The coagulopathy of trauma is a syndrome of non-surgical bleeding from mucosal lesions, serosal surfaces, and wound and vascular access sites associated with serious injury, hypothermia, acidosis, hemodilution, and occasionally with classic disseminated intravascular coagulation (DIC). It can be largely explained by the effects of cold on platelet function, the effect of pH on coagulation factor activity, and the dilutional effects of resuscitation fluids and conventional blood products. DIC occurs acutely after trauma when brain, fat, amniotic fluid, or other strong thromboplastins enter the

Nineteen patients with hereditary nephritis - members of six families - were studied for haemostasis on 11 occasions by the use of four capillary tests, 14 different blood coagulation tests and two different platelet function tests. Systemic capillaropathy was demonstrable in 8 out of 15 cases of the non-uraemic, and in all the 5 cases of the uraemic group. No completely normal blood coagulation was found in any of the cases and the coagulation disorders were never of exclusively haemorrhagic pattern. On the evidence of the complex studies, hypercoagulability as well as hyper-hypocoagulability

The discovery of TMEM173/STING-dependent innate immunity has recently provided guidance for the prevention and management of inflammatory disorders. Here, we show that myeloid TMEM173 occupies an essential role in regulating coagulation in bacterial infections through a mechanism independent of type I interferon response. Mechanistically, TMEM173 binding to ITPR1 controls calcium release from the endoplasmic reticulum in macrophages and monocytes. The TMEM173-dependent increase in cytosolic calcium drives Gasdermin D (GSDMD) cleavage and activation, which triggers the release of F3, the key in

INTRODUCTION: Mixing tests in activated partial thromboplastin time (APTT) are used for the differentiation between lupus anticoagulants (LA), coagulation inhibitors, and factor deficient samples with APTT prolongation. However, the indexes for the differentiation have not been established. The present study aimed to develop new mixing test indexes for the differentiation.
METHODS: Twenty-six LA-positive, 8 progressive coagulation factor VIII inhibitor, and 35 coagulation deficient samples were employed. APTT were measured for normal plasma, patient plasma, and mixing plasma prepared at a rat

Because von Willebrand factor (VWF) levels increase during pregnancy, many women with VWD, though not requiring support with hemostatic agents, are at increased risk for delayed postpartum hemorrhage as coagulation factor levels fall to their prepregnancy levels in the puerperium. Women with moderate or severe disease or complicated pregnancies are best served by delivering at a center with an obstetrician, hematologist, and anesthesiologist experienced in managing coagulation disorders. In addition, on-site laboratory facilities with specialized coagulation testing capability, pharmacy, and b

An internal jugular venous thrombus in an astronaut was first identified in 2020 following a two-month microgravity exposure. This raised concerns about thromboembolic events (TE) during spaceflights. Studies have suggested that microgravity can induce changes in blood composition, venous flow and endothelial dysfunction, which might all contribute to a hypercoagulable state. However, whether these proposed mechanisms translate into a clinically significant increase in TE risk remains unclear since, even though humans have spent >200 person-years in space, no studies of blood coagulation in mi

Rocky Mountain spotted fever and Mediterranean spotted fever are rickettsial infections primarily of endothelial cells that normally have a potent anticoagulant function. As a result of endothelial cell infection and injury, the hemostatic system is perturbed and shows changes that vary widely from a minor reduction in the platelet count (frequently) to severe coagulopathies, such as deep venous thrombosis and disseminated intravascular coagulation (rarely). Changes favoring a hypercoagulable state include endothelial injury and release of procoagulant components, activation of the coagulation

A 4-year-old child living with his parents in Kolokani, a town about 100 km from Bamako, was bitten on the left index finger by a snake of the species Echis ocellatus (fonfoni in local language). After 2 weeks of traditional treatment, local complications were observed. The child was admitted to the Néné clinic in Kati (Mali) on 19 July 2022. The signs observed were correlated with the degree of envenomation and the whole blood coagulation test showed coagulation disorders, which justified the administration of antivenom. Necrosis of the whole index finger required amputation of the finger, wh

In the first part of the medical education article on haemostatic disorders in ENT patients the basic physiology of haemostasis and main diagnostic tools were presented and discussed. The second part presents disorders of the coagulation system, thrombocyte function and blood vessels with special emphasis on clinical practice in ENT surgery. In this context, haemophilia A and B, von Willebrand disease and different forms of thrombocytopenia are of main clinical importance. Some underlying diseases such as malignomas, renal and hepatic insufficiency in combination with drug therapy (e.g.anticoag

BACKGROUND AND OBJECTIVES: The role of viscoelastic haemostatic assays (VHAs) versus conventional coagulation tests (CCTs) in chronic liver disease (CLD) in the emergency department (ED) is undefined. We aimed to characterize VHA profiles and concordance with CCT.
MATERIALS AND METHODS: Patients with CLD presenting to EDs (January 2016-August 2023) were included. Post-transfusion results were excluded. VHA was categorized as hypo/normo/hypercoagulable using manufacturer ranges. CCT-coagulopathy was defined as international normalized ratio (INR) >1.5, platelets <50 × 109/L or fibrinogen <1.0

While the conventional understanding of haemostatic plug formation is that coagulation proceeds efficiently on the surface of activated platelets at the vascular injury site to form a robust haemostatic plug, this understanding does not explain the clinical reality that platelet dysfunction results in a mild bleeding phenotype, whereas coagulation disorders exhibit severe bleeding phenotypes, particularly in deep tissues. Here, we introduce an in vivo imaging method to observe internal bleeding and subsequent haemostatic plug formation in mice and report that haemostatic plug formation after i

INTRODUCTION: Absence of pre-hospital coagulation tests challenges prompt management of hemostasis after trauma. The Viscoelastic Coagulation Monitor (VCM, Entegrion, Durham, NC) is a hand-held coagulation test for point-of-care. We evaluated VCM in a translational swine polytrauma model, hypothesizing that VCM correlates with a laboratory reference method, the TEG 5000 (Haemonetics, Boston, MA), and can identify coagulopathic phenotypes relevant to trauma. Our secondary hypothesis was that pre-warming of VCM disposable test cartridges using a heating plate versus pre-warming of cartridges by

Breast cancer is the primary cause of cancer-related deaths in women, especially in developing nations where early detection and comorbidity management are challenging. Coagulopathy significantly enhance the disease severity, but its impact on Ethiopian patients is unclear. Therefore, this study aims to investigate and compare coagulation profiles in chemotherapy-experienced, chemotherapy-naive breast cancer patients, and healthy controls. An Institutional-based comparative cross-sectional study was conducted at the University of Gondar and Felege Hiwot Comprehensive Specialized Hospitals from

Trauma-induced coagulopathy (TIC) induces anticoagulation and increases bleeding mortality. Inflammation and oxidative stress play an unknown role in TIC. We examined plasma from injured human trauma patients presenting to the Emergency Department compared to healthy controls to elucidate the contribution of inflammation and oxidative stress to anticoagulation during TIC. Trauma patients demonstrated coagulopathy by prolongation of clotting time assays and decreased thrombin generation in addition to increased pro-inflammatory cytokines and increased markers of oxidative stress. Clotting facto

Sickle cell disorders, such as Hb SS and Hb SC, are associated with a hypercoagulable state that may contribute to the vaso-occlusive episodes observed in the disorders. To what extent increased coagulation activity occurs in individuals with sickle cell trait has had limited study. Because such information may help clarify clinical and pathologic findings that may occur in these individuals and may be useful in clarifying the hypercoagulable state in sickle cell disease, we have examined individuals with Hb AS to determine the extent that increased coagulation activity does occur. We measured

Anaphylaxis is an unpredictable hypersensitivity reaction characterized by acute, life-threatening cardiopulmonary dysfunction, including shock, angioedema, and bronchospasm. Although coagulopathy and disseminated intravascular coagulation (DIC) have been reported in association with anaphylaxis, they remain uncommon manifestations. Based on limited case reports, proposed mechanisms of anaphylaxis-associated coagulopathy include the release of mast cell-derived mediators such as tryptase, platelet-activating factor, and heparin, as well as activation of the contact system. DIC may result from

BACKGROUND: Whereas lupus anticoagulant (LA) and anti-factor VIII (anti-VIII) antibody are both acquired autoimmune coagulation inhibitors, they exhibit different pathophysiologic mechanisms and opposite clinical manifestations. Distinguishing between these two inhibitors is therefore essential for optimizing appropriate management. Harboring both antibodies, which is a rare condition, is of a challenging and confounding laboratory work-up.
CASE PRESENTATION: We illustrate a case report of a 39-year-old man admitted for the management of recurrent deep-vein thrombosis. Curiously, the initial
