Theory versus Practice in the Twentieth-Century Search for the Ideal Anaesthetic Gas.

At the beginning of the twentieth century, an anaesthetist could choose between nitrous oxide, chloroform, and ether (diethyl ether) for the induction of painrelieving unconsciousness. By the end of century, the choice was between a small number of fluorinated aliphatic ethers such as Enflurane, Desflurane, and Sevoflurane, and (in some jurisdictions) the rare gas, xenon. Between these endpoints researchers had identified a surprisingly broad range of hydrocarbons, noble gases, organohalogens, and aliphatic ethers that possessed anaesthetic properties. None was entirely satisfactory, but clini

Clarifying the role of activated charcoal filters in preparing an anaesthetic workstation for malignant hyperthermia-susceptible patients.

Malignant hyperthermia (MH) is a life-threatening condition caused by exposure of susceptible individuals to volatile anaesthetics or suxamethonium. MH-susceptible individuals must avoid exposure to these drugs, so accurate and reproducible processes to remove residual anaesthetic agents from anaesthetic workstations are required. Activated charcoal filters (ACFs) have been used for this purpose. ACFs can reduce the time for preparing an anaesthetic workstation for MH patients. Currently, the only commercially available ACFs are the Vapor-Clean$trade; (Dynasthetics, Salt Lake City, UT, USA) fi

INVESTIGATION OF ANESTHESIA TECHNIQUES IN PAPER KITE BUTTERFLIES (IDEA LEUCONOE).

Despite the frequency of invertebrates in research, zoological, and personal collections, sedation and anesthesia in these species remains poorly studied. This study assessed anesthetic techniques in paper kite butterflies (Idea leuconoe). Thirty-six recently emerged butterflies were individually placed in 1.9 L air-tight containers and randomly exposed to one of six treatments (n = 6/group): 5% isoflurane via vaporizer (2 L min-1, fraction of inspired oxygen (FiO2) 1.0); (V), 0.5 mL (CB0.5) or 3.0 mL (CB3) liquid isoflurane on a cotton ball (FiO2 0.21); 100% carbon dioxide via flowmeter (2 L

In Vitro Performance of a Charcoal-capturing Device with Desflurane.

BACKGROUND: The use of capturing devices may become required for the continued use desflurane. This study tested the percentage of desflurane captured by a charcoal filter (CONTRAfluran; Zeosys GmbH, Germany)-workstation (Aisys; GE Healthcare, USA) combination in vitro . METHODS: Desflurane in oxygen/air was administered via an Aisys workstation into a 2-l test lung that was insufflated with carbon dioxide (160 ml/min). First, to confirm that all vaporized desflurane reached the capturing device, the amount of desflurane collected in a Douglas bag attached to the machine exhaust was compared

A systematic review on the use of sevoflurane in the management of status asthmaticus in adults.

BACKGROUND: To conduct a systematic review looking into the use of sevoflurane in the management of status asthmaticus (SA) in adults. METHODS: We performed a systematic search on PubMed, EMBASE, and The Cochrane Library - CENTRAL through 23rd August 2023, restricting to studies reported in English. We included studies reporting use of sevoflurane in asthmatics beyond its use as an anaesthetic agent in surgeries i.e. in the emergency department (ED) and critical care setting, and focused on patient's clinical parameters, ventilation pressures and weaning of invasive ventilation. RESULTS: A t

One node among many: sevoflurane-induced hypnosis and the challenge of an integrative network-level view of anaesthetic action.

Building on their known ability to influence sleep and arousal, Li and colleagues show that modulating the activity of glutamatergic pedunculopontine tegmental neurones also alters sevoflurane-induced hypnosis. This finding adds support for the shared sleep-anaesthesia circuit hypothesis. However, the expanding recognition of many neuronal clusters capable of modulating anaesthetic hypnosis raises the question of how disparate and anatomically distant sites ultimately interact to coordinate global changes in the state of the brain. Understanding how these individual sites work in concert to di

Assessing the potential climate impact of anaesthetic gases.

There is increasing concern within the health-care community about the role care delivery plays in environmental degradation, sparking research into how to reduce pollution from clinical practice. Inhaled anaesthetics is a particular research area of interest for two reasons. First, several gases are potent greenhouse gases, and waste gas is mostly emitted directly to the environment. Second, there are options to reduce gas waste and substitute medications and procedures with fewer embodied emissions while delivering high-quality care. Performance improvements are contingent on a proper unders

The science behind banning desflurane: A narrative review.

Potent inhaled anaesthetics are halogenated hydrocarbons with a large global warming effect. The use of fluorinated hydrocarbons (most are not anaesthetics) are being restricted but volatile anaesthetics have been exempted from legislation, until now: the EU has formulated a proposal to ban or at least severely restrict the use of desflurane starting January 2026. This narrative review addresses the implications of a politics-driven decision - without prior consultation with major stakeholders, such as the European Society of Anaesthesiology and Intensive Care (ESAIC) - on daily anaesthesia pr

[Malignant Hyperthermia - Update on Pathophysiology, Diagnostics and Treatment].

Malignant hyperthermia (MH) is a rare, heterogenic syndrome leading to potentially life-threatening skeletal muscle hypermetabolism following exposure to inhalational anesthetics and succinylcholine. In more than 50% of affected individuals a pathogenic variant in the RYR1 gene coding for the sarcoplasmic reticulum calcium channel is responsible for the underlying pathology of uncontrolled calcium liberation. While the genetic prevalence of MH is as high as 1 : 2750, the incidence of clinical MH reactions is considerably lower, suggesting a dominant pattern of inheritance with incomplete penet

Practical treatment recommendations for the safe use of anaesthetics.

General anaesthesia is the reversible depression of central nervous system function. There is still no agreement over what constitutes depth of anaesthesia, and the clinical anaesthetist must thus titrate drug input according to clinical signs (heart rate, blood pressure, somatic movement, autonomic responses). The potency of inhalational agents may be expressed in terms of the MAC (minimum alveolar concentration); comparable end-points (including blood concentrations) have been proposed for the intravenous agents. Kinetic infusion regimens can be constructed for the intravenous agents to achi

[The Draeger-Flaschenpaket - a portable emergency equipment for continuous support with gases for anaesthesia (author's transl)].

The Dräger Flaschenpaket for immediate supply of oxygen and nitrous oxide may be used in operating theatres or during the transportation of artificially ventilated patients. The equipment may be used with two oxygen bottles or one oxygen and one nitrous oxide bottle (bottle size in each case 2 1). A quick connection between the tubes of the anaesthesia apparatus or respirator and the equipment is ensured. The portable equipment may be stationed in operating theatres, in intensive care and in shock units.
