To evaluate a possible brainstem role in pathogenesis of obstructive sleep apnea syndrome, a study on brainstem auditory evoked potentials (BAEP), has been conducted. 15 OSAS patients, medium and severe form, with organic pathologies of the upper respiratory tract have been evaluated. 15 normal subjects were compared as control. All the patients were subjected to audiometry, including total liminal audiometry, timpanometry, acustic reflex, and BAEP study. A case of brain stem cavernous angioma associated with peduncular hallucination is reported. A 68-year-old man was admitted with complaints of left sensory disturbance. On the evening of the first hospital day, he developed vividly formed hallucinations. The hallucinations were accompanied by sleep disturbance and agitation. Brainstem acoustic evoked responses were recorded from 37 infants with cleft palate. Only 35% presented normal auditory thresholds. 27% had a bilateral 22% a unilateral hearing deficit of more than 30 dB. Controls after operative closure of the cleft showed no improvement of the conductive hearing loss.

A circumscribed vascular plexus is located between the conjoint proximal ganglion of cranial nerves IX-X and the lateral surface of the avian brain stem. Gross, light, and electron microscopic analyses reveal that this structure is a rete mirabile. In previous reports we have suggested that the auditory brain-stem responses (ABRs) of adults with Down's syndrome exhibit two abnormalities unique to that population: a shortened central conduction time and an insensitivity of peak latencies to increases in stimulation rate. Bilateral vocal cord immobility in adduction is an uncommon, yet potentially life threatening, disorder. Endolaryngeal and neuromuscular causes are well known and for the most part readily diagnosed by the otolaryngologist. Central neurologic etiologies are more unusual. A case of transient spastic laryngeal paralysis secondary to cardiac emboli and brain stem ischemia is presented. A mechanistic analysis suggests that this entity occurs in the setting of pseudobulbar paralysis, and that the ultimate ischemic event may be loss of glycine mediated inhibition of laryngeal motoneurons.

A patient with a rapidly progressive brain stem syndrome is reported. CT scan showed a ring enhancing lesion in the pons and CSF examination showed elevated protein and pleocytosis. A pulmonary abscess containing Nocardia was found and antimicrobial therapy produced progressive reduction in size of the pontine lesion and clinical improvement without surgical drainage. This is the first documented diagnosis and survival of a medically treated patient with a brain stem abscess of presumed Nocardia etiology.

We report 36 cases of post-traumatic "primary brain stem haemorrhage" visualized by the CT scan and confirmed at autopsy. Clinical experience shows that many technical factors influence the inability to visualize brain stem haemorrhages. Sixty-three children from 2 to 12 years of age were studied by means of brain stem evoked potentials (BEP) and otoscopic evaluation in a preliminary assessment of the sensitivity of BEP latency measures to middle ear abnormality. Central pontine myelinolysis was suspected in two chronic alcoholics who developed and recovered from a progressive spastic paresis of all muscles, that derived innervation at and below the level of the pons. In both cases, short-latency auditory-evoked responses aided in the diagnosis by indicating a slowing of conduction in the pontine auditory pathway, which varied in degree with the severity of the clinical manifestations of pontine demyelination.
