BACKGROUND: Congenital tracheobiliary fistula is a rare developmental anomaly with a persistent communication between the biliary system and the trachea.
CHARACTERISTICS: A 7-day-old baby with severe respiratory distress and aspiration pneumonia.
OUTCOME: Tracheobilliary fistula identified on bronchoscopy. Open surgical excision of fistula was followed by improvement.
MESSAGE: This condition should be considered in the differential diagnosis of intractable aspiration pneumonia.

A 47-year-old woman complaining of diarrhea and vomiting was admitted on the suspicion of gallstone ileus 4 days after onset. Upper gastrointestinal radiography by Gastrografin showed a cholecystoduodenal fistula. Laparoscopic-assisted simple enterolithotomy was performed. The omentum was severely adherent to the gall bladder and fistula, though biliary surgery was not performed. Without second look operation, for cholecystoduodenal fistula closed spontaneously.

Mirizzi syndrome (MS) is an uncommon complication of cholelithiasis, in which an impacted gallstone causes extrahepatic biliary obstruction via extrinsic duct compression. Advanced disease may progress to cholecystobiliary fistula formation. We report the case of a 75-year-old man with cardiometabolic comorbidities who presented with cholestatic liver biochemistry and imaging findings consistent with chronic calculous cholecystitis. Computed tomography and magnetic resonance cholangiopancreatography revealed a distended gallbladder packed with stones, hilar biliary narrowing, and a variant bil…

Small bowel obstruction due to ingested fruit pits is rare and often underrecognized. In exceptional cases, these foreign bodies may mimic the clinical and imaging features of gallstone ileus, a complication of cholelithiasis involving biliary-enteric fistula formation and gallstone migration into the intestinal lumen. Such presentations may lead to misdiagnosis and inappropriate management. This case highlights an uncommon but clinically relevant diagnostic pitfall: a pseudo-gallstone ileus caused by an impacted apricot pit in a patient with a prior ileocolic anastomosis. An Italian Caucasian

Portobiliary fistula is a rare complication of percutaneous biliary drainage. The authors report its occurrence in four patients who had multiple percutaneous biliary procedures. One patient was treated by simple tamponade over several days, the other three by temporary balloon tamponade and permanent coil occlusion.

Background and Clinical Significance: Biliary tract injuries are a recognized complication of laparoscopic cholecystectomy. Early diagnosis … y. The patient underwent a Roux-en-Y hepaticojejunostomy and subsequently developed a postoperative biliary fistula, which was managed with percutaneous drainage. A biodegradable biliary stent was later placed to address a stricture and minimize the need for future interventions. All patients had non-resectable HAE. Indications for ERCP were biliary fistula in seven patients, obstructive jaundice in five patients and cholangitis in one patient. Endoscopic sphincterotomy (ES) was carried out in 12 patients, and in one patient with biliary leakage, a stent was inserted into the right hepatic branch. ERCP …

Liver transplantation is the mainstay of treatment for end -stage liver disease. In this setting, endoscopic retrograde cholangiopancreatography is commonly used to assist T tube removal or to treat anastomotic biliary strictures. Iatrogenic portobiliary fistula is a rare but serious complication of endoscopic retrograde cholangiopancreatography, especially in the post-transplant setting. We describe the case of a 44 -year -old male patient with an iatrogenic Iatrogenic portobiliary fistula after an endoscopic retrograde cholangiopancreatography -assisted T tube removal and plastic biliary ste…

BACKGROUND: Internal biliary fistulas are uncommon complications of chronic cholelithiasis, with Cholecystogastric fistula being particularly rare. Due to their non-specific clinical presentation, they are often diagnosed intraoperatively.
CASE DESCRIPTION: A 54-year-old male presented with a 7-day history of intermittent right upper quadrant pain. Preoperative computed tomography (CT) revealed pneumobilia and a suspected communication between the gallbladder and the gastric antrum. Subsequent gastroscopy confirmed the diagnosis, identifying a mucosal depression in the posterior gastric antru…

We suggest that the natural history of Mirizzi syndrome does not end with a cholecystobiliary fistula but that the continuous inflammation in the triangle of Calot may result in a complex fistula involving not only the biliary tract but also the adjacent viscera.

Endoscopic retrograde cholangiopancreatography (ERCP) was carried out on 19 patients with hydatid cyst of the liver, either before or after operation. Cholangiographic findings included biliary tract obstruction in 8 cases, intrinsic compression in 4 cases, laminated membranes or daughter cysts in the biliary tree in 3 cases, perforation into the biliary tract in 2 cases, stricture in the biliary ductal system in 1 case, and biliary fistula in the remaining case. Biliary tract obstruction was secondary to chemical cholangitis by formalin in 2 patients. ERCP is a useful procedure in patients wi…

BACKGROUND: Cholecystoenteric fistula (CEF) is a rare complication of biliary disease, characterized by an abnormal connection between the gallbladder and the gastrointestinal tract. It typically arises in the setting of chronic cholelithiasis or inflammation, resulting in the erosion of the gallbladder wall into adjacent gastrointestinal organs. Although imaging modalities like computed tomography (CT), magnetic resonance imaging (MRI), and endoscopic retrograde cholangiopancreatography (ERCP) have advanced, preoperative diagnosis remains difficult. Post-cholecystectomy CEF is exceptionally r

INTRODUCTION: Gallstone ileus, a rare complication of cholelithiasis and cholecystitis, is a relatively rare cause of alimentary tract obstruction. It is usually associated with a cholecystoenteric fistula through which a gallstone has passed into the gastrointestinal tract. Cholecystoenteric fistula uncommonly closes spontaneously, the period between formation and closure having rarely been reported. In addition, endoscopic detection of cholecystoenteric fistulous closure has seldom been reported.
PRESENTATION OF CASE: We report a 51-year-old Japanese man with gallstone ileus in whom spontan

BACKGROUND: Incisional hernias (IH) are a significant postoperative complication with profound implications for patient morbidity and healthcare costs. The relationship between IH and perioperative factors in pancreatic surgery, with particular attention to preoperative biliary stents and pancreatic fistulas requires further exploration.
METHODS: This retrospective observational study examined adult patients who underwent open pancreatic surgeries via midline incision at a high-volume tertiary hepatopancreatobiliary center from January 2008 to December 2021. The study focused on IH incidence

BACKGROUND: EUS-guided biliary drainage (EUS-BD) with the use of a fully covered metal stent can result in a large-diameter fistula between the bile duct and the duodenum or stomach. This sustainable fistula may constitute a new endoscopic route to the bile duct.
OBJECTIVE: To assess the feasibility of performing endoscopic procedures through EUS-guided choledochoduodenostomy or hepaticogastrostomy with a fully covered self-expandable metal stent (FCSEMS).
DESIGN: Observational pilot study.
SETTING: Tertiary-care referral center.
PATIENTS: This study involved 3 consecutive patients who und

We encountered a very rare case of biliopancreatic fistula with portal vein thrombosis caused by pancreatic pseudocyst. A 57-year-old man was referred to our hospital because of abdominal pain, obstructive jaundice, and portal vein thrombosis due to acute pancreatitis. Computed tomography showed a 7-cm-diameter pseudocyst around the superior mesenteric vein extending towards the pancreatic head, dilatation of the intrahepatic bile duct, and portal vein thrombosis. Endoscopic retrograde pancreatography revealed a main pancreatic duct with a pseudocyst communicating with the common bile duct. Af
