[Follow-up of dissection of the right coronary artery and wall of the ascending aorta after directional coronary atherectomy. A case report].

We report on a patient in whom a restenosis after two PTCA procedures was treated with directional coronary atherectomy. In the second DCA maneuvre a spiral dissection, type D, developed from ostium to restenosis with a balloon shaped part in the wall of the aorta ascendens. This part disappeared nearly completely by next day. The dissection in the right coronary artery persisted three months later until a bypass operation and was not visible after two years.

Coronary atherectomy versus balloon angioplasty before drug-eluting stent implantation in severely calcified lesions: A systematic review and meta-analysis.

BACKGROUND: Severe coronary calcification can impede stent delivery and affect long-term outcomes. Few trials have compared coronary atherectomy with balloon-first strategies for lesion preparation before drug-eluting stent (DES) implantation. METHODS: We searched MEDLINE, Embase, and the Cochrane Library through October 2025 for randomized controlled trials (RCTs) comparing coronary atherectomy with balloon angioplasty for lesion preparation before DES implantation in severely calcified coronary lesions. RESULTS: Across three RCTs, there was no significant difference in major adverse cardio…

Structural degradation of a rotational atherectomy burr following treatment of a massively calcified coronary lesion: Insights from scanning electron microscopy.

BACKGROUND: Rotational atherectomy is an established strategy for the treatment of severely calcified coronary lesions. Although reduced burr efficiency is not uncommon during complex procedures, the structural changes responsible for this phenomenon have not been investigated following rotational atherectomy of native coronary lesions. CASE: In a patient undergoing rotational atherectomy for a massively calcified native coronary lesion, we encountered failure of the burr to cross the lesion despite preserved rotational speed. Macroscopic examination suggested surface alteration of the burr. …

Drug-Coated Balloons Versus Drug-Eluting Stents Following Coronary Atherectomy in Severely Calcified Lesions: A Systematic Review and Meta-analysis.

Severely calcified coronary lesions are associated with increased risk of procedural complications, stent underexpansion, and restenosis. While atherectomy prepares vessels for treatment, drug-eluting stents (DES) are traditionally preferred over drug-coated balloons (DCB). DCBs offer a stent-free alternative that may improve vessel remodeling and may require shorter antiplatelet therapy. However, for severely calcified lesions, it remains unclear whether DCB or DES is more effective. Following PRISMA guidelines, a systematic search of PubMed, Embase, Web of Science, Scopus, and Cochrane Centr…

Efficacy and Safety of Excimer Laser Coronary Atherectomy in Acute Coronary Syndrome: Real-World Evidence From a Propensity-Adjusted Analysis.

BACKGROUND: The clinical effectiveness of excimer laser coronary atherectomy (ELCA) in acute coronary syndrome (ACS) remains uncertain due to limited evidence from prior studies. AIMS: To evaluate the clinical characteristics of patients who underwent primary PCI with ELCA in the setting of ACS, and to asses the impact of ELCA on clinical outcomes. METHODS: This retrospective study included 586 consecutive ACS patients who underwent primary PCI, stratified by ELCA use (ELCA-group, n = 256; non-ELCA group, n = 330). Propensity score matching was performed to compare clinical outcomes between …

Combined Rotational excimer lASER coronary atherectomy (RASER) in non-crossable, non-dilatable coronary artery disease: observations from a single center.

BACKGROUND: Balloon non-crossable stenoses represent a challenging subset of coronary artery disease (CAD). They are clinically associated with patients who are older, frailer, and with multi-morbidities, and angiographically with increased tortuosity and coronary artery calcification. Combined rotational (RA) excimer laser coronary atherectomy (ELCA), or RASER, may facilitate stent delivery and deployment in non-crossable, non-dilatable severely calcified lesions. In this study, we assessed preliminary safety and efficacy of the RASER hybrid technique. METHODS: RASER feasible percutaneous co…

Intravascular Lithotripsy as a Novel Treatment Method for Calcified Unprotected Left Main Diseases-Comparison to Rotational Atherectomy-Short-Term Outcomes.

BACKGROUND: The unprotected calcified Left Main disease represents a high-risk subset for percutaneous coronary intervention (PCI), and it is associated with a higher number of periprocedural complications and an increased rate of in-stent thrombosis and restenosis. Adequate lesion preparation plays a crucial role in achieving a favorable PCI outcome. Rotational Atherectomy (RA) is a well-established plaque-modifying method; nevertheless, the data regarding the effectiveness of RA in LM diseases is scarce. Recently, the novel ShockWave-Intravascular-Lithotripsy(S-IVL) device has been introduce…

How Much Debulking with Atherectomy is Enough When Treating Infrainguinal Arterial Interventions? The Balance Between Residual Stenosis and Adventitial Injury.

Atherectomy is an effective vessel prepping device but not all atherectomy devices are equal. The depth of vessel injury and residual narrowing vary considerably among atherectomy devices with significant implications on outcome. Precision imaging is critical to optimize outcome using atherectomy as a vessel prepping technique. Prospective trials need to test the hypothesis that precision imaging has a significant impact on how operators approach the treatment of infrainguinal arterial disease.

Stentless percutaneous coronary intervention with directional coronary atherectomy and drug-coated balloon angioplasty in worsening angina patients with metal allergies.

UNLABELLED: Metal allergy is a concern in percutaneous coronary intervention (PCI) with stent implantation because of its potential association with poor cardiovascular outcomes, such as stent thrombosis and recurrent in-stent restenosis requiring revascularization. Although stentless PCI with drug-coated balloon (DCB) angioplasty is theoretically useful for patients with metal allergies, DCB angioplasty alone for huge plaques in large vessels may yield inadequate luminal enlargement and coronary deep dissection, leading to insufficient results. Directional coronary atherectomy (DCA) is effect…

Network Meta-Analysis of All Available Regimens Based on Drug-Coated Balloon Angioplasty and Laser Atherectomy for Femoropopliteal In-Stent Restenosis.

PURPOSE: Drug-coated balloon (DCB) angioplasty and laser atherectomy (LA) have been frequently utilized to treat femoropopliteal in-stent restenosis (ISR); however, no studies have concurrently compared available regimens, including DCB, LA+DCB, and LA + plain balloon angioplasty (PB). Therefore, we conducted this network meta-analysis to determine whether there were significant differences in outcomes among these regimens. MATERIALS AND METHODS: A comprehensive search was conducted in PubMed, EMBASE, and the Cochrane library to identify all randomized controlled trials comparing DCB or LA-ba…
