This is a Consensus Document of the Sociedad Española de Infectología Pediátrica, Sociedad Española de Reumatología Pediátrica and Sociedad Española de Ortopedia Pediátrica on the aetiology and diagnosis of uncomplicated acute osteomyelitis and septic arthritis. A review is presented of the aetiopathogenesis and pathophysiology of acute osteoarticular infection defined as a process with less than 14 days of symptomatology, uncomplicated, and community-acquired. The diagnostic approach to these conditions is summarised based on the best available scientific knowledge. Based on this evidence, a

Identifying viral infections related to rheumatic syndromes and understanding the pathophysiologic mechanisms by which they cause disease are crucial steps to furthering our understanding of the pathogenesis of rheumatic disease. Many common viral infections can induce autoantibody formation. Parvovirus B19 (B19) can cause acute arthritis, and occasionally chronic arthropathy, in infected adults. Persistent B19 infection can be found in synovium of some patients. Antibodies reactive with B19 epitopes can cross react with some autoantigens. Studies of rheumatic syndromes associated with other v

Musculoskeletal involvement, particularly arthritis, is a common feature of Lyme disease. Early in the illness, patients may experience migratory musculoskeletal pain in joints, bursae, tendons, muscle, or bone in one or a few locations at a time, frequently lasting only hours or days in a given location. Weeks to months later, after the development of a marked cellular and humoral immune response to the spirochete, untreated patients often have intermittent or chronic monoarticular or oligoarticular arthritis-primarily in large joints, especially the knee-during a period of several years. Septic arthritis (SA) is reviewed in the English literature with reference to registration of localisation of the infected joints, the bacteriological cause, predisposing factors, clinical information and laboratory data related to SA. Seventy-eight publications were available concerning 975 patients with 1086 infected joints caused by 1032 bacteria. Large joints such as the knee and hip were most frequently, however, infected, in recent years, SA in small axial joints has been reported with increased frequency. Gram-positive cocci and especially Staphylococcus aureus are most frequently the c

Septic sacroiliitis (SSI) is a rare form of pyogenic arthritis, which unrecognized or inadequately treated may cause longterm morbidity and eventually lasting disability. SSI is most frequently diagnosed among young adults, especially in drug abusers. Based on a literature survey three cases of SSI are presented, each representing characteristic aspects of SSI. The condition should be considered in cases of acute or subacute onset of pain in the gluteal region, hip or low back accompanied by fever. Laboratory findings are variable and non-specific. Definite bacteriologic diagnosis is only achi

A case of articular sporotrichosis occurring in the wrist and ankle of a 39-year-old man is reported. Review of the literature on this disorder confirms the diagnostic difficulties presented by this usually chronic, progressive, fungal arthritis. The diagnosis of this entity should be considered in the evaluation of any patient presenting with a progressive destructive arthritic process. The pathological and radiological features of a typical case are discussed and the mechanisms of infection and type of individual affected by this disease are described.

Septic arthritis is a relatively uncommon manifestation of listeriosis. Only four documented cases of Listeria arthritis have been reported in the medical literature. We now present a fifth case associated with Listeria peritonitis and briefly review the clinical spectrum of articular listeriosis. These cases suggest that (1) the early diagnoses of Listeria peritonitis and arthritis require a high degree of clinical suspicion, and (2) a cephalosporin should not be used as the sole antibiotic in cases of septic arthritis and peritonitis in which the causative organism has not been identified.

Salmonella dublin is a rare pathogen in man usually causing an enteric fever syndrome. We report a 32-year-old black male who developed septic polyarthritis with Salmonella dublin. The clinical features are similar to those seen with other Salmonella serotypes reported to cause arthritis. The initial presentation is suggestive of a reactive arthritis that is immunologically mediated.
