OBJECTIVE: Safe vaginal breech birth requires timely recognition of stagnation and skilled execution of maneuvers, yet detailed descriptions of upright (all-fours) breech techniques remain scarce. Video-based approaches provide an opportunity to capture real-life practice and inform refinement of existing algorithms. The aim was to describe the management of vaginal breech birth on all fours using systematic video analysis, document maneuvers applied, and generate exploratory insights to refine the physiological breech birth algorithm.

Uterus didelphus is a congenital abnormality arising from failure of fusion of Mullerian ducts, creating two separate uterine horns, two cervices and, in some cases, a vagina divided by a longitudinal septum. The effectiveness of external cephalic version with tocolysis when routinely used in the 37th week of gestation is reported. The procedure had earlier been ruled out in our department, was found effective in a prospective study, and afterwards settled as a routine. Among 1038 women with single breech presentation, 882 could be offered an attempt of external cephalic version during the period 1982-1988. We analyzed the records of 454,668 births in Ontario in the years 1979 to 1982 and found that the cesarean birth rate increased from 16.5 per hundred deliveries in 1979 to 18.7 in 1982. Cesarean births were classified according to four indications: previous cesarean birth, breech presentation, dystocia, and fetal distress. Spinal cord injury may occur as a severe complication to delivery. In the vast majority of such cases the injury results from a traumatic breech delivery, but cases of injuries after cephalic presentation and fetal malposition have also been described. Two cases were reported. One of the infants died at the age of 8 months and neuropathological examination of the brain and spinal cord was performed. The other child, now 6 years old, is still alive. Incidence, mechanism of injury, clinical and morphological features, and treatment are briefly discussed.

We report a novel application of intrapartum sonography, herein used to assist the internal podalic version and the vaginal delivery of a transverse-lying second twin. Following the vaginal delivery of the first cephalic twin, the internal podalic version was performed under continuous ultrasound vision, leading to the uncomplicated breech delivery of a healthy neonate.

A 36-year-old multiparous woman with fetus in the breech position applied ice to the fundus of the uterus and achieved successful cephalic version. No other reports of using ice to induce cephalic version are found with MEDLINE search; however, it has been used as a folk remedy. Further research to evaluate the efficacy and safety of ice is needed to determine whether it increases cephalic vaginal birth.
