to an increase in the prevalence of TB disease and human immunodeficiency virus (HIV) coinfection, 3) lapses in infection control practices, 4) delays in the diagnosis and treatment of persons

Outbreak of tuberculosis among homeless persons coinfected with human immunodeficiency virus.

We investigated a cluster of patients with tuberculosis (TB) in North Carolina and determined the extent of transmission of 1 strain of Mycobacterium tuberculosis. A retrospective cohort study was conducted. Homeless shelter attendance and medical records for 1999 and 2000 were reviewed. The period of exposure to M. tuberculosis was determined, and shelter residents were offered TB screening. DNA fingerprinting was performed on 72 M. tuberculosis isolates. In addition to the initial index cluster of 9 patients, another 16 patients were identified. Isolates of M. tuberculosis from all 25 patien

Treatment of latent tuberculosis infection in contacts of new tuberculosis cases in the United States.

BACKGROUND: Few data are available describing treatment completion rates among recently infected contacts of tuberculosis (TB) cases, a group at high risk for development of active TB. METHODS: Health department records were reviewed for all contacts of 360 culture-positive pulmonary TB cases reported from five health departments in the United States in 1996. RESULTS: Of 2,267 contacts who completed screening, 630 (28%) had newly documented positive skin tests (121 with skin test conversion). Treatment of latent TB infection was documented to have been recommended for 447 (71%). Among these,

[The immunological aspects in the HIV/AIDS-tuberculosis association in children].

HIV-infected subjects are submitted to various immunity disorders, the function of immune competent cells disturbing, the most important one, resulting in severe opportunistic infections. The progressive depletion of CD4 subset of lymphocytes in HIV-infected persons is the most significant one. When HIV infection is associated to tuberculosis there is a more accelerated immune deterioration. The immunological investigation consisted in the determination of cell-mediated immune mechanisms (PPD skin testing, lymphocyte count, subsets of lymphocytes) as well as those humorally mediated (Ig classe

Tuberculosis case-finding in Western Australia.

The records of a total of 497 patients notified to have active tuberculosis (TB) in Western Australia from 1986 to 1990 inclusive have been reviewed in relation to case-finding. Of these, 276 (55.5%) were Asian migrants, 92 (18.5%) Europeans and 110 (22.1%) Australians. The disease was confirmed bacteriologically in 247 (49.7%). The most common mode of diagnosis was symptomatic presentation (39.8%), followed by migrant health surveillance (36.8%), incidental finding (7.5%), regular follow-up (3.8%) and pre-employment chest radiography (1.8%). Contact examination in the non-Asians accounted for

Noninvasive testing of asymptomatic bilateral hilar adenopathy.

The diagnostic strategy for asymptomatic patients with persistent bilateral bilar adenopathy often involves invasive procedures. The authors used Bayesian analysis to: 1) estimate the relative prevalences of diseases causing bilateral bilar adenopathy; 2) assess changes in the prevalence of disease by race, the presence of other clinical symptoms, and geography; and 3) determine the value of relevant noninvasive tests, including the angiotensin-converting enzyme (ACE) assay, gallium scan, and purified protein derivative (PPD), in order to assess when a strategy of watchful waiting is appropria

Use of the QuantiFERON®-TB Gold in Tube test for screening TB contacts and predictive value for active TB.

BACKGROUND: This study estimated the incidence of progression to active tuberculosis (TB) among contacts categorized by QuantiFERON(®)-TB Gold in Tube (QFT-GIT) test results and investigated other risk factors related to progression to TB. METHODS: Contacts of patients with TB were tested using QFT-GIT and were followed up every 6 months at public health centers to detect clinical progression to TB. RESULTS: Analysis of a retrospective cohort revealed that, of the 625 contacts, 168 were QFT-GIT positive and 457 were negative. Of these, 10 (6%) QFT-GIT-positive and two (0.4%) QFT-GIT-negative

Evidence of mycobacteriaemias and mycobacterial co-infections uncovered in cattle at slaughter using a novel phage-based PhMS-qPCR assay for viable Mycobacterium bovis and Mycobacterium avium subsp. paratuberculosis.

Accurate diagnosis of Bovine Tuberculosis (bTB) in cattle is important for success of eradication programmes, but the current reliance on the single intradermal comparative cervical tuberculin (SICCT) test is not achieving disease eradication in all jurisdictions, including Northern Ireland (NI). In this study, a novel Phagomagnetic separation (PhMS)-qPCR assay to rapidly detect viable Mycobacterium bovis in bovine blood samples was evaluated. A total of 149 heart blood samples were collected from cattle at exsanguination point of the slaughter line at a NI abattoir between June and August 202

Low secondary attack rate after prolonged exposure to sputum smear positive miliary tuberculosis in a neonatal unit.

BACKGROUND: Several neonatal intensive care units (NICU) have reported exposure to sputum smear positive tuberculosis (TB). NICE guidelines give support regarding investigation and treatment intervention, but not for contact definitions. Data regarding the reliability of any interferon gamma release assay (IGRA) in infants as a screening test for TB infection is scarce. We report an investigation and management strategy and evaluated the viability of IGRA (T-Spot) in infants and its concordance to the tuberculin skin test (TST). METHODS: We performed an outbreak investigation of incident TB i

Combined analysis of whole blood interferon gamma release assay and complete blood count analysis for rapid discrimination of active tuberculosis and latent tuberculosis infection.

Tuberculosis (TB), which is caused by Mycobacterium tuberculosis (MTB), is a serious infectious disease with high infection and mortality rates and is a public health problem around the world. According to the World Health Organization (WHO) report, one-third of the world's population is latently infected with MTB, and 5 to 10% of those with latent TB infection (LTBI) have the potential to develop active TB once in their lifetime. Therefore, TB management for promptly distinguishing LTBI from active TB and for proper treatment is important. LTBI is currently diagnosed using the tuberculin skin

High prevalence of infection and low incidence of disease in child contacts of patients with drug-resistant tuberculosis: a prospective cohort study.

OBJECTIVE: We aimed to measure the prevalence and incidence of latent tuberculosis infection (LTBI) and tuberculosis (TB) disease in children in close contact with patients with drug-resistant TB (DR-TB) in a country with high DR-TB prevalence. DESIGN AND SETTING: This is a prospective cohort study of paediatric contacts of adult patients with pulmonary DR-TB in Armenia. Children were screened using tuberculin skin test, interferon-gamma release assay and chest X-ray at the initial consultation, and were reassessed every 3-6 months for a period of 24 months. Children did not receive preventiv

Cell-mediated immune response in Indian kala-azar and post-kala-azar dermal leishmaniasis.

Cell-mediated immune (CMI) response in 16 Indian kala-azar (KA) and 12 post-kala-azar dermal leishmaniasis (PKADL) patients was studied in detail by in vitro lymphocyte transformation experiments and by in vivo skin testing. Peripheral blood lymphocytes of active KA patients failed to be stimulated by leishmania antigen. On the other hand, lymphocytes from a majority of the active KA patients could be stimulated by phytohemagglutinin. Active KA patients also failed to show delayed type hypersensitivity reaction to leishmanin, although 72% of them showed delayed type hypersensitivity to a purif
