Re-examining the brain regions crucial for orchestrating speech articulation.

A traditional method of localizing brain functions has been to identify shared areas of brain damage in individuals who have a particular deficit. The rationale of this 'lesion overlap' approach is straightforward: if the individuals can no longer perform the function, the area of brain damaged in most of these individuals must have been responsible for that function. However, the reciprocal association, i.e. the probability of the lesion causing the deficit, is often not evaluated. In this study, we illustrate potential weaknesses of this approach, by re-examining regions of the brain essenti

Refractory dyslexia: evidence of multiple task-specific phonological output stores.

We investigated the case of a patient whose reading was characterized by multiple phonemic paraphasic errors. An error analysis of a large corpus of reading responses (758 words, 86 non-words) highlighted the preponderance of phonological errors which did not occur in his naming, repetition or spontaneous speech. His comprehension of the written word was relatively preserved, even for words he was unable to read aloud. We suggest that his impairment lies at the level of the phonological output store. We also demonstrate that his reading performance was facilitated by increasing the response-st

Articulatory consequences of Parkinson's disease: perspectives from two modalities.

Language production involves complex yet productively varying motor behavior. Rule-governed combinations yield a finite set of formational units combined in an infinite number of ways. The creativity of language ensures that no particular articulation will be highly automatized. Linguistic articulation is highly complex and varied. As such, it differs from the other more automatized motor behaviors typically studied such as learned movements in apraxia studies or repetitive behavior as occurs in walking or other everyday activities. Language also strives to maintain a balance between ease of a

Type and frequency of phonetic errors in aphasics: therapeutic aspects.

Fifty aphasics were studied in three separate groups: nonfluent Broca, fluent Wernicke and fluent anomics in order to form a therapeutic guide of their phonetic errors (omission substitution, addition, repeated phonemes, perseveration of phonemes belonging to the preceding word, reversed order, phonematic targetting and misplacing. Frequency of phonetic error was also assessed.

Acoustic tracking of articulation errors: [r].

The objective of this study was to determine the usefulness of acoustic tracking as a supplement to perceptual judgements during remediation. One child, receiving weekly individual treatment, participated in a drill-and-practice approach to remediation for a single [w]-for-/r/ substitution error. Imitations of consonant-vowel (CV), VCV, and matched /r/ and /w/ sentence stimuli were audiotape recorded. The temporal and spectral characteristics of the recorded stimuli (i.e., the F2 transition rates and F2 values for the changing [r]) were spectrographically tracked and analyzed over a 70-day rem

MED13L-related disorder characterized by severe motor speech impairment.

BACKGROUND: MED13L-related disorder is associated with intellectual disability, motor delay, and speech deficits. Previous studies have focused on broad clinical descriptions of individuals, but limited information regarding specific speech diagnoses and results of direct testing has been published to date. We conducted deep phenotyping to characterize the speech, language, motor, cognitive, and adaptive phenotypes of individuals with MED13L-related disorder. METHODS: In this cross-sectional study, we administered standardized articulation, language, motor, and cognitive testing to 17 childre

[Diseases of the acromioclavicular joint].

The acromioclavicular joint together with the sternoclavicular joint represents the only articulation between the shoulder girdle, the upper extremities and the trunk. The high load on the relative small joint surface results in a high risk for degenerative changes. The most common pathology is therefore osteoarthritis. In addition, joint instability and many inflammatory processes can occur, especially rheumatoid type pathologies and metabolic disorders. Acromioclavicular cysts represent a clinically evident disease, which are frequently associated with an underlying cuff tear arthropathy. A

[A rare cause of febrile cervical pain].

Febrile cervical pain is often secondary to meningitis or spondylodiscitis and, exceptionally, to microcrystalline arthropathy. We here report a case. A 81-year old man with no particular personal history was hospitalized with febrile cervical pain. Initial diagnoses were meningitis and spondylodiscitis. Clinical examination showed overall stiffness of cervical rachis. Spinal MRI showed abnormality of the atlanto-axial articulation signal, enhanced after gadolinium injection with synovial hypertrophy associated with irregular and heterogeneous aspect of the dens of the axis. Serial atlanto-axi

Prosthetic rehabilitation of hemimandibular hyperplasia: five-year follow-up.

Hemimandibular hyperplasia is a rare asymmetrical mandibular malformation, characterized by enlargement of the condyle, the condylar neck, the ramus, and the body of the mandible. This condition results in laterognathia, dental articulation disorders, and functional defects. Therapy largely depends on the patient's age and the desired esthetic and functional results. This clinical report describes the prosthetic rehabilitation of a 50-year-old woman with hemimandibular hyperplasia. During the diagnostic phase, facial asymmetry was observed, as was the chin midline shifting to the unaffected si…

Effects of acoustic startle priming on poststroke dysarthria rehabilitation: study protocol for a double-blinded, single-centre randomised controlled trial in China.

INTRODUCTION: Dysarthria is one of the most prevalent communication disorders following a stroke, and it can lead to social isolation and depression, significantly hindering rehabilitation progress. Recent studies suggest that acoustic startle priming (ASP) tasks may enhance speech performance and motor initiation through the corticoreticulospinal tract. This has opened up new possibilities for the application of ASP in the rehabilitation of poststroke dysarthria. The aim of this study is to investigate whether ASP can improve rehabilitation outcomes in individuals with poststroke dysarthria.

Modern Dual Mobility Implants in Total Hip Arthroplasty: Stability, Survival, and Ion Safety Evaluation: The Comprehensive Review.

» Dual mobility cup (DMC) implants reduce dislocation rates in both primary and revision total hip arthroplasty (THA), particularly in high-risk patients with neuromuscular disorders, cognitive impairment, or obesity. » The dual articulation mechanism enhances joint stability by increasing jump distance and the head-to-neck ratio, without compromising range of motion. » Incorporation of highly cross-linked polyethylene and vitamin E-infused liners improves wear resistance, supporting broader use in younger, more active patients. » Long-term survivorship of modern DMCs is comparable or superior

Temporomandibular joint disorder: Anatomy of pain (part 1).

The temporomandibular joint (TMJ) is a complex synovial articulation connecting the mandible to the temporal bone, facilitating essential functions like chewing and speaking through rotational and translational movements. Temporomandibular disorders (TMDs) cause significant pain and dysfunction, often involving myogenous, arthrogenous, or multifactorial etiologies. This review elucidates TMJ anatomy, embryology, and pain pathways, with a focus on central pain mechanisms underlying TMD. Key anatomical components include the mandibular condyle, articular disc, and masticatory muscles, innervated

The natural occurrence of generalization during articulation treatment.

This study examined the extent to which generalization of correct phoneme production occurred in various speaking environments and across word positions without direct training. Three preschool children with articulation disorders received treatment in a clinic setting. Treatment targets consisted of specific phonemes restricted to particular word positions. Data were collected on the children's productions of the target phonemes in treated and untreated word positions. In addition, the children's productions were monitored with different adults in the clinic and at home. The results indicated…

Retroflex versus bunched in treatment for rhotic misarticulation: evidence from ultrasound biofeedback intervention.

PURPOSE: To document the efficacy of ultrasound biofeedback treatment for misarticulation of the North American English rhotic in children. Because of limited progress in the first cohort, a series of two closely related studies was conducted in place of a single study. The studies differed primarily in the nature of tongue-shape targets (e.g., retroflex, bunched) cued during treatment. METHOD: Eight participants received 8 weeks of individual ultrasound biofeedback treatment targeting rhotics. In Study 1, all 4 participants were cued to match a bunched tongue-shape target. In Study 2, partic

Types of clefts and multianomaly craniofacial conditions.

Speech-language pathologists (SLPs) in the public schools or other nonmedical settings rarely see infants or small children with unrepaired clefts. When children with repaired clefts appear in their caseloads, it may be difficult to comprehend what the situation was before the child had surgery. Clefts vary widely in their original severity, which has a direct bearing on how the repaired cleft looks and how the orofacial structures (lip, teeth, and palate) affect speech when the child comes into the SLP's caseload. It is important to understand that a high percentage of children with nonsyndro

Transcribing disordered speech: by target or by production?

The ability to transcribe disordered speech is a vital tool for speech-language pathologists, as accurate description of a client's speech output is needed for both diagnosis and effective intervention. Clients in the speech clinic often use sounds that are not part of the target sound system and which may, in some cases, be sounds not found in natural language at all. While the IPA provides a wide range of symbols that can be used in clinical transcription, the extended IPA (extIPA) may also be needed to transcribe atypical sounds never or rarely encountered in natural language. When using th

Speech perception in preschoolers at family risk for dyslexia: relations with low-level auditory processing and phonological ability.

We tested categorical perception and speech-in-noise perception in a group of five-year-old preschool children genetically at risk for dyslexia, compared to a group of well-matched control children and a group of adults. Both groups of children differed significantly from the adults on all speech measures. Comparing both child groups, the risk group presented a slight but significant deficit in speech-in-noise perception, particularly in the most difficult listening condition. For categorical perception a marginally significant deficit was observed on the discrimination task but not on the ide
