Low-Intensity Transcranial Ultrasound Stimulation: Mechanisms of Action and Rationale for Future Applications in Movement Disorders.

Low-intensity transcranial ultrasound stimulation (TUS) is a novel non-invasive brain stimulation technique that uses acoustic energy to induce changes in neuronal activity. However, although low-intensity TUS is a promising neuromodulation tool, it has been poorly studied as compared to other methods, i.e., transcranial magnetic and electrical stimulation. In this article, we first focus on experimental studies in animals and humans aimed at explaining its mechanisms of action. We then highlight possible applications of TUS in movement disorders, particularly in patients with parkinsonism, dy

A flexible photoacoustic retinal prosthesis.

Retinal degenerative diseases of photoreceptors are a leading cause of blindness with no effective treatment. Retinal prostheses aim to restore sight by stimulating residual retinal cells. Here, we present a photoacoustic retinal stimulation technology. We designed a polydimethylsiloxane and carbon-based flexible film that converts near-infrared laser pulses into a localized acoustic field with 56-μm lateral resolution, allowing precise stimulation of mechanosensitive retinal cells. This photoacoustic stimulation robustly and locally modulated retinal ganglion cell activity in both wild-type a…

Low Intensity Noise Exposure Enhanced Auditory Loudness and Temporal Processing by Increasing Excitability of DCN.

Sound stimulation is generally used for tinnitus and hyperacusis treatment. Recent studies found that long-term noise exposure can change synaptic and firing properties in the central auditory system, which will be detected by the acoustic startle reflex. However, the perceptual consequences of long-term low-intensity sound exposure are indistinct. This study will detect the effects of moderate-level noise exposure (83 dB SPL) on auditory loudness, and temporal processing was evaluated using CBA/CaJ mice. C-Fos staining was used to detect neural activity changes in the central auditory pathway

Oto-traumatic effects of computer simulated magnetic coil impulse noise: analysis of mechanisms.

The brief impulse noise artifacts of 1.0 ms or less generated by some magnetic coils used in extracranial magnetic stimulation may induce acoustic trauma. We investigated the effects of these magnetic coil acoustic artifacts (MCAA) on the inner ear by exposing rabbits to computer stimulated impulse noise designed to mimic the impulse noise of the coil in spectrum and acoustic energy. The simulated impulse noise stimuli (50 impulses) were varied in maximum peak sound pressure (160, 157, and 155 dB re: 20 muPascal), rise-time (100 microseconds and 1,000 microseconds) and duration. The frequency

Modeling otoacoustic emission and hearing threshold fine structures.

A class of cochlear models which account for much of the characteristic variation with frequency of human otoacoustic emissions and hearing threshold microstructure is presented. The models are based upon wave reflections via distributed spatial cochlear inhomogeneities and tall and broad cochlear activity patterns, as suggested by Zweig and Shera [J. Acoust. Soc. Am. 98, 2018-2047 (1995)]. They successfully describe in particular the following features: (1) the characteristic quasiperiodic frequency variations (fine structures) of the hearing threshold, synchronous and click-evoked emissions,

[Acoustic distortion products. Clinical values and limitations in the study of low frequencies].

Distortion products are otoemissions produced when the cochlea is stimulated by two pure, continuous sounds f1 and f2. The most ample distortion product in Man corresponds to a frequency of 2f1-f2. The aim of our work is to appreciate the possibilities to study the distortion products for the exploration of low frequencies in Man. Three populations of subjects have been studied: young subjects have been studied: young subjects with a normal hearing, subjects with pathological otoscopic findings in spite of a normal hearing, subjects with pathological otoscopic findings in spite of a normal aud

Normal function of the middle ear and its measurement.

Some principles underlying acoustic impedance measurements in the ear canal and their evaluation are reviewed. The following topics are included: some methods of measurement, useful frequency range, the middle-ear system, impedance characteristics of normal and some pathological ears, and the relation between the impedance and tympanometric results. Advantages and disadvantages of measurements with an acoustic bridge and parallel probe tubes are contrasted and the same is done for tympanometry versus static impedance measurements. The relation between tympanograms and static impedance data ref

The facial nerve in medial acoustic neuromas.

OBJECT: Functional results after surgery for acoustic neuromas that have little or no growth within the internal auditory canal are controversial, because these medial tumors can grow to a considerable size within the cerebellopontine angle (CPA) before symptoms occur. METHODS: A prospective study was designed to evaluate the surgical implications of the course of the facial nerve within the CPA on medial acoustic neuromas. This study included a consecutive series of 22 patients with medial acoustic neuromas (mean size 32 mm, range 17-52 mm) who underwent surgery via a suboccipitolateral appr

[Tullio phenomenon after cochlear implantation].

The Tullio phenomenon is defined as an acoustically inducible vestibular disorder that was first described in 1929. In an animal experiment Tullio provoked acoustic oscillations at the labyrinth followed by signs of imbalance. In the literature this phenomenon can be found in healthy but sensitive persons as well as in patients with Meniere's disease and patients with lesions between the stapes, footplate and the membranous labyrinth caused by fractures, stapes dislocations, labyrinthitis or perilymphatic fistulas. In this case report a patient complained about vertigo after cochlear implantat

Non-ossicular signal transmission in human middle ears: Experimental assessment of the "acoustic route" with perforated tympanic membranes.

Direct acoustic stimulation of the cochlea by the sound-pressure difference between the oval and round windows (called the "acoustic route") has been thought to contribute to hearing in some pathological conditions, along with the normally dominant "ossicular route." To determine the efficacy of this acoustic route and its constituent mechanisms in human ears, sound pressures were measured at three locations in cadaveric temporal bones [with intact and perforated tympanic membranes (TMs)]: (1) in the external ear canal lateral to the TM, P(TM); (2) in the tympanic cavity lateral to the oval wi…

Spatial overlap of combined electroacoustic stimulation determines the electrically evoked response in the guinea pig cochlea.

HYPOTHESIS: Limiting spatial overlap between electrical stimulation (ES) and acoustical stimulation (AS) in the cochlea reduces the effects of AS on electrically evoked auditory nerve activity. BACKGROUND: Some hybrid cochlear implant systems have a regular array, whereas others have short arrays that spatially segregate ES from AS. AS settings in hybrid implants may also affect electroacoustic interaction. METHODS: ES (900 μA) was delivered in the high-frequency part of the cochlea, and the electrically evoked compound action potential (eCAP) was recorded to assess auditory nerve activity.

Neural coordination of bilateral hand movements: evidence for an involvement of brainstem motor centres.

Bilateral hand movements are assumed to be coordinated by a neural coupling mechanism. Neural coupling is experimentally reflected in complex electromyographic (EMG) responses in the forearm muscles of both sides to unilateral electrical arm nerve stimulation (ES). The aim of this study was to examine a potential involvement of the reticulospinal system in neural coupling by the application of loud acoustic stimuli (LAS) known to activate neurons of this system. LAS, ES and combined LAS/ES were applied to healthy subjects during visually guided bilateral hand flexion-extension movements. Muscl

Wideband detection of middle ear muscle activation using swept-tone distortion product otoacoustic emissions.

The measurement of efferent-induced suppression of otoacoustic emissions (OAEs) using contralateral acoustic stimulation (CAS) is complicated by potential contamination by the middle ear muscle reflex (MEMR), particularly at moderate to high CAS levels. When logarithmically sweeping primaries are used to measure distortion product otoacoustic emissions, the level and phase of the primaries at the entrance of the ear canal may be monitored simultaneously along with the OAEs elicited by the swept-tones. A method of detecting MEMR activation using swept-tones is presented in which the differences…

How do tympanic-membrane perforations affect human middle-ear sound transmission?

Although tympanic-membrane (TM) perforations are common sequelae of middle-ear disease, the hearing losses they cause have not been accurate … ross the TM. Measurements of middle-ear air-space sound pressures show that transmission via direct acoustic stimulation of the oval and round windows is generally negligible. A quantitative model predicts the influence of middle-ear air-space volume on loss; with larger volumes, loss is smaller.

[Intraoperative electroneurophysiological monitoring of basal cranial nerve surgery].

INTRODUCTION: Since 1980 when intraoperative monitoring was introduced or acoustic nerve surgery, the technique rapidly was outspreaded to the other basal cranial nerves, and today is the most important and indispensable piece of work first of all because of its effectiveness in the prevention of the neurologic deficits. OBJECTIVE: The aim of this work is to describe the basic requisites needed (personnel, equipment, stimulation and recording electrodes, patient handling), as well as to review the different techniques used (electromyography and electroneurography) in the motor cranial nerve m
