Wide and narrow QRS tachycardias: What is the mechanism?

A 50-year-old lady was referred for radiofrequency catheter ablation of narrow QRS tachycardia that was terminated with intravenous adenosine. Twelve-lead Electrocardiogram (ECG) was normal during sinus rhythm. The electrophysiological study showed an Atrio-Hisian (AH) interval of 104 ms and Hisio-Ventricular (HV) interval of 45 ms during sinus rhythm. Atrial pacing reproducibly induced regular broad (left bundle branch block morphology) and narrow QRS tachycardias. A spontaneous premature ventricular ectopic from right ventricular apex has resulted in transition of the tachycardia from wide t

Recording the Accessory His Bundle Potential from a Right Atriofascicular Accessory Pathway.

The author discusses the case of a 42-year-old man with a long history of episodes of rapid palpitations. Recordings from the proximal end of a right atriofascicular accessory pathway at the lateral tricuspid annulus are discussed. There was successful catheter ablation of the right atriofascicular accessory pathway, without recurrence of tachycardia.

Cardiac pacing in the elderly.

Sinus node disease and atrioventricular block are common etiologies of symptomatic bradyarrhythmias in the elderly and remain the leading indications for permanent pacemaker implantation. In fact, the vast majority (>80%) of all pacemakers are implanted in the elderly. Whereas indications of pacemaker therapy have been largely unchanged over the past several years, several questions, such as differences in pacemaker mode selection, remained unanswered. Recent large, randomized, multicenter trials have evaluated the benefits of pacemaker therapy in sinus node dysfunction and acquired atrioventr

[Nonpharmacological therapy of supraventricular arrhythmia].

Between 1985 and 1990, 1242 patients with supraventricular arrhythmias were followed-up at our Institution. Six hundred and twenty patients had atrial fibrillation or flutter; 7 of them (1%) underwent modulation of atrioventricular conduction. Four hundred and twenty-eight patients had ventricular preexcitation; in 23 (5%) surgical or transcatheter ablation of an accessory pathway were performed. Atrioventricular node reentrant tachycardias were diagnosed in 111 patients; 8 patients (7%) underwent antitachycardia pacemaker implantation. Surgical and catheter ablative techniques eliminate the s…

Ultrastructural and electrophysiologic changes of experimental acute cardiac lymphostasis.

Experimental impairment of cardia lymph flow in dogs produced histologic and electrophysiologic changes in the heart. Interstitial edema, lipid swelling of myofibrils, dilatation of lymph vessels, and fibrinoid degeneration of small coronary arteries occurred in and near the sinus node and the atrioventricular conduction system. On electrical stimulation of the heart, significant shortening of the atrial and ventricular effective refractory periods, increases in the sinus node recovery time and in the atrioventricular conduction time, and ventricular extrasystoles and ventricular fibrillation

Verapamil: an effective calcium blocking agent for pediatric patients.

Verapamil is a slow-channel calcium-blocking agent that has been released recently by the Food and Drug Administration for treatment of cardiac dysrhythmias in all age groups. Its primary action is to slow conduction in the atrioventricular node, thereby abolishing those types of supraventricular tachycardia using the atrioventricular node as a part of the reentry circuit or slowing the ventricular rate in someone with atrial flutter. Other investigations have shown that it is likely to relieve left ventricular outflow obstruction in patients with hypertrophic obstructive cardiomyopathy. Becau…

Beneath the surface: Complete atrioventricular block combined with Wolff-Parkinson-White syndrome and sick sinus syndrome.

The combination of Wolff-Parkinson-White (WPW) syndrome, sick sinus syndrome (SSS), and complete atrioventricular block (AVB) is seldom repo … lowed by the ablation of the accessory pathway. AVB may be masked by ventricular preexcitation, and atrioventricular node function should be evaluated before ablation.

[Pre- and postoperative study of arrhythmia in atrial septal defects (ostium secundum and sinus venosus)].

An electrophysiological study was carried out pre- and postoperatively in 25 patients (children and adults) presenting with an atrial septal defect of the ostium secundum type (OSASD). The purposes of the study were to find out whether early electrophysiological abnormalities were present in children, to compare the results obtained in children and in adults and to evaluate the impact of surgery by comparing preoperative and late postoperative electrophysiological data. Patients were divided into two groups. Group I was composed of 11 children (mean age 8.18 +/- 1.03 years) with a mean pulmona

Novel method to avoid serious injurious effects on the atrioventricular nodal (AVN) conduction during catheter ablation of the AVN slow pathway utilizing cryofreezing energy.

BACKGROUND: Slow pathway elimination of the atrioventricular node (AVN) is essential to treat AVN reentrant tachycardia (AVNRT). However, injury to the AVN conduction (IAVN) is one of the serious complications. Cryofreezing energy is expected to reduce the incidence of IAVN. This study aimed to investigate the usefulness of a novel method to avoid IAVN during cryoablation of AVNRT. METHODS: A total of 157 patients (average age, 65.8 years; male, 71) suffering from AVNRT were included. Once the AVNRT terminated during cryo-ablation, then rapid atrial constant pacing (RACP) was performed during…

Anatomy for ablation of atrioventricular nodal reentry tachycardia and accessory pathways.

The atrioventricular (AV) valve plane and the central septum are of particular importance for electrophysiological diagnosis and interventional therapy of supraventricular tachycardias because accessory electrical connections of various types may be present in addition to the specific conduction system. Although modern 3D electroanatomic reconstruction systems including high-density mapping can be of great assistance, detailed knowledge of the anatomic structures involved, their complex three-dimensional arrangement, and their electrical properties in conjunction with electrophysiological feat

[Cardiac pacemakers in children. 15 years' experience].

Between 1971 and 1986, 85 pacemakers were implanted at the St Justine Hospital, Montreal, in 57 young patients (25 girls, 32 boys) then aged from one day to 23 years (mean 10.3 years). The patients were followed up for periods ranging from 15 days to 13.5 years (mean 4.5 years); 119 epicardial electrodes were positioned by thoracotomy in 52 patients and by sternotomy in 5 patients; the 85 pacemaker cases were placed in the left retroperitoneal cavity. The pacemakers were programmed in modes VVI (28), AAI (1) and DDD (28). The indications for pacemaker implantation were: complete atrioventricul

Morphological Substrates for Atrial Arrhythmogenesis in a Heart With Atrioventricular Septal Defect.

Due to advances in corrective surgery, congenital heart disease has an ever growing patient population. Atrial arrhythmias are frequently observed pre- and post-surgical correction. Pharmaceutical antiarrhythmic therapy is not always effective, therefore many symptomatic patients undergo catheter ablation therapy. In patients with atrioventricular septal defects (AVSD), ablation therapy itself has mixed success; arrhythmogenic recurrences are common, and because of the anatomical displacement of the atrioventricular node, 3-degree heart block post-ablation is a real concern. In order to develo…

The cardiac conduction system: A narrative review.

The cardiac conduction system (CCS) is a hierarchically organized yet structurally heterogeneous network of specialized tissues that initiat … ratrial bundle and posterior connections supporting rapid activation without strict insulation. The atrioventricular node enables physiological delay through slow, yet safe conduction shaped by specialized cellular architecture, connexin gradients, and dual-pathway organization. Distal to the node, the atrioventricular bundle and its branches - collectively known as th …

[Study of atrial conduction tissue in the 17 mm V-C human embryo. Morphological contribution to the pathogenesis of sinoauricular node dysfunction].

Is there a specialised conduction pathway connecting the sinoatrial and atrioventricular nodes within the crista terminalis? As lesions of this zone can cause sinus node dysfunction and the sick-sinus syndrome, we decided to study this structure at the beginning of its embryonic development. Serial 10 micronthichness transverse sections of a 17 mm vertex-coccyx (Stage 19) human embryo were studied by light microscopy, anatomical reconstruction (Born) and morphometry. The region of the sinus venosus and venous valves, of the superior vena cava and the atrioventricular region were drawn and reco…
