Percutaneous tricuspid valve therapies: the new frontier.

Moderate-to-severe tricuspid regurgitation (TR) affects ∼1.6 million patients in the USA, of whom only 8000 undergo tricuspid surgery annually; this results in an extremely large number of untreated patients with significant TR. Therefore, there is a large unmet clinical need for patients with severe TR who are not referred for conventional surgery, mainly due to expected high surgical risk. Percutaneous procedures are an attractive alternative to surgery for patients deemed to be high-risk surgical candidates. Whereas over the past few years, the development and clinical use of percutaneous a

Health technology assessments as a mechanism for increased value for money: recommendations to the Global Fund.

The Global Fund is experiencing increased pressure to optimize results and improve its impact per dollar spent. It is also in transition from a provider of emergency funding, to a long-term, sustainable financing mechanism. This paper assesses the efficacy of current Global Fund investment and examines how health technology assessments (HTAs) can be used to provide guidance on the relative priority of health interventions currently subsidized by the Global Fund. In addition, this paper identifies areas where the application of HTAs can exert the greatest impact and proposes ways in which this

Functional foods and dietary supplements: products at the interface between pharma and nutrition.

It is increasingly recognized that most chronic diseases of concern today are multifactorial in origin. To combat such diseases and adverse health conditions, a treatment approach where medicines and nutrition complement each other may prove to be the most successful. Within nutrition, apart from (disease-related) dietetic regimes, an increasing number of functional foods and dietary supplements, each with their own health claim, are marketed. These food items are considered to be positioned between traditional foods and medicines at the so-called 'Pharma-Nutrition Interface'. This paper encom

Future directions in knee replacement.

The use of artificial joints for the treatment of osteoarthritis is expected to expand considerably over the next decade. While newer technologies can offer yet further improvements in total knee systems, implementation will be strongly affected by the need to satisfy apparently competing requirements. Patients expect quicker rehabilitation, improved performance, and lifelong durability; on the other hand, economic constraints require a reduction in cost for each procedure, as well as early intervention and preventative measures, while there is increased pressure from health care systems to us

New Swedish occupational standards for some organic solvents.

In 1987, the Swedish government requested a unique investigation of the consequences of a 50% reduction of permissible exposure levels (PEL) for organic solvents. As a result, 29 solvents were investigated and for 25 of them a reduction--if not to 50%--was suggested. For 13 of the solvents, there existed scientific arguments for a reduction of the PEL. For 12 solvents, no specific biomedical information was available. Presumed interactions between solvents may have influenced the decisions, but technological/economic feasibility criteria may also have been used by the regulators. This suggests
