BACKGROUND: The use of granulocyte-colony-stimulating factor (G-CSF) for mobilization, collection, and transplantation of autologous hematopoietic progenitor cells (HPCs) in patients with hemoglobinopathies can be complicated by severe vasoocclusive crises. Low density lipoprotein (LDL) apheresis is a safe procedure to treat severe hypercholesterolemia in patients with chronic heart disease (CHD). The first long-term experience with lipid apheresis in a hemodialysis patient with refractory combined hyperlipidemia and coronary heart disease is described. Simultaneous treatment was performed once a week using two separate machines connected by parallel flow. Correction of hyperlipidemia was achieved, accompanied by inhibition of progressive coronary heart disease, enabling kidney transplantation.

Reduction of polyglobulia is one of the principal rules in therapeutic management of decompensated cor pulmonale syndrome. Using a hypothetical formula to calculate exchange volumes, erythropheresis was performed on 47 occasions in 20 children with secondary polycythaemia. Following this procedure the mean haematocrit in the patients was reduced from 69.0 to 58.0% (P less than 0.001), and the individual haematocrit to 61% or below. INTRODUCTION: Progressive encephalomyelitis with rigidity and myoclonus (PERM) is characterized by brainstem symptoms, muscle rigidity, and myoclonus. While autoantibodies to inhibitory neurons have been associated with the pathology, about 30% of cases are negative for autoantibodies. There are few reported cases of antibody-negative PERM and its clinical course and prognosis are unknown.

In 1984, we started using therapeutic plasmapheresis (plasma exchange) as a method of extracorporeal lipoprotein elimination for the treatment of hypercholesterolemic patients. We evaluated the results of long-term therapy in 14 patients, 8 men and 6 women.
