Paradoxical hypomagnesemia caused by excessive ingestion of magnesium hydroxide.

Excessive ingestion of magnesium may lead to hypermagnesemia even without kidney dysfunction. Several cases of development of hypermagnesemia after overdose of magnesium hydroxide have been reported. Although magnesium hydroxide is widely used as laxative, its overdose may induce diarrhea, which is followed by excessive magnesium loss. I report a case of paradoxical hypomagnesemia developed after excessive ingestion of magnesium hydroxide. A 39-year-old woman was presented to the emergency department complaining of severe watery diarrhea and carpopedal spasm after ingesting a handful of magnes

The black widow: is she deadly to children?

Pediatric cases of black widow spider envenomation (BWSE) were reviewed in order to assess the morbidity and mortality in children with BWSE, and to make recommendations for optimal therapy. The methodology includes a retrospective chart review in an urban pediatric tertiary care hospital. Included were all pediatric patients admitted with the BWSE diagnosis in the last 10 years (1984-1994). The results are based on 12 children with ages ranging from 15 months to 18 years. The BWSE syndrome is characterized by several common systemic effects. Our cases revealed abdominal pain (100%), hypertens

Spiders and spider bites.

The only spiders of real medical importance to humans in the United States are the Loxosceles and Latrodectus species, primarily the brown recluse and the black window, respectively. These spiders bite humans only when trapped or crushed against the skin. Severe brown recluse spider bites produce dermonecrosis within 72-96 hours, which should be treated with rest, ice compresses, antibiotics, dapsone, and surgery delayed for several weeks. Using systemic corticosteroids may be helpful in treating the much rarer systemic loxoscelism, which has minimal skin changes but produces massive hemolysis

The pivotal role of intracellular calcium in oxaliplatin-induced inhibition of neurite outgrowth but not cell death in differentiated PC12 cells.

The antineoplastic efficacy of oxaliplatin, a widely used anticancer drug, is restricted by its adverse effects such as peripheral neuropathy. Infusing a combination of calcium gluconate and magnesium sulfate (Ca/Mg) suppresses the acute neurotoxic side effects of oxaliplatin, although the mechanism is unclear. To elucidate the molecular mechanisms of oxaliplatin-induced neurotoxicity and the effects of Ca/Mg against this toxicity, we examined the effect of Ca/Mg on oxaliplatin-induced inhibition of neurite outgrowth in PC12 cells, a commonly used neuronal cell model. Oxaliplatin and oxalate s

Acute renal failure in African children with Burkitt's lymphoma: a comparison of two treatment regimens.

BACKGROUND: The outcome for patients presenting with acute renal failure and Burkitt lymphoma (BLARF) without dialysis is poor. This was a retrospective non-randomized comparative study designed to determine the outcome of two different treatment protocols. METHODS: One group of patients (TPA) received oral allopurinol, intravenous (IV) cyclophosphamide, vincristine, methotrexate, furosemide, 8.4% sodium bicarbonate, and intrathecal (IT) methotrexate; the other (TPB) alternate day IV infusion of low dose cyclosphosphamide (125 mg/m(2) x 4 doses), IT methotrexate (Days 1 and 5) and aggressive

Effects of treatments by calcium and sex hormones on vertebral fracturing in osteoporosis.

Lateral radiographs of the thoracic and lumbar spine were taken periodically in 49 patients with osteoporosis. Thirty patients were postmenopausal, and 19 nonmenopausal with osteoporosis due to steroids, male hypogonadism, alcoholism, thyrotoxicosis or unknown cause. Patients were studied before, during and after treatment with high calcium alone, or with combined calcium and sex steroids. Calcium was given as effervescent calcium lactate gluconate, and sex hormones as oestradiol valerate, testosterone oenanthate, or methenolone oenanthate. A total of 964 films covering 409 patient-years were

Identification of a SCN4A mutation in a large Chinese family with atypical normokalemic periodic paralysis using whole-exome sequencing.

OBJECTIVES: Normokalemic periodic paralysis (NormoKPP) of skeletal muscle is an autosomal dominant disorder caused by mutations in the gene encoding voltage-gated sodium channel protein type 4 subunit alpha (SCN4A), which leads to ion channel dysfunction. Little is known about the relationship between genotype and the clinical symptoms of NormoKPP. The present study aimed to evaluate the genetic variation in a large Chinese family with NormoKPP. The patients in this pedigree did not respond to saline treatment, but calcium gluconate treatment was effective. METHODS: We performed a series of c

[The heart and hyperkalemia].

Hyperkalaemia is a common and serious clinical condition in medical and surgical intensive care units. It is mainly encountered in patients with overt or latent cardiac and/or renal failure, and often aggravated by dietetic and therapeutic mismanagement. Hyperkalaemia depresses cardiac contractility, automaticity, conductivity and excitability, leading to ECG changes which must be recognized before clinical cardiovascular deterioration occurs if treatment is to be given to reverse an irremediably fatal evolution: ST-T changes and widening of the QRS with axial deviation shortly precede sinoatr

Refractory hypocalcaemia-induced laryngeal spasm requiring emergency intubation in a malnourished young woman: overcoming diagnostic and therapeutic barriers in resource-limited settings.

BACKGROUND: Hypocalcaemia-induced laryngospasm is a rare but life-threatening cause of upper airway obstruction. Management is particularly challenging in resource-limited settings due to delayed laboratory results, limited monitoring, and unavailability of preferred medications. CASE PRESENTATION: A 20-year-old previously healthy woman presented with severe stridor, respiratory distress, and carpopedal spasm. Laboratory findings (obtained after initial treatment) revealed ionized calcium 0.9 mmol/L (reference range: 1.12–1.32 mmol/L), magnesium 1.6 mg/dL (reference range: 1.7–2.3 mg/dL), and

Zn2+ regulates human oxalate metabolism by manipulating oxalate decarboxylase to treat calcium oxalate stones.

A high concentration of oxalate is associated with an increased risk of kidney calcium oxalate (CaOx) stones, and the degradation of exogenous oxalate mostly depends on oxalate-degrading enzymes from the intestinal microbiome. We found that zinc gluconate supplement to patients with CaOx kidney stones could significantly improve the abundance of oxalate metabolizing bacteria in humans through clinical experiments on patients also subjected to antibiotic treatment. The analysis of clinical samples revealed that an imbalance of Lactobacillus and oxalate decarboxylase (OxDC) was involved in the f

Medical and social issues of cardiovascular diseases and their solution based on the experimental study of myocardial fibrosis.

OBJECTIVE: Introduction: The prevalence and incidence of cardiovascular diseases have been attracting considerable attention in recent decades. This is partly due to the fact that myocardial fibrosis is the major consequence of the most nosological units of cardiovascular diseases. We believe that early pathogenic therapy of myocardial fibrosis should be taken into consideration as a solution to this issue. The change of the connective tissue metabolism in myocardium is the central chain in pathogenesis of diffuse ischemic necrotic cardiosclerosis (DINC) occurs after repeated epinephrine injur

Two-site assay of intact parathyroid hormone in primary hyperparathyroidism: studies in basal conditions, following adenoma removal and during calcium and EDTA infusion.

This study has been carried out in order to investigate parathyroid hormone secretion in patients with primary hyperparathyroidism in basal conditions, during stimulation and suppression tests and following successful surgery. Parathyroid gland secretory activity has been evaluated by a highly sensitive immunoradiometric assay (IRMA) which detects only the biologically intact active hormone and with a well established midmolecule (MM) PTH RIA. There was a good correlation between the two assays in basal state (r = 0.779); however the correlation found between serum PTH levels and total calcium

Are we undertreating calcium deficiency in metabolic bone disease of prematurity? A case report and review.

BACKGROUND: Both calcium (Ca) and phosphorus (P) are needed to prevent and treat metabolic bone disease (MBDP). However, the predominant focus of many treating neonatologists lies in supplementing P and vitamin D. In this report, we describe a VLBW infant with severe MBDP due to inadequately treated calcium deficiency and discuss the need to recognize this entity. CASE DETAILS AND MANAGEMENT: A 25-week, 700 gm baby boy had chronic lung disease and necrotizing enterocolitis. He received total parenteral nutrition, budesonide, furosemide, and caffeine. With high serum alkaline phosphatase (ALP:

Regional citrate anticoagulation in predilution continuous venovenous hemofiltration using prismocitrate 10/2 solution.

Regional citrate anticoagulation (RCA) for continuous renal replacement therapy (CRRT) is associated with a longer filter life and fewer bleeding events. Complexity of the regimen is the major hurdle preventing widespread application. This study describes a simple predilution continuous venovenous hemofiltration (CVVH) protocol utilizing a commercially prepared replacement solution containing citrate (Prismocitrate 10/2). Ten patients with acute renal failure were evaluated. The Prismaflex system was used for predilution CVVH, with Prismocitrate 10/2 running at 2500 mL/h as the main predilutio
