Effect of chronic benzodiazepine and benzodiazepine receptor agonist use on sleep architecture and brain oscillations in older adults with chronic insomnia.

STUDY OBJECTIVES: Insomnia in older adults is associated with widespread benzodiazepine (BZD) and benzodiazepine receptor agonist (BZRA) use despite evidence that chronic use disrupts sleep regulation and cognition. Little is known about BZD/BZRA effects on Non-rapid eye movement (NREM) slow oscillations (SO), spindles and their coupling, which is crucial for memory in older adults. Our objective was to investigate the effects of chronic BZD/BZRA use on sleep macro-architecture, electroencephalogram (EEG) relative power, SO and spindle characteristics and coupling. METHODS: After habituation

Characteristics of high-dose benzodiazepine use: nationwide cohort study on new benzodiazepine users with 5-year follow-up.

BACKGROUND: A nationwide register-based cohort study from Finland including 48 124 incident benzodiazepines and related drug (BZDR) users aged 18-65 years who initiated use in 2006 and were not dispensed BZDRs during 2004-2005. The follow-up was 5 years or until death, whichever occurred first. AIMS: To investigate sociodemographic and clinical factors associated with high-dose use of BZDRs (i.e. Z-drugs) among new BZDR users. METHOD: The temporal BZDR dose was calculated as a point estimate every 6 months after initiation as defined daily doses (DDDs) per day, based on the PRE2DUP method (a…

Differences in detected safety signals between benzodiazepines and non-benzodiazepine hypnotics: pharmacovigilance study using a spontaneous reporting system.

Introduction: In recent years, there has been an increasing number of people who feel sleep-deprived owing to sudden changes in the social environment. Patients prescribed benzodiazepine-based hypnotics (BZ drugs) also develop movement disorder action and memory disorders as adverse events (AEs), and they have further problems such as dependency and tolerance because of long-term use. Therefore, the use of non-benzodiazepine-based hypnotics (Z-drugs) is recommended for patients with insomnia. However, as AEs have also been reported for Z-drugs, it is important to identify these when switching

Chronic treatment with a benzodiazepine agonist in vivo increases the actions of the benzodiazepine partial inverse agonist, FG7142, on the hippocampal slice in vitro.

We have shown previously that chronic treatment of mice with a benzodiazepine agonist, flurazepam, increased the pharmacological actions of the partial inverse agonist, FG7142. We have investigated the neurophysiological basis for this using extracellular recordings of evoked field potentials in area CA1 of isolated hippocampal slices. The slices were prepared 48 h after the end of the chronic in vivo treatment, a time when no evidence of residual benzodiazepine agonist activity was found in the CNS. During perfusion with standard Ringer solution, no significant differences were seen between t

Pre- and Postoperative Prescription of Opioids and Benzodiazepines in the US.

BACKGROUND: Postoperative opioid and benzodiazepine use often extends beyond recovery, yet research on surgical prescribing tends to focus on opioid-naive patients. Less is known about those already using these medications preoperatively, who face different risks and care needs. STUDY DESIGN: Using a national US claims database (PearlDiver, 2010 to 2022), we identified insured adults receiving 1 of 18 common surgeries. We examined patterns of filled prescriptions for opioids and benzodiazepines before and after surgery, including (1) opioids without benzodiazepines (opioid monotherapy), (2) b…

Hypersensitivity Reactions to Benzodiazepines Used for Perioperative Premedication: A Narrative Review.

Background: Hypersensitivity reactions to benzodiazepines, although uncommon, represent a clinically relevant issue in perioperative practice. Benzodiazepines are widely used medications with anxiolytic, sedative, and anticonvulsant properties. The objective of this review is to synthesize current knowledge on benzodiazepine hypersensitivity, focusing on underlying mechanisms, clinical manifestations, diagnostic considerations, and strategies for management and prevention in the perioperative setting. Methodology: A narrative synthesis of the current literature on hypersensitivity reactions to…

Benzodiazepines on Admission UDS: Indicator of Worse Outcomes After Geriatric Falls.

BackgroundUnintentional falls are the leading cause of injury among adults 65 years and older. It's believed that adverse benzodiazepine side effects increase the risk and frequency of falls. We hypothesize that geriatric patients taking benzodiazepines prior to admission experience worse clinical outcomes compared to those without preadmission use.MethodsUsing our level 1 trauma database, patients 65 years or older admitted following a ground level fall were divided into 2 groups, benzodiazepines positive on admission urine drug screen (+Benzo) and negative (-Benzo). Primary outcomes included…

Potential Neuroendocrine Disruption by Benzodiazepines and Its Impact on Male Fertility.

Neuroactive substances have emerged as potent endocrine disruptors affecting male reproductive health through central and peripheral mechanisms. Among these, benzodiazepines, which are widely used for the management of anxiety, insomnia, epilepsy, and neuromuscular disorders, are known as GABA A (gamma-aminobutyric acid type A) receptor positive allosteric modulators. These are traditionally believed to have central nervous system effects. However, recent experimental findings suggest that benzodiazepines may have peripheral effects through testicular homeostasis and the hypothalamic-pituitary…

Successful symptom-based management of active withdrawal from multiple illicit benzodiazepines.

Benzodiazepines are among the most often misused medications. Medication tapering is required to prevent lethal withdrawal symptoms associated with abrupt cessation. The authors report a case of a male in his early 20s who presented following a seizure precipitated by the abrupt cessation of multiple illicit substances. Further history identified bromazolam, flubromazepam, alcohol, kratom, tianeptine and phenibut to be the predominant substances involved in his presentation. The lack of approved long-acting benzodiazepine equivalents for bromazolam and flubromazolam and kratom's inhibition of …

Harm reduction approaches for the use of benzodiazepines: a scoping review.

BACKGROUND: Benzodiazepines are widely prescribed but are associated with significant risks, particularly when used long-term. The anxiolytic and hypnotic properties of these medications increase their risk of dependence, which can lead to nonmedical and illicit use. Illicit use further compounds these harms, particularly with the emergence of potent novel benzodiazepines on the unregulated market. While tapering remains the standard treatment, not all individuals seek discontinuation. In such cases, harm reduction becomes a key approach to minimise associated risks. This review aimed to ident…

Opioids and benzodiazepines in oncology: Perspectives on coprescribing and mitigating risks.

INTRODUCTION: Opioids and benzodiazepines are commonly prescribed for cancer symptoms. In combination, they can increase the risk of adverse events, particularly for older adults with multimorbidity, who represent most patients with cancer. We aimed to understand cancer care providers' practices for opioid and benzodiazepine coprescribing and mitigating potential harms. MATERIALS AND METHODS: We interviewed oncology and palliative care providers from two health systems. Interviews focused on attitudes about and current practices for coprescribing opioids and benzodiazepines. We analyzed inter…

Driving performance of long-term users of sedating antidepressants and benzodiazepines.

OBJECTIVE: Using benzodiazepines and certain antidepressants is associated with an increased risk of motor vehicle crashes due to impaired driving skills. Hence, several countries prohibit people who use these drugs from driving. Traffic regulations for driving under the influence of these drugs are, however, largely based on single-dose studies with healthy participants. The effects of drugs on chronic users may be different because of potential development of tolerance or by adapting behavior. In this study, we test the effects of anti-depressants, hypnotics, or anxiolytics use on driving pe…

A detox dilemma beyond benzodiazepines; clonidine's quandary in alcohol withdrawal management.

BACKGROUND AND OBJECTIVES: Benzodiazepines are the primary method of treatment of alcohol withdrawal, though the American Society of Addiction Medicine guidelines also include alternative agents for consideration. Observations in a Department of Veterans Affairs (VA) psychiatric emergency room noted consistent benzodiazepine use with an overall lack of use of alternative agents, even with low Clinical Institute Withdrawal Assessment for Alcohol (CIWA) scores and in the absence of other concerning symptoms. Due to concerns of potential more-than-necessary benzodiazepine use, we analyzed adjunct…

Long-term use of benzodiazepines in chronic insomnia: a European perspective.

Chronic insomnia occurs in ~10% of the general population and has numerous negative health effects. The recommended first line treatment of cognitive behavior therapy for insomnia is not widely available for patients in Europe, so pharmacotherapies such as benzodiazepine receptor agonist agents (benzodiazepines and Z-drugs) are commonly used. However, their use is only recommended for ≤4 weeks due to unproven long-term efficacy in treatment of chronic insomnia, and the risk of tolerance, and the potential for dependence and misuse. In Europe, recommendations limiting the use of benzodiazepines…

Evidence of the emergence of illicit benzodiazepines from online drug forums.

Illicit or 'designer' benzodiazepines are a growing contributor to overdose deaths. We employed natural language processing (NLP) to study benzodiazepine mentions over 10 years on 270 online drug forums (subreddits) on Reddit. Using NLP, we automatically detected mentions of illicit and prescription benzodiazepines, including their misspellings and non-standard names, grouping relative mentions by quarter. On a collection of 17 861 755 posts between 2012 and 2021, we searched for 26 benzodiazepines (8 prescription; 18 illicit), detecting 173 275 mentions. The rate of posts about both prescript…
