What is the impact of home non-invasive ventilation on the health-related quality of life of patients with chronic obstructive pulmonary disease? A systematic review.

OBJECTIVES: To ascertain the impact of home non-invasive ventilation (NIV) on the health-related quality of life (HRQL) of patients with moderate-to-severe chronic obstructive pulmonary disease (COPD). DESIGN: Systematic review. METHODS: A preliminary search of computerised databases (CINAHL, Medline, Clinical Key, Cochrane) was conducted in June 2021, without any limitations on publication date. Inclusion criteria focused on home NIV prescribed for patients with moderate-to-severe COPD. Identified papers were critically appraised for rigour and validity. Data were extracted, analysed, and a

[GERD and Barett: Natural Course of One Disease - Update Diagnostics and Therapy].

GERD and Barett: Natural Course of One Disease - Update Diagnostics and Therapy Abstract. The gastroesophageal reflux disease (GERD) represents a relatively frequent condition, which clinically includes orocervical, thoracic and abdominal complaints. GERD is defined as pathological gastroesophageal acidic reflux, which consecutively leads to mucosal damage of the esophagus such as reflux esophagitis. The most common symptom of GERD is heartburn but GERD symptoms include various complaints, which need to be considered in diagnosis and therapy. Besides endoscopy, barium swallow, pH metry and man

Development of an Image-Based Methodology for the Evaluation of Histopathological Features in Human Meningioma.

Meningioma is the most common intracranial tumor in adulthood. With a clear female predominance and a recurrence rate that reaches 20%, it is, despite being considered a benign tumor, a pathology that greatly compromises post-diagnosis quality of life. Its prone to recur or progress to a higher degree is difficult to predict in the absence of obvious histological criteria. This project aims to develop an automatic methodology to aid in the diagnosis of meningiomas that is objective and easily reproducible. The methodology is based on histopathological image analysis using artificial intelligen

Clinical symptoms of limited exercise capacity linked to AAI-DDD functionality: An in silico and in vivo approach.

BACKGROUND: Exercise capacity is an important aspect of quality of life in patients undergoing pacemaker implantation. Device algorithms for ventricular pacing avoidance have been developed to avoid unnecessary and potentially harmful effects of right ventricular pacing. However, little data exists on the immediate response of these algorithms to sudden AV block during exercise. METHODS: The ventricular pacing avoidance algorithms of four pacemaker manufacturers were tested in an ex-vivo model. The RSIM-1500-USB Device-Interactive Heart Simulator (Rivertek Medical Systems, Inc.) was used to s

Hard-to-heal wound treated with Integra Flowable Wound Matrix: analysis and clinical observations.

Skin healing defects severely impair the quality of life of millions of people and burden healthcare systems globally. The therapeutic approach to these pathologies still represents a challenge. Novel scaffolds, used as dermal substitutes, possibly represent a promising strategy in complex wound management. Integra Flowable Wound Matrix (IFWM) is composed of a lyophilised, micronised form of collagen/chondroitin sulphate matrix, already used in regenerative medicine and endorsed in the therapy of diabetic foot lesions. In this paper, IFWM was applied to a tunnelling hard-to-heal skin lesion in

The Importance of Selecting Permanent Pacemaker, Evaluating the Heart and Assessing Quality of Life in Cardiac Patient.

The use of pacemakers is necessary for patients with symptomatic bradycardia. Pacemaker implantation also acts as a life-saving procedure. However, there are several reports that patients with a pacemaker (the most widely used pacemaker is on the right ventricle, known as single chamber pacemaker) had disturbances in left ventricle contraction lead to left ventricular systolic dysfunction. Global Longitudinal Strain (GLS) Echocardiography can confirm these left ventricular disturbances. Echocardiography examination is best carried out before and after single-chamber PPM implantation. Management of incontinence following pre-pubic urethrostomy in a cat using an artificial urethral sphincter.

A 14-year-old male neutered domestic shorthair cat presented with a 4-year history of urinary incontinence following prepubic urethrostomy. Physical examination confirmed peristomal dermatitis, urine scalding and constant urine leakage. Haematology, serum biochemistry and urinalysis were unremarkable. Ultrasonography revealed a non-distended urinary bladder. An artificial urethral sphincter mechanism, with subsequent incremental inflation of the cuff 6 weeks later, resulted in complete resolution of clinical signs, return to normal urinary function and excellent quality of life. The cat was st

Xerostomia in geriatric patients: a burgeoning global concern.

Saliva plays a key role in maintaining oral homeostasis, function, and health. The prevalence of xerostomia and its consequences are rising due to the increasing aging population, the effects of some systemic diseases, medical management, and commonly-prescribed medications that reduce saliva production. When salivary function is diminished, patients are at a greater risk of developing caries, discomfort in wearing dentures, and opportunistic diseases, such as candidiasis. The psychosocial aspects of xerostomia can range from a mild effect on self-rated oral health to frustration, embarrassmen

"The death rattle" in the intensive care unit after withdrawal of mechanical ventilation in neurological patients.

The noise produced by oscillatory movements of secretions in the oropharynx, hypopharynx, and trachea during inspiration and expiration in unconscious terminal patients is often described as "the death rattle." The secretions are produced by the salivary glands and bronchial mucosa. These patients are usually too weak to expectorate or swallow the migrating secretions. Sputum usually only accumulates in these areas if there is a significant impairment of the cough reflex, as in deep coma or near death. Reported incidence of death rattle in terminal patients varied between 6 and 92%. Death ratt

[Considerations on the therapy with physiologic pacemakers].

Physiologic pacemakers offer hemodynamic benefits in comparison to conventional pacing, but demand more detailed diagnosis of the arrhythmia in need of treatment. In order to prevent endless loop tachycardias, ventriculoatrial conduction must be analyzed. Normal sinus node and atrial function should be confirmed in atrial triggered ventricular stimulation. The techniques and values of preimplantation investigations, including intracardiac electrophysiological procedures, are discussed.

Aeroplanes also fly: Analytic functionalism and the possibility of machine consciousness.

Seth mounts a rich and persuasive case that human consciousness cannot be understood without life's distinctive machinery - autopoiesis, allostatic control, predictive processing under the free-energy principle, and substrate-entangled dynamics. I agree that this undercuts the expectation that humanlike consciousness will "come for free" with more intelligent machines. However, we should also be cautious about drawing extrapolations from features of biological consciousness - even essential features - to the possibility of exotic forms of consciousness in artificial systems.

A note on the transmission of Babesia bovis (syn B argentina) by the one-host tick, Boophilus microplus.

Boophilus microplus infected with Babesia bovis were transferred artificially from one splenectomised calf to another during each moult in the parasitic life cycle of the tick. Eggs from the engorged female ticks recovered at the end of the cycle were incubated and the resulting larvae used to infest more splenectomised calves. Babesia bovis was transmitted only by the original larvae used at the commencement of the experiment and it was concluded that the protozoan parasite did not persist in an infective form in the ticks beyond the larval stage.

Diagnostic tools and methods for dermatological assessment.

Advanced clinical practitioners (ACPs) play an essential role in dermatological care but often encounter challenges due to limited training in dermatological assessments and investigations. This two-part series aims to address these gaps by offering a structured approach specifically for ACPs. Part one focused on conducting a thorough history-taking and physical examination to establish a solid foundation for diagnosis. Part two contributes to the existing literature by detailing essential diagnostic tools and emerging technologies that empower ACPs to enhance diagnostic accuracy and patient-c

Common arrhythmias and their management in the adolescent with congenital heart disease.

PURPOSE OF REVIEW: Congenital heart disease (CHD) is a relatively common congenital anomaly with a wide range of complexity. Different types of CHD carry specific risks for bradyarrhythmias, tachyarrhythmias, or both. These risks and any symptoms guide testing and monitoring strategies. Novel techniques and devices allow for individualized care incorporating short- and long-term risks and benefits. RECENT FINDINGS: Changes to approaches in pacemaker lead placement have improved the treatment of cardiac dyssynchrony and the prevention of pacing-induced cardiomyopathy. Catheter ablation is incr

Opportunities and challenges of artificial intelligence and distributed systems to improve the quality of healthcare service.

The healthcare sector, characterized by vast datasets and many diseases, is pivotal in shaping community health and overall quality of life. Traditional healthcare methods, often characterized by limitations in disease prevention, predominantly react to illnesses after their onset rather than proactively averting them. The advent of Artificial Intelligence (AI) has ushered in a wave of transformative applications designed to enhance healthcare services, with Machine Learning (ML) as a noteworthy subset of AI. ML empowers computers to analyze extensive datasets, while Deep Learning (DL), a spec

Identification of outcome domains in primary Sjögren's disease: A scoping review by the OMERACT Sjögren disease working group.

OBJECTIVES: Sjögren's disease (SjD) is a heterogenous disease with a wide range of manifestations, ranging from symptoms of dryness, fatigue, and pain, to systemic involvement. Considerable advances have been made to evaluate systemic activity or patient-reported outcomes, but most of the instruments were not able to assess all domains of this multifaceted disease. The aim of this scoping review was to generate domains that have been assessed in randomized controlled trials, as the first phase of the Outcome Measures in Rheumatology (OMERACT) process of core domain set development. METHODS: W
