Sudden infant apnea and insidious hydrocephalus.

Hydrocephalus secondary to intracranial-intraventricular hemorrhage is a common complication in the clinical course of the high-risk preterm newborn. Hydrocephalus in this population may be insidious without obvious intracranial hypertension. Apnea and respiratory arrest continue to cause concern following nursery discharge of the high-risk preterm newborn. We report a child who presented to the neonatology service with episodes compatible with serious sudden cardiorespiratory arrest. Insidious "non-hypertensive" hydrocephalus was documented as being responsible for these episodes, which resol…

Sleep disorders: sleep apnea and narcolepsy.

Symptoms of obstructive sleep apnea, a potentially life-threatening disorder, include excessive daytime sleepiness and sleep attacks, nocturnal breath cessation, and snorting and gasping sounds. These symptoms usually become manifest before age 40 and cluster within a few years. Most patients are obese, hypertensive men who eventually develop cardiovascular abnormalities. If sleep apnea is suspected based on clinical information, a sleep laboratory evaluation is indicated. For severe obstructive sleep apnea, tracheostomy is the most effective treatment. Narcolepsy, another sleep disorder, is a…

Obstructive sleep apnea syndrome. A review.

Obstructive sleep apnea syndrome (OSAS) is a complex disorder characterized by a sleep-related collapse of the upper airway. The most likely candidate for the common pathway linking various abnormalities casually associated with OSAS (such as adenotonsillar hypertrophy, obesity, retro- or micrognathia, acromegaly, or more subtle structural anomalies) is an abnormally small upper airway lumen. Symptoms of OSAS that appear during sleep include snoring, abnormal motor activity, disturbed nocturnal sleep, a sensation of choking, heartburn, nocturia, nocturnal enuresis, and heavy sweating. Daytime …

Sleep apnea and autonomic cerebrovascular dysfunction.

Changes in common carotid blood flow (CCF) and resistance index (RI), calculated from velocity waveforms by a noninvasive pulsed Doppler technique, were measured during apneic episodes and voluntary breath holding in five sleep apnea patients (SA) and during breath holding in five normal subjects (N). During apneic episodes averaging 27 s, CCF was reduced by 9% and RI increased by 4%, both trends being related to apneic duration. Internal carotid artery measurements in one SA indicated more dramatic changes in blood flow and RI than noted in CCF. During breath holding, CCF decreased significan…

Surgical treatment of obstructive sleep apnea.

In selected patients with obstructive sleep apnea, the uvulopalatopharyngoplasty procedure may be performed to remove excessive tissue in the oropharyngeal airway. This may improve the symptoms of sleep apnea as well as snoring. Tracheostomy may be indicated in patients with obstructive sleep apnea with associated severe medical problems.

Sympathetic rhythms during hyperventilation-induced apnea.

The effect of hyperventilation-induced apnea on the respiratory rhythmicity of sympathetic nerve activity was determined using spectral analysis of sympathetic nerve frequencies. Left phrenic, external intercostal, and inferior cardiac sympathetic nerves were recorded in alpha-chloralose-anesthetized, vagotomized, paralyzed, artificially ventilated cats. The respiratory modulation of sympathetic activity during normoventilation was indicated by spectral peaks of sympathetic activity coinciding with respiratory frequencies determined from the phrenic nerve activity of each cat. The spectral pea…

Computerized acoustic detection of obstructive apnea.

Cardiac, respiratory and neurologic abnormalities have been identified as causes of Sudden Infant Death Syndrome (SIDS). Recurrent central apnea (no respiratory effort or nasal/oral airflow) and obstructive apnea (respiratory effort without concurrent nasal/oral airflow) in infants are considered risk factors for SIDS. However, using currently available monitoring techniques, normal activities such as yawns, stretches and swallows cannot be distinquished from short obstructive episodes lasting less than 20 s. A system was developed to more accurately detect obstructive apnea in infants using a…

Syringobulbia-myelia with obstructive sleep apnea.

Central nervous system disorders may cause obstructive sleep apnea, but this sleep-related breathing disorder is rarely associated with a structural abnormality of the medulla. We present a patient with obstructive apnea resulting from autopsy-proven syringobulbia-myelia.

Anatomic abnormalities in obstructive sleep apnea.

Both muscular hypotonia and anatomic abnormalities have been implicated in the pathogenesis of the obstructive sleep apnea syndrome (OSAS). The upper airways of 25 patients with OSAS were evaluated to determine potential sites of obstruction. Significant airway compromise was found in all patients with some patients having multiple sites of airway narrowing.

Sleep apnea syndrome and supraglottic edema.

Obstructive sleep apnea is becoming more widely recognized. Otolaryngologists will be called upon to evaluate affected patients and offer consultation as to the appropriateness of surgical intervention. The extent to which this syndrome is attributable to edema of the larynx and hypopharynx is unknown. It may, on occasion, explain chronic fatigue, weight loss, and daytime somnolence seen in some patients following irradiation of the head and neck. Sleep apnea associated with upper airway obstruction can be confirmed by polysomnography during sleep. Monitoring the EEG, electro-oculogram, electr…

Obstructive sleep apnea in family members.

Two sons and their father had severe hypersomnolence and obstructive sleep apnea. A third son, although asymptomatic, was shown to have upper-airway obstruction during sleep. Electromyographic recordings of genioglossus activity in the two symptomatic sons revealed loss of tonic activity in early stages of sleep at times when sleep apnea occurred. The asymptomatic son showed loss of tonic activity during rapid-eye-movement sleep, the sleep period when upper-airway obstruction occurred. Two sudden deaths occurred in this family. A 30-year-old brother died at home while asleep, and a child of th…

Beyond the Apnea-Hypopnea Index: Exploring Time-Dependent Hazard Ratios of Respiratory Events in Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is associated with increased risks of systemic comorbidities, leading to significant morbidity and mortality. This study investigates predictors of all-cause mortality, emphasizing the interplay of clinical symptoms, polysomnographic findings, and comorbidities. The aim of this study was to identify and compare respiratory predictors of all-cause mortality over 5, 10, and 15 years. A single-center study was conducted at a Sleep Medicine Department between 2005 and 2019, 4025 patients with suspected OSA who underwent polysomnography were admitted, 853 died during t…

Association between sleep apnea severity and symptoms of depression and anxiety among individuals with obstructive sleep apnea.

OBJECTIVE: We determined whether the severity of sleep apnea is associated with symptoms of depression and anxiety in patients with obstructive sleep apnea (OSA) and whether symptoms of OSA, other than respiratory events, are associated with depression and anxiety symptoms. METHODS: Depressive and anxiety symptoms were defined as a Patient Health Questionnaire-9 score of ≥10 and a Generalized Anxiety Disorder-7 score of ≥8, respectively. Sleep apnea severity and rapid eye movement-related OSA were evaluated using the apnea-hypopnea index (AHI). Subjective symptoms of OSA were assessed using t…

Relations between obstructive sleep apnea syndrome and specific cephalometric measurements, body mass index, and apnea-hypopnea index.

PURPOSE: To evaluate the relation of specific cephalometric landmarks, body mass index, and the apnea-hypopnea index in patients diagnosed with obstructive sleep apnea syndrome (OSAS) and treated with functional upper airway surgery. MATERIALS AND METHODS: This was a retrospective cohort analysis of 89 consecutive patients over a 3-year period diagnosed with overnight-attended polysomnogram-confirmed OSAS who underwent functional upper airway surgery. Five predetermined specific cephalometric parameters were analyzed: posterior airway space, soft palate length, hyoid to mandibular plane angle…

Characteristics of Japanese patients with complex sleep apnea syndrome: a retrospective comparison with obstructive sleep apnea syndrome.

OBJECTIVE: The prevalence of complex sleep apnea syndrome (CompSAS) among Asian patients with obstructive sleep apnea syndrome (OSAS) has not yet been reported. Distinguishing CompSAS from pure OSAS is difficult using only diagnostic polysomnography (PSG). We examined the prevalence of CompSAS in Japanese patients with OSAS and the possibility to distinguish CompSAS from pure OSAS by analyzing the severity of respiratory events based on either sleep body position or sleep stage using a diagnostic PSG. PATIENTS AND METHODS: A retrospective chart review of 297 consecutive Japanese patients who …

Achondroplasia and obstructive sleep apnea: correction of apnea and abnormal sleep-entrained growth hormone release by tracheostomy.

Severe obstructive sleep apnea in a patient with achondroplasia syndrome was found to result in a definitive deficiency of overnight growth hormone secretion related to absence of slow-wave sleep. Resolution of the apnea by tracheostomy resulted in normalization of growth hormone release and normal growth rates postoperatively. Sleep-related growth hormone deficiency may contribute to the short stature so often seen in a variety of craniofacial syndromes. Furthermore, this short stature may be reversible.

A shift from central and mixed sleep apnea to obstructive sleep apnea resulting from low-flow oxygen.

Low-flow oxygen decreases the frequency of the 3 types of apnea (central, mixed, and obstructive) in patients with predominantly obstructive sleep apnea. The decrease in frequency appears to be accompanied by a shift in apnea distribution, consisting of a decrease in the proportion of central and mixed apneas and an increase in that of obstructive apneas. To determine whether this shift represents a greater inhibitory effect on central and mixed apneas or an increased tendency toward obstructive apneas, we administered low-flow oxygen during sleep to 9 patients who demonstrated predominantly c…
