Squamous cell carcinoma of the nipple following radiation therapy for ductal carcinoma in situ: a case report.

INTRODUCTION: Radiation-induced nonmelanoma skin cancer was first reported seven years after the discovery of X-rays, but has received relatively little consideration in the literature. Specifically, nonmelanoma skin cancer after conservative surgery and radiation for early stage breast cancer has not been well studied. We report the case of a woman who developed squamous cell carcinoma of the nipple nine years after conservative surgery and radiation for ductal carcinoma in situ of the ipsilateral breast. We also review the relevant literature available to date. CASE PRESENTATION: A 66-year-

A Case of Taxane-Induced Scleroderma Presenting in a Unilateral Upper Limb and a Literature Review.

A 64-year-old woman with progressive thickening of the skin on her left upper limb was referred to our clinic. She had previously undergone partial mastectomy and axillary lymph node dissection for left-sided breast cancer, followed by postoperative radiation therapy and chemotherapy with docetaxel. She exhibited edema and sclerosis of the left arm and contracture limited to the fingers of the left hand, while her right arm and lower extremities remained unaffected. There were no signs of Raynaud's phenomenon or periungual capillary abnormalities suggestive of systemic sclerosis. Skin biopsy f

Radiation-induced malignant and atypical peripheral nerve sheath tumors.

The reported peripheral nerve complications of therapeutic irradiation in humans include brachial and lumbar plexus fibrosis and cranial and peripheral nerve atrophy. We have encountered 9 patients with malignant (7) and atypical (2) peripheral nerve tumors occurring in an irradiated site suggesting that such tumors represent another delayed effect of radiation treatment on peripheral nerve. The neoplasms appeared 4 to 41 years following radio-therapy, at a time when the patients (6 females and 3 males) were 16 to 70 years old and cured of their original tumors. Eight patients had received irr

Fernblock (Polypodium leucotomos Extract): Molecular Mechanisms and Pleiotropic Effects in Light-Related Skin Conditions, Photoaging and Skin Cancers, a Review.

Healthier life styles include increased outdoors time practicing sports and walking. This means increased exposure to the sun, leading to higher risk of sunburn, photoaging and skin cancer. In addition to topical barrier products, oral supplementations of various botanicals endowed with antioxidant activity are emerging as novel method of photoprotection. Polypodium leucotomos extract (PL, commercial name Fernblock(®), IFC Group, Spain) is a powerful antioxidant due to its high content of phenolic compounds. PL is administered orally, with proven safety, and it can also be used topically. Its

Disorders of endocrine function following cancer therapies.

There is a growing body of literature detailing the endocrine consequences of cancer therapy. Certain conclusions can be drawn from the data presented. Patients who have received incidental hypothalamic--pituitary gland irradiation need to be followed carefully with serial dynamic hormonal evaluations, as they are at high risk of developing growth hormone and prolactin abnormalities and can develop other pituitary tropic hormone deficiencies as well. Children especially should be monitored closely as GH deficiency can be corrected if detected early. Patients who have received radiation to the

Live imaging of radiation-induced apoptosis by yolk injection of Acridine Orange in the developing optic tectum of medaka.

To observe the sequential radiation-induced apoptosis in a living embryo, we injected Acridine Orange (AO) solution into the yolk of embryo and visualized radiation-induced apoptosis in developing optic tectum (OT). Medaka embryos at stage 28, when neural cells proliferate rapidly in the OT, were irradiated with 5 Gy X-rays which is a non-lethal dose for irradiated embryos at hatching. The irradiated embryos hatched normally without morphological abnormalities in their brains, even though a large number of apoptotic cells were induced transiently in OT. By yolk injection, apoptotic cells in OT

Pathophysiology of radiation-induced growth hormone deficiency: efficacy and safety of GH replacement.

Radiation-induced growth hormone deficiency (GHD) is primarily due to hypothalamic damage. GH secretion by the pituitary may be affected either secondary to some degree of quantitative deprivation of hypothalamic input or, if the radiation dose is high enough, by direct pituitary damage. As a consequence, the neurosecretory profile of GH secretion in an irradiated patient remains pulsatile and qualitatively intact. The frequency of pulse generation is unaffected, but the amplitude of the GH pulses is markedly reduced. Over the last 25 years, the final heights achieved by children receiving GH

Low-dose-rate radiation exposure leads to testicular damage with decreases in DNMT1 and HDAC1 in the murine testis.

This study examined the effects of continuous low-dose-rate radiation exposure (3.49 mGy/h) of gamma rays on mice testicles. C57BL/6 mice were divided into sham and radiation groups (n = 8 each), and were exposed to either sham irradiation or 2 Gy for 21 days, 0.2 Gy for 2 days, or 0.02 Gy for 6 h of low-dose-rate irradiation. Testicular weight, seminiferous tubular diameter, and seminiferous epithelial depth were significantly decreased in the mice irradiated with 2 Gy at 1 and 9 days after exposure. Moreover, the low-dose-rate radiation exposure induced an increase in malondialdehyde levels,

Radiation-induced changes in the central nervous system and head and neck.

Radiation therapy of the brain, neck, and spine can be associated with radiation-induced changes that are increasingly evident radiologically. These changes affect the central nervous system (focal necrosis, diffuse white matter injury, atrophy, mineralizing microangiopathy, telangiectasia, optic neuropathy, large vessel vasculopathy), bone (fatty replacement of marrow, osteoradionecrosis, sinus inflammation), and superficial and deep soft tissues and include neoplasia. Focal necrosis, which most commonly occurs at the treatment site, is seen at computed tomography (CT) and magnetic resonance

An animal model to study toxicity of central nervous system therapy for childhood acute lymphoblastic leukemia: effects on behavior.

Central nervous system prophylactic therapy used in the treatment of acute lymphoblastic leukemia can reduce intelligence quotient scores and impair memory and attention in children. Cranial irradiation, intrathecal methotrexate, and steroids are commonly utilized in acute lymphoblastic leukemia therapy. How they induce neurotoxicity is unknown. This study employs an animal model to explore the induction of neurotoxicity. Male and female Sprague-Dawley rats at 17 and 18 days of age were administered 18 mg/kg prednisolone, 2 mg/kg methotrexate, and 1000 cGy cranial irradiation. Another 18-day-o

Genetic effects of radiotherapy for childhood cancer.

Radiation-induced heritable diseases have not been demonstrated in humans and estimates of genetic risks for protection purposes are based on mouse experiments. The most comprehensive epidemiologic study is of the Japanese atomic bomb survivors and their children, which found little evidence for inherited defects attributable to parental radiation. Studies of workers exposed to occupational radiation or of populations exposed to environmental radiation appear too small and exposures too low to convincingly detect inherited genetic damage. In contrast, survivors of childhood cancer form the lar

Changes in mammalian sperm morphology after X-ray and chemical exposures.

Sperm head morphology in mammals provides a unique approach to quantiating the effects of environmental agents on the germ cells. In unperturbed male mice, the sperm of each genotype can be reproducibly characterized by the shape of the head, the overall percent of sperm with head-shape abnormalities and the types of shape abnormalities seen. Genetic studies show that sperm shape is highly heritable, and that the fraction of abnormal sperm is controlled by a multitude of autosomal factors plus probably involvement of the sex chromosomes.--Exposure to ionizing radiation or certain chemical agen

Immunomodulatory Effects of Histone Variant H2A.J in Ionizing Radiation Dermatitis.

PURPOSE: Histone variant H2A.J is associated with premature senescence after ionizing radiation (IR) and modulates senescence-associated secretory phenotype (SASP). Using constitutive H2A.J knock-out mice, the role of H2A.J was investigated in radiation dermatitis. METHODS AND MATERIALS: H2A.J wild-type (WT) and knock-out (KO) mice were exposed to moderate or high IR doses (≤20 Gy, skinfold IR). Radiation-induced skin reactions were investigated up to 2 weeks post-IR at macroscopic and microscopic levels. H2A.J and other senescence markers, as well as DNA damage and proliferation markers, wer

Chronic exposure of Sprague-Dawley rats to 20 kHz triangular magnetic fields.

PURPOSE: As a continuing study of 20 kHz triangular magnetic fields (MF) [Lee et al. 2006 ], we investigated the chronic toxicity and possible health effects of exposure to 20 kHz MF at the flux density of 30 microT, which is the limit standard for the occupational population in South Korea, with the use of Sprague-Dawley rats. MATERIALS AND METHODS: Rats were exposed to 20 kHz triangular MF at 30 microT Root Mean Square for 8 h/day for 18 months. Body and organ weights were measured and urinalysis, hematological and blood biochemistry analyses were performed in individual animals. Histopatho

Microfibril-Associated Protein 5 Contributes to the Elastic Fiber Abnormalities in Aged Skin.

Elastic fibers, which contribute to the flexibility of tissues such as the skin, alveoli, and arteries, have a long half-life and are not regenerated once formed during the fetal stage. Consequently, the degradation of elastic fibers due to aging or inflammation can significantly impact tissue function. In the dermis, degeneration of elastic fibers is characterized by degradation in photoaging, driven by UV radiation, and structural abnormalities of elastic fibers in intrinsic aging. However, the mechanisms driving the abnormalities associated with intrinsic aging remain incomplete. This study

Relapsing polychondritis after treatment with PD-1 blockade.

Nivolumab, a programmed death 1 blockade drug, is used in various types of cancers and can cause a unique immune-related adverse event (irAE). Relapsing polychondritis (RP) is a rare autoimmune disease that mainly involves inflammation of the auricle, nose and airway cartilage. A 72-year-old man with mandibular cancer received nivolumab after surgery for the primary lesion and radiation therapy for lung metastases. He then developed radiation pneumonitis, and prednisolone (PSL) was started. During the tapering of PSL, he developed exertional dyspnea and cough. The condition of mandibular cance

Membrane receptor signaling and control of DNA repair after exposure to ionizing radiation.

Accumulated evidence indicates that activation of erbB family of receptors, when mutated or over-expressed, mediates chemo- and radiotherapy resistance. In this context signaling pathways down-stream of epidermal growth factor receptor (EGFR), when abnormally activated, invoke cell survival mechanisms, which leads to resistance against radiation. In several reports it has been demonstrated that molecular targeting of EGFR signaling enhances the cytotoxic effects of radiotherapy. The radiosensitizing effects of EGFR antagonists correlate with a suppression of the ability of tumor cells to repai
