The optimum heparin monitoring method during extracorporeal membrane oxygenation (ECMO) is unknown. We report a protocol utilizing only anti-factor Xa (anti-Xa) to manage anticoagulation in 22 consecutive ECMO patients. It is a complex activity, with ethical, psychosocial, and demographic dimensions, as well as clinical, technical, and community service components. The author describes the complexities of caregiving and the burdens on the family in a rapidly aging society. He points out that caregiving is associated with significant risk for psychopathology and that a more comprehensive approach to the evaluation and management of chronically ill patients is necessary.

Postoperative deep vein thrombosis has been demonstrated in four out of seven (57%) unselected patients undergoing homograft cadaveric renal transplantation, with pulmonary embolism in one. The diagnosis being made by using the combined 125I-fibrinogen and Doppler ultrasound techniques.
