[Surgical treatment of the polyostotic form of fibrous dysplasia in children and adolescents].

The work presents an experience with the treatment of 50 patients with polyossal form of fibrous osteodysplasia on whom 99 operations were performed. A method of plasty of bone defects was elaborated and used in order to eliminate severe deformations of lower extremity bones after removal of pathological tissue in combination with epiosseous nailing osteosynthesis. In patients with short bones the compression-distraction osteosynthesis with bone alloplasty was used. Long-term results of treatment of 48 patients make it possible to recommend the methods of bone alloplasty with extraosseous and

Comparison of carbon fibre and titanium intramedullary nails in orthopaedic oncology.

AIMS: Due to their radiolucency and favourable mechanical properties, carbon fibre nails may be a preferable alternative to titanium nails for oncology patients. We aim to compare the surgical characteristics and short-term results of patients who underwent intramedullary fixation with either a titanium or carbon fibre nail for pathological long-bone fracture. METHODS: This single tertiary-institutional, retrospectively matched case-control study included 72 patients who underwent prophylactic or therapeutic fixation for pathological fracture of the humerus, femur, or tibia with either a tita

Evaluation of methods and timing in nail dynamisation for treating delayed healing femoral shaft fractures.

INTRODUCTION: Dynamisation is a simple method for treating femoral shaft fractures (FSFs) in patients with delayed healing after intramedullary nailing. However, only around half of patients with a slow-healing FSF have been successfully treated with this strategy. Although it is thought that dynamisation with a screw preserved in the dynamic locking hole improves treatment outcome in these patients, to date, no studies have demonstrated empirical evidence supporting this technique. In this study, we investigated the effects of dynamisation with and without rotation and length control as well

Commercially available trabecular metal ankle interpositional spacer for tibiotalocalcaneal arthrodesis secondary to severe bone loss of the ankle.

Retrograde tibiotalocalcaneal nailing arthrodesis has proved to be a viable salvage procedure; however, extended bone loss around the ankle has been associated with high rates of nonunion and considerable shortening of the hindfoot. We present the surgical technique and the first 2 cases in which a trabecular metal™ interpositional spacer, specifically designed for tibiotalocalcaneal nailing arthrodesis, was used. The spacer can be implanted using either an anterior or a lateral approach. An integrated hole in the spacer allows a retrograde nail to be inserted, which provides excellent primary

Flexible nailing of fractures in children using stainless steel Kirschner wires.

Elastic titanium nails are widely used for the established procedure of Elastic Stable Intramedullary Nailing (ESIN), but are costly. Thirty-five diaphyseal fractures in various long bones in children were treated by flexible nailing using stainless steel Kirschner (K) wires. The results were comparable with reports by other authors using titanium nails and stainless steel K wires. Cost-effective K wires and instrumentation are easily available. A stainless steel K wire is 92% cheaper than a Nancy nail and 84% cheaper than a Synthes titanium elastic nail. K wires can be used for flexible naili

Experimental stabilization of segmental defects in the human femur. A torsional study.

One of each of thirty-five pairs of fresh intact femora were tested to failure in torsion, recording the dynamic torque, the absorbed energy, and the angle of rotation. These results were compared with the results obtained with the contralateral femur, reconstituted after removal of a segment. Intramedullary nails with polymethylmethacrylate cement, strips of titanium mesh with cement, bone plates with and without cement, and multiple Steinmann pins with cement were the reconstituting configurations. Bone plates were the strongest configuration; the failure torques in all cases were limited by

[Nail-patella syndrome associated with monoclonal gammopathy. Review regarding a case].

A 54 years old female with significant impairment of renal function is presented. On physical examination, the presence of dystrophic nails, elbow dysplasia and prominent iliac horns. Familial study showed similar nail and bone deformities in 3 of six sons. The nail patella syndrome diagnosis was based on these findings. This is a rare autosomal dominant hereditary disease, probably related to congenital alterations in collagen metabolism. Clinical characteristics include bone abnormalities that principally involve knees and elbows, nail alterations and the presence of iliac horns, that are co

Nail unit squamous cell carcinoma: updates on diagnosis, surgical approach, and the use of mohs micrographic surgery.

Nail unit squamous cell carcinoma (NSCC) is a malignant neoplasm that can arise from any part of the nail unit including the nail bed, nail matrix, nail groove, and nail fold. Nail unit SCC is the most common malignant nail neoplasm, and yet its diagnosis often is delayed, partly due to the clinical presentation of NSCC mimicking benign conditions such as onychomycosis, warts, and paronychia. Nail unit SCC has a low rate of metastasis; however, a delayed diagnosis often can result in local destruction and bone invasion. It is imperative for dermatologists who are early in their training to rec

An evolutionary perspective to intramedullary nail fit - comparing present-day data with a historical specimen collection.

Manufacturers aim to design implants fitting for the broadest possible population segment. Due to the scarcity of available morphological data of intact long bones, anatomical collections of historical bone specimens may represent valuable additional sources. Previous work on femoral morphology measurements suggests that historical specimens are widely consistent with data from present-day populations. This study aimed to investigate whether this also applies to the anatomical fitting of a clinically used femoral nail. Nail fit was computer-graphically quantified through virtual implantation i

The efficacy of single-stage open intramedullary nailing of neglected femur fractures.

BACKGROUND: Neglected femur fractures are not rare in the developing world. Treatment options include single-stage open reduction and intramedullary nailing, or open release, skeletal traction, and then second-stage open intramedullary nailing, with bone grafting. Single-stage procedures have the potential advantage of avoiding neurovascular complications secondary to acute lengthening, but they require a second operation, with potentially increased resource use and infection risk. QUESTIONS/PURPOSES: We sought to determine the (1) likelihood of union, (2) complications and reoperations, and

Salvage of Bone Transport with the Induced Membrane Technique in an Open Tibia Fracture with a Segmental Defect: A Case Report.

CASE: A 29-year-old man with an open tibia fracture and a 15-cm bone defect was treated with irrigation, debridement, intramedullary nailing, soft-tissue flap, and bone transport with a frame over the nail. He developed osteomyelitis of his bone transport segment close to docking, which required bone resection and an induced membrane technique to salvage his transport segment and achieve union. CONCLUSIONS: This report illustrates the combined use of bone transport and induced membrane technique to achieve union in a 15-cm tibial defect. It demonstrates the use of the Masquelet technique to s

Atypical use of pediatric flexible nails in the treatment of diaphyseal fractures in adults.

BACKGROUND AND AIM OF THE WORK: Elastic intramedullary nails are commonly used for the treatment of diaphyseal fractures in adolescents and children. The major advantages are the minimally invasive nature of the technique, the short operation time, and the preservation of the growth plate and periosteum thus allowing bone healing within a closed and intact biological environment. Elastic nails are rarely applied to the adult fractures. METHODS: Five selected adult patients affected by diaphyseal fractures were treated using paediatric flexible nails T2 Kids (Stryker®, Mahwah, NJ, USA) as cons

Biomechanical comparison of different fixation methods in metatarsal shaft fractures: A cadaver study.

BACKGROUND: Various fixation methods are available for the operative treatment of metatarsal shaft fractures: Kirschner wire, Titanium elastic nail, plate, or an intramedullary bone stabilization system within a balloon catheter. The aim of this study was to compare the stability of these techniques. METHODS: 72 metatarsals II to V from fresh frozen human cadaver feet were used. A shaft fracture was performed and fixed with a 1.6-mm Kirschner wire, a 1.5-mm Titanium elastic nail, a locking 6-hole-plate, or an intramedullary bone stabilization system. In a cantilever configuration, the head of

Risk factors for re-fracture in children with diaphyseal fracture of the forearm treated with elastic stable intramedullary nailing.

A retrospective study aims to investigate predisposing factors leading to re-fracture in patients with nails still in place and to identify potential correlations between nail size-to-medullary canal diameter ratio and re-fracture, working to the hypothesis that a higher ratio correlates with a higher risk of re-fracture. Rates of re-fracture with nail still in place after forearm elastic stable intramedullary nailing are higher in younger pediatric patients. Re-fractures occurred at a mean age of 6.7 years (range 5-8.7). While the role of body frame and bone medullary diameter remains unclear

The importance of intramedullary hip nail positioning during implantation for stable pertrochanteric fractures: biomechanical analysis.

PURPOSE: Proximal femoral fractures are among the most commonly sustained fractures. The current treatment of stable proximal femoral fractures located in trochanteric region primarily involves the use of two systems: extramedullary dynamic hip screws and intramedullary hip nails. Given that these fractures are mainly found in the elderly population, the necessity of a repeat, due to failure of the first, may jeopardize the patient's life. Decisive factors contributing to the healing of a fracture (or the failure thereof) include fracture pattern, technical implementation of the operation (i.e

Multidisciplinary management of soft tissue sarcomas.

BACKGROUND: Osteoporosis, characterised by decreased bone mass and degradation of bone tissue, poses a major global health concern, particularly for the ageing population. The traditional fixation techniques often fail in osteoporotic bones due to their diminished density and strength. Technological advancements in orthopaedic implants, specifically nails, plates, and external fixators, have emerged to address these challenges. MATERIALS AND METHODS: Improvements in implant design focus on material properties, surface modifications, and geometric advancements. Titanium and its alloys are favo

Prospective study of the clinico-radiological outcome of interlocked nailing in proximal third tibial shaft fractures.

BACKGROUND: Almost 9% of tibial shaft fractures occur in the proximal third of the bone. In order to address the problems of mal-alignment and late loss of fixation, all the specific surgical techniques described in the literature were used in nailing these fractures in our study. METHODS: From December 2001 to December 2003, 45 consecutive patients with fracture of the proximal third of the tibial shaft underwent nailing. The clinico-radiological outcome of these cases and the complications encountered were analysed. RESULTS: The average time to knee mobilisation and partial weight-bearing

Evaluation of the mechanical behaviour of the expandable wedge locked nail fixation in retrograde use: A finite element study.

INTRODUCTION: In literature, there have been many studies conducted to research the alternatives of standard interlocking intramedullary nailing. The expandable wedge locked nail fixation, which is thought as a new alternative to the standard interlocking nailing, has been presented in previous numerical studies. The antegrade usage of the wedge locked nail fixation has provided promising results. From this point, the aim of the study is to evaluate mechanical behavior of its retrograde usage on femur models. Additionally, another aim of the study is to investigate the effect of fracture level
