Bridging therapy in pediatric and AYA patients under 25 years with relapsed/refractory B-ALL undergoing anti-CD19 CAR-T therapy: a multicenter retrospective study by the Spanish group GETH-TC.

CAR-T therapy is a powerful treatment for pediatric B-cell acute lymphoblastic leukemia (B-ALL). However, the impact of bridging therapy prior to CAR-T infusion on patient outcomes remains insufficiently explored. In this retrospective study of patients with relapse/refractory B-ALL (N = 103), we compared outcomes across different bridging therapy groups: Low-Intensity Chemotherapy (LIC, N = 50), Immunotherapy (N = 20), or High-Intensity Chemotherapy (HIC, N = 33). At day +28 post-infusion, LIC and Immunotherapy showed significantly higher rates of Complete Remission with negative Minimal Resi

Ambulatory femoral versus axillary intra-aortic balloon pump support as a bridge to advanced heart failure therapies: Insights from a single-center experience.

BACKGROUND: Ambulatory intra-aortic balloon pump (IABP) support is increasingly used to preserve mobility in patients with advanced heart failure awaiting heart transplantation or left ventricular assist device therapy. Axillary IABP facilitates ambulation but is associated with device-related complications. Structured ambulatory femoral IABP protocols have emerged as an alternative; however, direct comparisons of clinical and functional outcomes are lacking. OBJECTIVES: To compare safety, device-related complications, and functional mobility between ambulatory axillary and ambulatory femoral

Impact of low-dose alteplase bridging therapy on outcomes after mechanical thrombectomy in patients with atrial fibrillation-associated acute stroke: A retrospective cohort study.

The optimal alteplase dose for bridging therapy in atrial fibrillation related large vessel occlusion stroke is uncertain. We compared low-dose alteplase plus mechanical thrombectomy with standard-dose bridging and direct thrombectomy. This single-center retrospective cohort included all consecutive eligible patients (n = 375) treated between January 2021 and December 2024; no a priori sample size calculation was performed. Patients received low-dose alteplase 0.6 mg/kg plus thrombectomy (group A, n = 120), standard-dose 0.9 mg/kg plus thrombectomy (group B, n = 125), or direct thrombectomy wi

[Choice of bridging therapy prior to reinjection of autologous CAR-T cells in patients aged 0-25 years treated for B-ALL (SFGM-TC)].

CAR-T-cells represent a major change in the field of relapsed/refractory B-cell acute lymphoblastic leukemia treatment in children, adolescents and young adults. However, despite a high early response rate of more than 90%, almost half of patients relapse after CAR-T-cells treatment. Several clinical and biological factors predicting success and failure of CAR-T-cells have been identified thanks to real life studies. Bridging period between leukapheresis and CAR-T-cells reinfusion is considered as a key stone to allow CAR-T-cells procedure in optimal condition. No prospective study and few ret

Cutaneous Laser Therapy for Residual Limb Skin Pathology: A Narrative Review Bridging Dermatologic Laser Research With Clinical Perspectives and Considerations for Multidisciplinary Care Teams.

Cutaneous laser therapies are widely employed in dermatology for hair removal, scar remodeling, and skin rejuvenation, yet their use in managing skin conditions specific to residual limbs after amputation remains underexplored. Individuals with limb loss frequently experience skin pathology such as folliculitis, hyperkeratosis, scar contracture, chronic wounds, and volume instability, which can significantly impair prosthetic fit, function, and quality of life. This narrative review synthesizes existing dermatologic laser applications and clinical experience to explore their potential adaptati

Impact of cladribine, cytarabine, and G-CSF (CLAG) as a bridging therapy prior to allogeneic hematopoietic stem cell transplantation in relapsed or refractory acute myeloid leukemia.

The combination of cladribine, cytarabine, and G-CSF (CLAG) has exhibited robust synergistic anti-leukemia activity as an induction therapy (IT) in acute myeloid leukemia (AML). However, the impact of CLAG as a bridging therapy (BT) administered between IT and allogeneic hematopoietic stem cell transplantation (allo-HSCT) for patients with relapsed or refractory (R/R) AML remains uncertain. In this retrospective study, we examined the efficacy of CLAG as a transitional strategy prior to allo-HSCT in R/R AML. We included 234 patients with R/R AML who received the modified busulfan plus cyclopho

Approaches for bridging therapy prior to chimeric antigen receptor T cells for relapsed/refractory acute lymphoblastic B-lineage leukemia in children and young adults.

The ongoing development of immunotherapies, including chimeric antigen receptor (CAR) T cells, has revolutionized cancer treatment. In pediatric relapsed/refractory B-lineage acute leukemia antiCD19-CAR induce impressive initial response rates, with event-free survival plateauing at 30-50% according to long-term follow-up data. During the interval between diagnosis of relapse or refractoriness and CAR T-cell infusion, patients require a bridging therapy. To date, this therapy has consisted of highly variable approaches based on local experience. Here, in an European collaborative effort of ped

Activating adaptive immunity by bispecific, T-cell engager antibodies bridging infected and immune-effector cells is a promising novel therapy for chronic hepatitis B.

Bispecific antibodies (bsAbs) are engineered immunoglobulins that combine two different antigen-binding sites in one molecule. BsAbs can be divided into two molecular formats: IgG-like and non-IgG-like antibodies. Structural elements of each format have implications for engaging the immune system. T cell engager antibodies (TCEs) are bsAbs designed to engage T cells with target cells. TCEs can be applied not only in cancer but also in infectious disease therapy to activate T-cell responses. In this review, we focus on current literature on the design and use of bsAbs as an innovative strategy

Bridging Programmatic Gap in Post-TB Treatment Prevention: Applying the COM-B Model and Behavior Change Wheel to Improve the Uptake of TB Preventive Therapy.

Uganda's National Tuberculosis and Leprosy Program (NTP) recommends tuberculosis preventive therapy (TPT) immediately after TB treatment completion (hereafter post-TB treatment TPT or PTT-TPT) for people with retreatment TB, as they are at high risk for recurrent/drug-resistant TB. However, uptake of PTT-TPT is limited. To examine determinants of uptake, identify program-relevant strategies, and improve implementation, we applied a behavioral framework. We used the COM-B model (Capability-Opportunity-Motivation Behavior) to conceptualize the behavioral and health system determinants and the Be

Bridging evidence and practice in heart failure care: barriers and pragmatic solutions to guideline-directed medical therapy implementation in India and other low-resource settings.

Heart failure with reduced ejection fraction (HFrEF) remains a leading cause of morbidity, hospitalization, and mortality in low- and middle-income countries (LMICs). Guideline-directed medical therapy (GDMT) including angiotensin receptor-neprilysin inhibitors (ARNIs), evidence-based beta-blockers, mineralocorticoid receptor antagonists (MRAs), and sodium-glucose co-transporter 2 (SGLT2) inhibitors has consistently demonstrated reductions in mortality and heart failure (HF) hospitalizations. Despite strong evidence, real-world implementation of GDMT in LMICs remains suboptimal. This article s

Single antiplatelet therapy and tirofiban bridged with surface modified flow diverters for ruptured blood blister-like aneurysms: single center experience and systematic review.

BACKGROUND: Blood blister-like aneurysms (BBAs) of the internal carotid artery are rare but high risk lesions that frequently re-rupture due to their fragile structure and dissecting pathology. Treatment is particularly challenging in ruptured cases, given the risks associated with dual antiplatelet therapy. Recent advancements in flow diverter stents (FDSs) with surface modifications, and the use of single antiplatelet therapy (SAPT), offer a potential alternative strategy. METHODS: We conducted a retrospective review of 17 patients with ruptured internal carotid artery BBAs treated with sur

The Dilemma of Edoxaban Interruption and Heparin Bridging Before Upgrading to Cardiac Resynchronization Therapy in an Older Patient with Atrial Fibrillation, Chronic Kidney Disease, and Mitral Bioprosthesis.

The peri-procedural management of novel oral anticoagulants (NOAC) should be individualized based on the patient (age, body weight, renal function, medications, previous thromboembolic/bleeding event, presence of prosthetic valve) and procedural (bleeding risk) characteristics. Less invasive procedures carry a relatively low bleeding risk and may be performed under minimally- or uninterrupted NOAC therapy. However, upgrading from implantable cardioverter defibrillator (ICD) to cardiac resynchronization therapy (CRT) is more complex than the initial implantation procedure. Thus, the timing of t

Low-Dose vs. Standard-Dose Intravenous Alteplase in Bridging Therapy Among Patients With Acute Ischemic Stroke: Experience From a Stroke Center in Vietnam.

Background: To date, the role of bridging intravenous thrombolysis before mechanical thrombectomy (MTE) is controversial but still recommended in eligible patients. Different doses of intravenous alteplase have been used for treating patients with acute ischemic stroke from large-vessel occlusion (LVO-AIS) in Asia, largely due to variations in the risks for intracerebral hemorrhage (ICH) and treatment affordability. Uncertainty exists over the potential benefits of treating low-dose alteplase, as opposed to standard-dose alteplase, prior to MTE among patients with LVO-AIS. Aim: The aim of the
