Cardiovascular actions of insulin.

Insulin has important vascular actions to stimulate production of nitric oxide from endothelium. This leads to capillary recruitment, vasodilation, increased blood flow, and subsequent augmentation of glucose disposal in classical insulin target tissues (e.g., skeletal muscle). Phosphatidylinositol 3-kinase-dependent insulin-signaling pathways regulating endothelial production of nitric oxide share striking parallels with metabolic insulin-signaling pathways. Distinct MAPK-dependent insulin-signaling pathways (largely unrelated to metabolic actions of insulin) regulate secretion of the vasocon

Normal pressure hydrocephalus: an analysis of aetiology and response to shunting based on mathematical modeling.

The dynamics which maintain the state of enlarged cerebral ventricles and normal intracranial pressures (normal pressure hydrocephalus) are not completely understood, making the response to cerebrospinal fluid diversion difficult to predict. Using our previously described mathematical model of intracranial physiology which allows nonlinear relationships of pressure, volume, and flow in 7 distinct compartments, we desired to determine factors which could be responsible for the development and maintenance of the steady state of normal pressure hydrocephalus. Using typical starting values for CSF

Effect of trimazosin on hemodynamics in chronic heart failure.

The effect of the oral vasodilator trimazosin on hemodynamics in 16 patients with chronic left ventricular failure was compared with placebo in a double-blind, randomized study. Eight patients received trimazosin 100 to 300 mg, and 8 received placebo. Over the 6-hr period after medication trimazosin caused no significant change in heart rate, stroke work index, or pulmonary vascular resistance, but there were significant reductions in mean arterial pressure (p less than 0.05), mean right atrial pressure (p less than 0.01), mean pulmonary artery pressure (p less than 0.01), mean pulmonary capil

A Predictive Design Framework for Ultrarobust Superhydrophobic Coatings Based on Lyophobic Interconnected Close-Packed Nanostructures.

Superhydrophobic (SH) coatings have immense potential across numerous applications but remain limited by inadequate pressure resistance, mechanical durability, and environmental stability. Traditional SH architectures, such as hierarchical micro/nanostructures, fail to address these critical challenges. Here, we introduce a conceptually new structural design principle based on lyophobic interconnected close-packed (LICP) nanostructures, derived from quantitative analyses of the coupled relationships among apparent contact angle, capillary pressure, bending stress, and a newly defined robustnes

Identification of plasmid encoded qepA efflux pump gene in Citrobacter freundii isolates from a renowned hospital, Bangladesh.

BACKGROUND AND OBJECTIVES: The emergence of plasmid-mediated resistance to quinolones is a growing global threat that complicates the control of multidrug-resistance (MDR) in Enterobacteriaceae. This study was conducted to determine the frequency of the qepA gene and its association with antibiotic resistance in Citrobacter freundii isolates from various clinical samples. MATERIALS AND METHODS: To conduct this study, a total of 27 C. freundii isolates collected from urine, endotracheal aspirates, sputum, blood, and stool samples were identified using standard biochemical tests and culture met

Molecular pathways involved in the control of contractile and metabolic properties of skeletal muscle fibers as potential therapeutic targets for Duchenne muscular dystrophy.

Duchenne muscular dystrophy (DMD) is caused by mutations in the gene encoding dystrophin, a subsarcolemmal protein whose absence results in increased susceptibility of the muscle fiber membrane to contraction-induced injury. This results in increased calcium influx, oxidative stress, and mitochondrial dysfunction, leading to chronic inflammation, myofiber degeneration, and reduced muscle regenerative capacity. Fast glycolytic muscle fibers have been shown to be more vulnerable to mechanical stress than slow oxidative fibers in both DMD patients and DMD mouse models. Therefore, remodeling skele

Current opinion in management of septic shock due to Gram-negative bacteria.

PURPOSE OF REVIEW: The COVID-19 pandemic has caused multiple challenges to ICUs, including an increased rate of secondary infections, mostly caused by Gram-negative micro-organisms. Worrying trends of resistance acquisition complicate this picture. We provide a review of the latest evidence to guide management of patients with septic shock because of Gram-negative bacteria. RECENT FINDINGS: New laboratory techniques to detect pathogens and specific resistance patterns from the initial culture are available. Those may assist decreasing the time to adequate antimicrobial therapy and avoid unnec

Microcirculation and the metabolic syndrome.

The present review, to the authors' knowledge, is the first to specifically address the relationships between microcirculation and metabolic syndrome, a cluster of metabolic and cardiovascular modifications highly prevalent in the general population. Its close link to overweight and insulin resistance makes it the main cause of the worldwide burden of type 2 diabetes. However, metabolic syndrome is also observed in many other diseases, particularly, but not exclusively, those where insulin resistance is a main feature. Analysis of the literature reveals that this clinical situation is invariab

Surveillance study of bacteraemic episodes in febrile neutropenic children.

We prospectively studied the type, frequency and outcome of infections in 513 patients with 762 consecutive episodes of febrile neutropenia (FN) over a five-year period between 1995 and 1999 in a single paediatric oncology unit. The findings were then compared with a similar study carried out in our unit between 1990 and 1994. The types of bacterial isolates and sensitivity patterns were also studied to identify trends and to gauge the suitability of antibiotics chosen for empirical therapy. Bacteraemia was documented in 35.4% of FN episodes, although 70% of patients did not have an obvious si

Effects of captopril on pulmonary haemodynamics.

The effects of oral captopril on pulmonary haemodynamics were studied in two groups of 6 patients, one of subjects with chronic respiratory failure (PaO2 52 +/- 5.1 mmHg, PaCO2 54 +/- 2.1 mmHg), and the others with chronic heart failure and high plasma renin activity. Two potential mechanisms of its actions were assessed, namely inhibition of hypoxic vasoconstriction and inhibition of the possible effects of angiotensin II on the pulmonary circulation. There were significant (p less than 0.05) decreases in mean arterial pressure, pulmonary wedge pressure and in systemic arterial resistance ass

Modulating cellular deformability via 3D dextran hydrogel cultivation to regulate the microcirculation of mesenchymal stem cells in murine spleen and liver.

For mesenchymal stem cell (MSC) therapy to be effective, the vascular system may be used to deliver and steer the cells to the target tissue. However, the expanded MSCs in petri dishes typically exhibit limited deformability and commonly excluded by the capillary networks when homing to the downstream organs via microcirculation. Here, we propose to utilize specially designed 3D dextran hydrogels and tuning the microscopic heterogeneity of hydrogel composition to make the administrated cells mechanically comply with the structure and mechanics of the capillary. The deformability of cells cultu

Observation of trapped gas during electrical resistance heating of trichloroethylene under passive venting conditions.

A two-dimensional experiment employing a heterogeneous sand pack incorporating two pools of trichloroethylene (TCE) was performed to assess the efficacy of electrical resistance heating (ERH) under passive venting conditions. Temperature monitoring displayed the existence of a TCE-water co-boiling plateau at 73.4°C, followed by continued heating to 100°C. A 5cm thick gas accumulation formed beneath a fine-grained capillary barrier during and after co-boiling. The capillary barrier did not desaturate during the course of the experiment; the only pathway for gas escape being through perforated w

Pulmonary hemorrhage induced by jararhagin, a metalloproteinase from Bothrops jararaca snake venom.

Jararhagin is the most important hemorrhagic component in the venom of the snake Bothrops jararaca, a species of medical importance in South America. It is a P-III zinc-dependent metalloproteinase comprising catalytic, disintegrin-like, and cysteine-rich domains. Jararhagin injected intravenously into mice induced rapid and prominent bleeding in the lungs, whereas other organs were devoid of overt hemorrhagic manifestations. This action depends on the proteolytic activity of jararhagin, since it was abrogated by the synthetic inhibitor batimastat. There were conspicuous ultrastructural alterat

Reduction of pulmonary capillary blood volume in patients with severe unexplained pulmonary hypertension.

BACKGROUND: Unexplained or primary pulmonary hypertension results in an obliteration and obstruction of resistance pulmonary arteries. In these patients gas exchange is impaired and the measurement of gas transfer for carbon monoxide is usually reduced. This has been thought to represent a reduction in pulmonary alveolar capillary blood volume (Vc). A single breath test, measuring simultaneously the uptake of both nitric oxide (NO) and carbon monoxide (CO), provides a simple and practical measurement of membrane diffusion (Dm) and Vc. METHODS: A standard single breath test for the measurement

Targeted therapy for capillary-venous malformations.

Sporadic venous malformations are genetic conditions primarily caused by somatic gain-of-function mutation of PIK3CA or TEK, an endothelial transmembrane receptor signaling through PIK3CA. Venous malformations are associated with pain, bleedings, thrombosis, pulmonary embolism, esthetic deformities and, in severe cases, life-threatening situations. No authorized medical treatment exists for patients with venous malformations. Here, we created a genetic mouse model of PIK3CA-related capillary venous malformations that replicates patient phenotypes. We showed that these malformations only partia

[The histochemical and histoenzymological changes in the gastric mucosa and the state of the connective tissue in peptic ulcer patients during dynamic rehabilitative treatment].

Combined differentiated rehabilitative and staged treatment of 167 patients with peptic ulcer resulted in stable reduction in cellular membrane permeability, subsiding of catabolic processes in cells, steady normalization of enzymic activity of tissue respiration, as well as activation of reparation processes in connective tissue elements and structures located in the connective tissue (capillaries included). The described mode of treatment appeared to be superior to the conventional one, resulting in a substantial improvement in microcirculation in gastric mucosa. Normalization of protein-syn
