Reconstruction of parotidectomy and lateral skull base defects.

PURPOSE OF REVIEW: The purpose of this review is to summarize the options for reconstruction of parotid and lateral skull base defects based on the size and functional requirements of the defect. RECENT FINDINGS: Free grafts with dermal fat or acellular human dermis, superficial musculoaponeurotic system flaps, and sternocleidomastoid flaps have been successful in preventing Frey's syndrome and restoring facial contour defects after superficial and total parotidectomy. Lateral skull base resections often require reconstruction with pedicled or free flaps to restore extensive soft tissue and d

Acellular Dermal Matrix-Associated Complications in Implant-Based Breast Reconstruction: A Multicenter, Prospective, Randomized Controlled Clinical Trial Comparing Two Human Tissues.

BACKGROUND: Implant-based breast reconstruction accounts for the vast majority of breast reconstruction procedures and is commonly performed with human acellular dermal matrix. There is no consensus as to the optimal human acellular dermal matrix preparation, and high-quality evidence concerning comparative effectiveness is lacking. This study is the first prospective, multicenter, randomized controlled clinical trial to compare human acellular dermal matrix-related complications of the two most commonly used human acellular dermal matrices in implant-based breast reconstruction. The authors h

Use of acellular dermal matrix combined with a component separation technique for repair of contaminated large ventral hernias: a possible ideal solution for this clinical challenge.

Repair of large contaminated ventral hernias is always challenging because of massive loss of muscular and fascial tissues, high risk of surgical infection and recurrence, and contraindication to use of a permanent prosthesis. This study reviewed retrospectively data of 35 patients with contaminated large ventral hernias who received repair using acellular dermal matrix combined with a component separation technique from 2009 to 2011. Twenty-one males and 14 females were identified with a mean age of 45.5 ± 12.5 years and a mean body mass index of 22.5 ± 5.8 kg/m(2). Simultaneously, nine patie

Safety and histologic characteristics of aseptic and sterile acellular dermal matrices in irradiated implant-based breast reconstruction.

BACKGROUND: Acellular dermal matrices (ADMs) are widely used in implant-based breast reconstruction to enhance esthetic outcomes and reduce capsular contracture. However, concerns persist regarding their performance in patients undergoing postmastectomy radiation therapy (PMRT). The impact of ADM processing method-aseptic versus sterile-on clinical outcomes and postirradiation tissue characteristics remains incompletely defined. OBJECTIVES: To compare clinical outcomes, patient-reported satisfaction, and histologic characteristics of aseptic and sterile ADMs in implant-based breast reconstruc

Decellularized dermal scaffold biofunctionalized with quercetin nanoparticles enhances angiogenesis and attenuates inflammation in type 2 diabetic wounds.

Chronic diabetic wounds are characterized by excessive inflammation, impaired angiogenesis, and oxidative stress, leading to delayed tissue regeneration and poor healing outcomes. In this study, a biofunctionalized decellularized dermal scaffold incorporating quercetin nanoparticles (SCS-QNP) was developed to enhance the biological performance of the scaffold for type 2 diabetic wound repair. The scaffold was fabricated and systematically characterized in terms of its structural integrity and physicochemical properties. Its regenerative potential was evaluated in a streptozotocin-induced diabe

Bioengineered dermal matrix (Integra®) reduces donor site morbidity in total phallic construction with radial artery forearm free-flap.

Total phallic construction with radial artery forearm free-flap (RAFFF) is widely regarded as the gold standard approach for phalloplasty. However, donor-site morbidity remains a significant concern, which is typically managed by using a full-thickness skin graft (FTSG) on the forearm. Split thickness skin grafts (STSG) have been proposed as an alternative, along with the use of an acellular dermal matrix substitute. A retrospective comparative analysis was performed to assess the differences in operative, functional and cosmetic outcomes between FTSG (Group A) and the combination of acellular

Immediate Prepectoral Implant-Based Breast Reconstruction after J-Pattern Skin-Reducing Mastectomy.

SUMMARY: Skin-reducing mastectomy for single-stage reconstruction is an oncologically safe procedure used in women with large and ptotic breasts. This study describes a new technique, the J-pattern skin-reducing breast reconstruction with prepectoral implant and acellular dermal matrix (Braxon fast), which is indicated for patients with large and ptotic breasts who would benefit from a breast reduction and need a mastectomy for curative or prophylactic purposes. The authors present a case series of 35 breasts of 19 women who underwent this procedure from January of 2021 through December of 202

Prevention of postoperative adhesions after flexor tendon repair with acellular dermal matrix in Zones III, IV, and V of the hand: A randomized controlled (CONSORT-compliant) trial.

BACKGROUND: Various techniques have been attempted for preventing postoperative flexor tendon adhesion, such as modification of suture technique, pharmacological agents, and mechanical barriers. However, there is no evidence of the efficacy of these methods in clinical settings. In this study, we present the long-term outcomes of a randomized prospective study in which acellular dermal matrix (ADM) was used to prevent postoperative adhesions after tendon injury in the hand. METHODS: From January 2017 to January 2020, all patients with an acute single flexor tendon injury in hand Zones III, IV

Prepectoral versus Subpectoral Tissue Expander Breast Reconstruction: A Historically Controlled, Propensity Score-Matched Comparison of Perioperative Outcomes.

BACKGROUND: The rising popularity of prepectoral tissue expander placement with acellular dermal matrices in immediate breast reconstruction has prompted many studies on the safety of this technique. However, a comprehensive propensity-matched, historically controlled trial comparing perioperative outcomes following prepectoral versus partial subpectoral (dual-plane) placement of tissue expanders is lacking. METHODS: Retrospective propensity-matched cohort analysis was performed on all patients of two senior reconstructive surgeons who underwent bilateral tissue expander placement following a
