Incoherence in the Brain Death Guideline Regarding Brain Blood Flow Testing: Lessons from the Much-Publicized Case of Zack Dunlap.

At age 21, following a severe traumatic brain injury, Zack Dunlap was declared brain-dead according to the American Academy of Neurology guideline (Guideline) when he met the clinical criteria of brain death (minus apnea testing because of bradycardia) with technetium-99m diethylene-triamine-pentaacetate scintigraphy reported as showing no intracranial blood flow. His parents agreed to organ donation. During preparations for organ donation, Zack manifested a purposeful movement in response to a noxious stimulus made by his cousin. Following subsequent neurological recovery, he has returned to …

Residual brain viability, evaluated by (99m)Tc-ECD SPECT, in patients with suspected brain death and with confounding clinical factors.

BACKGROUND: In 1968, the Harvard criteria for brain death diagnosis were introduced in clinical practice. These include no movements or breathing, no reflexes, and flat electroencephalogram in the absence of confounding factors, including hypothermia, drugs, electrolyte, and endocrine disturbances. When confounding factors occur, confirmatory tests documenting the absence of cerebral blood flow, such as cerebral angiogram, transcranial Doppler sonography, computed tomography angiography, and nuclear techniques, are required. AIM: The aim of this study was to evaluate the clinical value of sin…

Brain Death and Cold Storage Induce Renal Inflammation and Injury in an Experimental Model.

BACKGROUND: Brain death (BD) and cold storage (CS) are key determinants of donor kidney quality and transplant outcomes. Although both processes are known to induce inflammation, the temporal hierarchy and molecular specificity of inflammatory activation during organ preservation remain poorly defined. This study investigated the inflammatory and histopathological evolution of donor kidneys following BD and during CS using an experimental rat model. METHODS: Rats were assigned to sham, BD, or BD followed by 12 or 24 hours of CS. In BD animals, contralateral kidneys were stored for different d…

Sudden Prehospital Deaths From Brain Arteriovenous Malformations: A Population-Based Study.

BACKGROUND AND OBJECTIVES: The number of unexpected prehospital deaths from brain arteriovenous malformations (AVMs) is unknown because existing mortality data are derived from hospital-based cohorts. In this study, we aimed to determine the rate and causes of AVM-related deaths in a large population-based cohort, both in and outside hospitals. We hypothesized that AVMs infrequently cause unexpected prehospital deaths, and when they do, it is typically due to hemorrhages rather than epileptic seizures. METHODS: In this population-based cross-sectional study, we examined the AVM-related deaths

Demography of brain stem death and factors leading to successful consent for organ donation.

BACKGROUND: There is a widening gap between the patients requiring transplants in proportion to the availability of organs donated. After brain stem death (BSD), counselling for consent is mandatory and if successful, and viable organs are recovered. A low consent rate for donation results in failed donation in a number of potential donors. Many potential donors fail to make a donation due to various social and familial reasons resulting in a poor consent rate. The aim of this study was to identify the variables that lead to donation denials. METHODS: This retrospective study, carried out fro

Distinct Patterns of Brain Metabolism in Patients at Risk of Sudden Unexpected Death in Epilepsy.

Objective: To characterize regional brain metabolic differences in patients at high risk of sudden unexpected death in epilepsy (SUDEP), using fluorine-18-fluorodeoxyglucose positron emission tomography (18FDG-PET). Methods: We studied patients with refractory focal epilepsy at high (n = 56) and low (n = 69) risk of SUDEP who underwent interictal 18FDG-PET as part of their pre-surgical evaluation. Binary SUDEP risk was ascertained by thresholding frequency of focal to bilateral tonic-clonic seizures (FBTCS). A whole brain analysis was employed to explore regional differences in interictal meta

Apnea test for brain death diagnosis in adults on extracorporeal membrane oxygenation: a review.

Among the potential complications of extracorporeal membrane oxygenation, neurological dysfunctions, including brain death, are not negligible. In Brazil, the diagnostic process of brain death is regulated by Federal Council of Medicine resolution 2,173 of 2017. Diagnostic tests for brain death include the apnea test, which assesses the presence of a ventilatory response to hypercapnic stimulus. However, gas exchange, including carbon dioxide removal, is maintained under extracorporeal membrane oxygenation, making the test challenging. In addition to the fact that the aforementioned resolution…

Global Scientific Outputs of Brain Death Publications and Evaluation According to the Religions of Countries.

In 1950s, the concept of brain death, which began to be discussed primarily in terms of medicine and then in terms of religion, law, and ethics, became a central topic in all world countries as it was an early diagnosis of death. Despite the fact that brain death (BD) diagnosis is of importance for benefitting from organ and tissue transplantation of patients in the world, the literature still involves no bibliometric studies that made a holistic evaluation of the publications about this issue. The present study aims to investigate the top-cited articles about BD published between 1980 and 201…

Current Evidence on Cell Death in Preterm Brain Injury in Human and Preclinical Models.

Despite tremendous advances in neonatal intensive care over the past 20 years, prematurity carries a high burden of neurological morbidity lasting lifelong. The term encephalopathy of prematurity (EoP) coined by Volpe in 2009 encompasses all aspects of the now known effects of prematurity on the immature brain, including altered and disturbed development as well as specific lesional hallmarks. Understanding the way cells are damaged is crucial to design brain protective strategies, and in this purpose, preclinical models largely contribute to improve the comprehension of the cell death mechani

Guidelines for the management of a brain death donor in the rhesus macaque: A translational transplant model.

INTRODUCTION: The development of a translatable brain death animal model has significant potential to advance not only transplant research, but also the understanding of the pathophysiologic changes that occur in brain death and severe traumatic brain injury. The aim of this paper is to describe a rhesus macaque model of brain death designed to simulate the average time and medical management described in the human literature. METHODS: Following approval by the Institutional Animal Care and Use Committee, a brain death model was developed. Non-human primates were monitored and maintained for …

Anticonvulsant drug-induced cell death in the developing white matter of the rodent brain.

OBJECTIVE: During critical periods of brain development, both seizures and anticonvulsant medications can affect neurodevelopmental outcomes. In rodent models, many anticonvulsants trigger neuronal apoptosis. However, white matter apoptosis (WMA) has not been examined after anticonvulsant drug treatment. Herein, we sought to determine if anticonvulsant drugs induced apoptosis in the developing white matter (WM) in a rodent model. METHODS: Postnatal day (P)7 rats were treated with phenobarbital (PB-75), MK-801 (dizocilpine, 0.5), lamotrigine (LTG-20), carbamazepine (CBZ-100), phenytoin (PHT-50

Contribution of brain inflammation to neuronal cell death in neuronopathic forms of Gaucher's disease.

Gaucher's disease, the most common lysosomal storage disorder, is caused by the defective activity of glucocerebrosidase, the lysosomal hydrolase that degrades glucosylceramide. The neuronopathic forms of Gaucher's disease are characterized by severe neuronal loss, astrocytosis and microglial proliferation, but the cellular and molecular pathways causing these changes are not known. In the current study, we delineate the role of neuroinflammation in the pathogenesis of neuronopathic Gaucher's disease and show significant changes in levels of inflammatory mediators in the brain of a neuronopath

Mitochondrial mechanisms of cell death and neuroprotection in pediatric ischemic and traumatic brain injury.

There are several forms of acute pediatric brain injury, including neonatal asphyxia, pediatric cardiac arrest with global ischemia, and head trauma, that result in devastating, lifelong neurologic impairment. The only clinical intervention that appears neuroprotective is hypothermia initiated soon after the initial injury. Evidence indicates that oxidative stress, mitochondrial dysfunction, and impaired cerebral energy metabolism contribute to the brain cell death that is responsible for much of the poor neurologic outcome from these events. Recent results obtained from both in vitro and anim

Embolism of brain tissue in intrapartum and early neonatal deaths: report of 9 cases.

We report the clinical features and pathological findings in 9 cases of intrapartum and early neonatal death associated with embolism of brain tissue to the pulmonary and systemic circulation following difficult delivery. All except 1 of the cases in this series were born at, or near to, term, were vertex presentations, and were normally grown. All followed instrumental delivery and in 5, at least one mode of assisted delivery had failed. Whilst some evidence of cranial trauma was present in 7 cases, only 3 had skull fractures and 7 showed intracranial hemorrhage, although this was described a

[Nutritional support in a pregnant woman with brain death. Case report and literature review].

The occurence of brain death in pregnancy represents a catastrophic entity although infrequent. The aims of continue medical management are focused in a double purpose: to preserve intrauterine product's life and fetal maturation until delivery and to consider the mother as a potential organ donor. Ethical considerations together with gestational age, fetal well being and relatives' wishes are cardinal for continuing medical support. Modern critical care units allow us to obtain favourable results, supported in scientifical reports that describe successful outcomes. Nutritional aspects plays a…
