Mechanical equilibrium of the ankle joint: Implications for Achilles tendon injury.

BACKGROUND: A longer Achilles tendon (AT) moment arm (MA) relative to the foot length requires less muscle force for a given plantarflexion moment, thereby lowering mechanical load on the tendon. We examined whether the AT and foot MA lengths in healthy (N = 28) differ from height and body mass matched individuals with AT tendinopathy (N = 28) and rupture (N = 29). We hypothesized that a longer AT MA has a protective effect on tendon loading with the patient groups having smaller MA compared to asymptomatic individuals. METHODS: MAs were assessed using 2-D image-based method. AT MA was define

A Case Report on a Syndesmotic Ankle Injury Treated With Arthrex TightRope.

Syndesmotic ankle injuries, often referred to as "high ankle sprains," pose intricate challenges in orthopedic practice, particularly among athletes engaged in high-impact sports. Conventional treatments have encompassed conservative approaches and the use of syndesmotic screws, each beset by inherent limitations. The Arthrex TightRope system has emerged as a pioneering alternative, heralded for its capacity to facilitate physiologic micromotion, eliminate the necessity for hardware removal, and expedite early rehabilitation. This case report delineates the management of a 29-year-old male pro…

Syndesmotic Ligaments of the Ankle: Anatomy, Multimodality Imaging, and Patterns of Injury.

Injuries to the syndesmotic ligaments of the ankle or "high ankle sprains" are common in acute ankle trauma but can be difficult to diagnose both clinically and on imaging. Missed injuries to the syndesmosis can lead to chronic ankle instability, which can cause persistent pain and lead to early osteoarthritis. This review will illustrate the anatomy of the syndesmotic ligamentous complex, describe radiographic, CT, and MR imaging of the syndesmosis, demonstrate typical mechanisms of injuries and associated fracture patterns, and provide an overview of important management considerations.

Repair of syndesmosis injury in ankle fractures: Current state of the art.

Conventional treatment of syndesmosis injuries in rotationally unstable ankle fractures is associated with an unacceptably high rate of malreduction, and this has led to a paradigm shift in the approach to a newer concept of anatomical repair. In the anatomical approach, the principle is to 'directly fix what is broken and repair what is torn'. The approach is effective in reducing the rate of syndesmosis malreduction, increasing the biomechanical strength of syndesmosis fixation and avoiding the need for trans-syndesmotic fixation and its secondary removal. The objective of this review article

[Ankle fracture in adolescents: triplanar injury associated to fracture of leg].

the vast mayority of epiphyseal children fractures could be classified according to Salter Harris. There are other types of patterns such as triplanar and Tillaux fractures. The former is a complex type with fracture lines in sagital, axial and frontal planes. These fractures have main complications such as early epiphyseal closure, mechanical axis malalignment and gait disturbances. A clinical case is pictured which had a tibial shaft and ipsilateral triplanar fracture, this complex scenario of rare ocurrence was treated with reduction and osteosynthesis of both fractures. This clinical case

Comparative assessment of the acute ankle injury by ultrasound and magnetic resonance.

We compared ultrasound (US) with magnetic resonance (MR) findings of muscle tendon and ligaments (mt&l) of 17 men and 13 women, 16-66 years old, who suffered from acute ankle injury without bone fracture visible on conventional radiographs. Joint effusion (JE), and injury of the Tibials anterior muscle tendon (TAmt), Calcaneofibular ligament (CFl), Long flexor of the great toe muscle tendon (LFGTmt), Short peroneus muscle tendon (SPmt), Long peroneus muscle tendon (LPmt), and Anterior talofibular ligament (ATFl) were assessed by the US, at seven days, and MR, at seventeenth day. Measuring foot and ankle injury outcomes: common scales and checklists.

Functional health outcome measures are increasingly being used in both clinica trials and practice as measures of patient health. Whilst outcome measures can be generic, there are a number of foot and ankle specific measures available and in use. These are being used as not only region specific but also disease specific measures of patient function. Unfortunately not all of these outcome measures have been completely validated which leads to challenges in applying the results of outcomes research to specific patients. Continued work however is being done in this area and these challenges provi

Injury risk factors in parachuting and acceptability of the parachute ankle brace.

INTRODUCTION: This investigation examined risk factors for injuries during military parachute training and solicited attitudes and opinions regarding a parachute ankle brace (PAB) that has been shown to protect against ankle injuries. METHODS: Male Army airborne students (N = 1677) completed a questionnaire after they had successfully executed 4 of the 5 jumps necessary for qualification as a military paratrooper. The questionnaire asked about injuries during parachute descents, demographics, lifestyle characteristics, physical characteristics, physical fitness, airborne recycling (i.e., repe…

Compartment syndrome with foot ischemia after inversion injury of the ankle.

This article reports the case of a 17-year-old girl who presented with compartment syndrome and acute ischemia of the foot after a minor ankle sprain. The suspected cause of compartment syndrome was secondary emergence of swelling and palsy of the foot. The posterior tibial and dorsalis pedis pulses were nonpalpable. The syndrome was confirmed by measurement of the pressures in the compartments of the foot, which were >30 mm Hg. Foot fasciotomy was successfully performed by using a three-incision technique. In contrast with previous case reports, no bone or vessel lesion was detected to explai

[Thrombosis of the posterior tibial artery complicating closed injury of the ankle].

Closed ankle injury without fracture is a common finding in the emergency room. Outcome is generally spontaneously favorable, the injury having no long-term clinical impact. Exceptionally, these injuries can be associated with arterial damage. We report a case of an apparently benign closed ankle injury which was found to be associated with serious arterial damage. Forced dorsal flexion of the ankle joint during a traffic accident caused an arterial lesion without any apparent damage to the bone and joints. The posterior tibial artery was interrupted leading to subacute ischemia of the foot. T

Comparison of acute swelling and function in subjects with lateral ankle injury.

STUDY DESIGN: Prospective correlational study involving a convenience sample. OBJECTIVES: To investigate the relationships among figure-of-eight girth measurements and functional level in patients with acute lateral ankle sprains to determine the appropriate use of these clinical measures. BACKGROUND: Research has shown that subjective scales of perceived athletic ability and measurements of swelling are useful in assessing clinical improvement following an acute ankle sprain; however, the relationship between ankle swelling and level of function is not known. METHODS AND MEASURES: Twenty-n

Torsional strength of the ankle in vitro. The supination-external-rotation injury.

Nineteen fresh-frozen anatomic specimens of the ankle were mounted in an experimental test device with the foot in supination. Torque versus rotation curves were recorded as each foot was rotated externally to failure. The mean failure torque was 45.3 Nm, the mean rotation to failure was 41.4 degrees, and the mean energy absorbed to failure was 10.6 Nm. Twelve specimens failed by fracture of the fibula, five by failure of lateral ligaments with no fibular fracture, one by a subtalar dislocation, and one by fracture of the calcaneous through a fixation screw hole. There were no statistically si

Stabilometry in functional instability of the ankle and its value in predicting injury.

Stabilometry is an objective method used for studying postural equilibrium quantitatively. Stabilometric recordings were made in 127 soccer players to demonstrate functional instability of the ankle joint. The presence of previous ankle joint injuries, i.e., sprains or fractures, was documented. Reference values for stabilometry were obtained from a group of 30 normally-active non-soccer players without a history of injury to the ankle joint. A pathological stabilometric value was defined as one exceeding the mean value of the reference group by 2 SD. In players with a history of previous ankl

[Rare tendon injuries of the foot and ankle : Tibialis posterior-Tibialis anterior-Peroneus longus and brevis].

Compared to ruptures of the Achilles tendon, injuries to the tibialis anterior and tibialis posterior tendons or the peroneal tendons are relatively rare, so that the literature on this subject largely consists of individual case reports and small case series. There are major differences between the individual entities in terms of the pathogenesis, possible sequelae and treatment recommendations. This article is intended to provide an overview and instruction manual for the clinical management.
