Characteristics and outcomes of multiple organ dysfunction syndrome among severe-burn patients.

AIM: Determining the characteristics and outcomes of multiple organ dysfunction syndrome (MODS) in severe-burn patients. METHOD: A prospective study was conducted in 117 severe-burn, adult patients admitted to the National Institute of Burns, Hanoi, Vietnam with burn area > or =40% of the total body surface area. The diagnosis of MODS was based on Sequential Organ Failure Assessment (SOFA) score. RESULTS: MODS was recorded in 45.3% of the patients. A higher rate of MODS was recorded in patients over 40 years of age (51.61%), those presenting with inhalation injury (60.37%) and having a large

Staged Biodegradable Temporizing Matrix and Meek Micrografting as a Donor-Sparing Strategy for Extensive Burn Injuries.

Definitive closure of extensive burns remains constrained by donor-site availability, particularly in patients with burns of 50% total body surface area (TBSA) or greater. Biodegradable Temporizing Matrix (BTM) can stabilize excised wounds while donor sites recover, whereas Meek micrografting permits controlled expansion of limited autologous skin. We retrospectively reviewed adult patients admitted between May 2022 and May 2025 with burns of 50% TBSA or greater who underwent BTM application followed by Meek micrografting as part of definitive wound-closure management. Demographic, injury, ope

Characteristics of bloodstream infections in burn patients: An 11-year retrospective study.

AIMS: The principal aim of this study was to describe infection related characteristics of blood stream infections (BSI) in patients with burns. We sought to determine the organisms that caused BSI and factors that could predict the outcome of BSI. METHODS: Data was collected on admitted patients with burns from January 1998 to December 2008. Selected information from databases was analysed using SPSS version 17 (SPSS Inc., Chicago). Descriptive, univariate and multivariate analysis was undertaken to determine factors predictive of clinical outcome. The factors analysed by univariate analysis

"Scorched air": A narrative review of inhalational injuries in fire onboard ships.

Fires aboard ships remain a serious hazard, posing significant risks to crew members due to the confined and often poorly ventilated environments. Modern ships, especially military vessels with airtight compartments, are particularly vulnerable to rapid accumulation of smoke, even from minor fires. In such settings, smoke inhalation-not burns-is the leading cause of fire-related mortality. This article explores the pathophysiology, clinical manifestations, and management of inhalational injuries resulting from onboard ship fires. It aims to highlight the unique challenges faced in maritime set

Upper airway sequelae in burn patients requiring endotracheal intubation or tracheostomy.

During a period of 11 1/2 months, 41 of 217 adult burn patients admitted to the U.S. Army Institute of Surgical Research Burn Center required endotracheal intubation or tracheostomy for management of the airway and/or ventilatory assistance. Permanent upper airway sequelae were recorded and related to presence of inhalation injury, duration of tube placement, cuff pressure, and pulmonary compliance. An "inhalation injury scoring system" based upon history, physical examination, bronchoscopic findings, and abnormalities at 133xenon lung scan correlated well with postinjury alteration in complia

Haemophilus Influenzae contributes to morbidity but not mortality in severely burned patients.

Due to a successful Haemophilus Influenzae (HIF) vaccination program, this pathogen is no longer a major contributor to childhood morbidity and mortality. The effect on adult acquisition of HIF is unknown. The purpose of this study was to determine HIF infection frequency in severely burned patients. In a 62-month period, there were 1486 admissions. Twenty-four of 1,486 (1.6%) burn patients had HIF pneumonia: 16 (1%) within 72 h after injury; 8 (0.5%) patients developed it after the seventh day of hospitalization; 74 of 1,486 (5.0%) burn patients had pneumonia due to other organisms. There was

Clinical analysis of multiple organ failure in burned patients.

Severely burned patients often show various degrees of organ failures, and when several vital organs are involved mortality becomes extremely high. A study was undertaken to elucidate the aetiology and clinical significance of multiple organ failure (MOF) in burned patients. One hundred and fifty-eight burned patients were analysed for organ failures in five vital organs or systems, the heart, lung, liver, kidney and blood clotting system. There were 91 organ failures observed in 34 patients, of which 26 had MOF. The most frequently affected organ was the lung, followed by the heart, kidney, l

The burned trachea.

Tracheal burns caused by thermal or chemical injuries are uncommon, but they can be difficult to manage. A high index of suspicion is required for early detection of these injuries. Acutely, they are often life threatening unless they are diagnosed quickly and managed properly. Tracheal burn injuries can lead to long-term stenoses, which require specialized surgical techniques and chronic therapy--often with Montgomery T-tubes--for their proper management.

Sensing Inhalation Injury-Associated Changes in Airway Wall Compliance by Anatomic Optical Coherence Elastography.

Quantitative methods for assessing the severity of inhalation (burn) injury are needed to aid in treatment decisions. We hypothesize that it is possible to assess the severity of injuries on the basis of differences in the compliance of the airway wall. Here, we demonstrate the use of a custom-built, endoscopic, anatomic optical coherence elastography (aOCE) system to measure airway wall compliance. The method was first validated using airway phantoms, then performed on ex vivo porcine tracheas under varying degrees of inhalation (steam) injury. A negative correlation between aOCE-derived comp

Effect of early grafting on improvement of lethal area index (la50) in burn patients: a 7-year investigation in a burn referral centre in the North of Iran.

Burning is a major health challenge in all societies. In this descriptive cross-sectional study, health information about hospitalized burn patients was extracted from the hospital information system from April 2009 to February 2015. Logistic regression method was used to identify risk factors and mortality predictors. Lethal area index (LA50) was investigated to evaluate the quality of annual hospital medical care. A total of 7725 patient medical files were examined. Mean age of the patients was 32.3±22.5 years old. The most common cause of burning accidents was hot liquids. Mean percentage o

A study of burn injuries in the elderly in a regional burn centre.

We evaluated the epidemiology and outcome of 94 elderly burn patients of age 60 years or older treated in a local burn centre over a period of 6 years. There were 44 males and 50 females with a male to female ratio of 0.88 and an average age of 73.8+/-9 years. The mean extent of burn was 13.3+/-18.4% total body surface area (TBSA) with 14 patients (15%) suffering from a burn size >20% TBSA. The vast majority of injuries (90%) occurred at home. Scalds resulted in 62 admissions (66%) and flame burns accounted for another 29 admissions (31%). The burns predominantly involved the extremities and t

Ventilator associated pneumonia in major paediatric burns.

More than three-quarters of deaths related to major burns are a consequence of infection, which is frequently ventilator associated pneumonia (VAP). A retrospective study was performed, over a five-year period, of ventilated children with major burns. 92 patients were included in the study; their mean age was 3.5 years and their mean total body surface area burn was 30%. 62% of the patients sustained flame burns, and 31% scalds. The mean ICU stay was 10.6 days (range 2-61 days) and the mean ventilation time was 8.4 days (range 2-45 days). There were 59 documented episodes of pneumonia in 52 pa

A review of the complications of burns, their origin and importance for illness and death.

Complications are the major causes of illness and death after burning and most of them stem from the burn wound. Their origin and importance are reviewed with emphasis on problems and growing points in knowledge. Fluid leakage from the circulation into the burn is the cause of hypovolemic shock, but the underlying permeability changes in the burn are only partly understood. Other nonbacterial complications include acute cardiac failure, acute anemia, hemolytic jaundice, renal failure, encephalopathy, complex hypermetabolic effects including pseudodiabetes, gastric and duodenal ulceration, deep
