High attenuation gyri CT in postasphyxial encephalopathy.

Two term infants suffering severe asphyxial events were found to have diffusely high attenuation of the cortical gyri on CT scanning within 12 days. The findings are consistent with haemorrhagic necrosis or microcalcification. This feature has rarely been described in the literature. The evolution of the CT appearance, the possible neuropathological findings and the relationship with hypoxic-ischaemic encephalopathy are discussed.

Clinicopathological correlations in postasphyxial organ damage: a donor organ perspective.

OBJECTIVES: 1) To assess pathology in the heart, kidneys, lungs, liver, and intestine in babies dying of postasphyxial hypoxic ischemic encephalopathy (HIE), and their suitability for organ transplantation. 2) To analyze the correlation between in vivo clinical markers of collateral organ damage and autopsy findings. STUDY DESIGN: Retrospective cohort study of 58 infants who died of postasphyxial HIE and had autopsies during 1985 to 1994. Collateral organ damage was evaluated clinically and pathologically, and the results compared. RESULTS: Severe pathological changes were observed in 6% to

[Delayed-onset dystonia due to asphyxia in the perinatal period].

The phenomenon of delayed-onset dystonia following presumed "static" brain injuries was described after stroke and head trauma. Burke et al. described a different category of secondary dystonia, where perinatal injury (asphyxia) caused minimal or no immediate neurological deficit, with the delay of years before dystonia emerged. This type of dystonia following perinatal injury has been termed "delayed onset dystonia due to static encephalopathy of childhood". According to the definition of dystonia, we were able to select 5 patients with the aetiologic diagnosis of perinatal asphyxia from the

Umbilical cord ulceration as a cause of hypoxic-ischemic encephalopathy: report of a case and review of the literature.

We report a case of hypoxic-ischemic injury caused by acute hemorrhage from an umbilical cord ulceration in a newborn infant with an antenatal diagnosis of small bowel obstruction. Recognition of the association between umbilical cord ulceration and small bowel obstruction may alter obstetric and delivery room management.

Differential involvement of the brain in neonatal asphyxia: a pathogenic explanation.

Multiple cystic lesions in brain parenchyma supplied by the anterior cerebral circulation is a recognized pattern of cerebral injury associated with hypoxic-ischemic encephalopathy in the term infant. This report presents a series of seven infants (gestational age, 39.3 +/- 2.8 weeks; range, 36 to 44 weeks) who developed multicystic encephalomalacia in the distribution of the anterior cerebral circulation after severe neonatal asphyxia. Cerebral imaging and pathologic studies demonstrate relative preservation of the cerebellum, brain stem, and cerebral structures supplied by the vertebrobasila

Vasopressin in cerebrospinal fluid of newborns with hypoxic-ischemic encephalopathy. Preliminary report.

Vasopressin in cerebrospinal fluid has been measured in 27 fullterm newborns with hypoxic-ischemic encephalopathy. These newborns were divided into three groups according to the degree of neurological involvement, and they have been compared with a control group of 10 newborns. Determinations of vasopressin in cerebrospinal fluid and plasma were done by RIA. The cerebrospinal fluid vasopressin in asphyxiated newborns was higher than in the control group (p < 0.001); the mean concentration in the group of newborns classified as moderate or severe hypoxic-ischemic encephalopathy was higher than

Birth asphyxia and hypoxic-ischaemic encephalopathy: incidence and severity.

Clinically significant birth asphyxia was assessed over a 3-year period in a tertiary referral hospital in Nigeria. The overall incidence was 26.5/1000 live births of whom 12.1/1000 showed severely abnormal features comprising persistent seizures and coma. There was no appreciable difference in incidence for the consecutive years of the study. There was a marked involvement of infants who had suffered intrauterine growth retardation: 51 (30.7%) of these were asphyxiated, whereas only 3% were large for gestational age. The Apgar scoring system seemed not to have compared well with the clinical

Use of activated charcoal to reduce elevated serum phenobarbital concentration in a neonate.

The course of a neonate administered activated charcoal orally for elevated phenobarbital concentration is described. The neonate required a serum concentration of phenobarbital eventually exceeding 80 micrograms/mL for seizure control. However, due to the severe CNS injury as a result of the asphyxia at birth, a diagnosis of cerebral death by electroencephalogram and apnea test was pursued. This required the phenobarbital concentration to be less than 30 micrograms/mL. Serial phenobarbital concentrations during this time indicated a 250 hour half-life, which would require two weeks for the ph

Failure to breathe at birth: causes and assessment.

A frequent misconception is that delayed onset of respiration is the result of intrapartum asphyxia. However, many factors other than asphyxia can delay the onset of respiration after delivery. To understand the effect of asphyxia on both the fetus and the neonate, and in particular in delaying the onset of spontaneous respiration, it is convenient to consider chronic partial asphyxia and acute asphyxia separately. In this article, the values of the Apgar score and cord blood pH were also evaluated.

Delayed-onset dyskinetic 'cerebral palsy--a late effect of perinatal asphyxia?

A 12-year-old girl, born full term after severe pre- and peripartal asphyxia, with transient abnormal newborn neurology and slight motor development deviations but no cerebral palsy syndrome, is described. At the age of 5, she developed a slowly progressive dyskinetic hemisyndrome, initially uncoordinated hyperkinetic movements in her left arm, dystonic posturing then supervened. Since the age of 11, her right side had also slowly become involved. As extensive examinations excluded other causes, it is concluded that this case represents a delayed onset dyskinetic 'CP-syndrome' subsequent to a

Perinatal asphyxia: clinical aspects.

This article reviews the significance of hypoxic-ischemic encephalopathy and the associated patterns of cerebral injury in the context of the most probable timing of the insult and prognosis. The evolution of the clinical features of significant hypoxic-ischemic encephalopathy, and the diagnostic value of imaging and electrophysiologic and metabolic studies are discussed. Current approaches to management are outlined.

Retinal hemorrhages in newborn.

Two hundred and fifty eight newborn babies were studied for the presence of retinal hemorrhages between 1-3 days of birth. The overall incidence of retinal hemorrhages was found to be 18.9%. It was observed that the incidence of retinal hemorrhages was higher in unassisted vaginal deliveries than in assisted births. Also, a two fold higher incidence was noted in term infants as compared to preterm babies. No association was seen with birth asphyxia.
