Global inactivation of the metalloproteinase ADAM17 during mouse development results in perinatal lethality and abnormalities of the heart, including late embryonic cardiomegaly and thickened semilunar and atrioventricular valves. These defects have been attributed in part to a lack of ADAM17-mediated processing of HB-EGF, as absence of soluble HB-EGF results in similar phenotypes. Because valvular mesenchymal cells are largely derived from cardiac endothelial cells, we generated mice with a floxed Adam17 allele and crossed these animals with Tie2-Cre transgenics to focus on the role of endoth…

Excessive release or administration of beta-mimetic catecholamines may induce cardiomegaly, necrotic lesions and accumulation of connective tissue in the heart of adult homoiotherms. It was examined here whether similar changes can also be observed at different stages of evolution of the cardiovascular system, i.e. in poikilotherms and in homoiotherms during embryonic life. Sensitivity of the poikilothermic hearts (carp, frog, turtle) to isoproterenol (IPRO) was significantly lower than in the homoiotherms. Necrotic lesions, if present, were localized in the inner spongious musculature which h…

Anomalous coronary arteries (ACAs) are a potential cause of myocardial ischemia resulting in arrhythmias, angina, infarction, and sudden dea … cardiac defects. Growth retardation was noted in 50% of the cases. Examination of the heart showed cardiomegaly in 92% of cases, which was marked (> 95 percentile) in 63%, but histologically apparent ischemic damage was seen in only 50% of cases. ACAs arising from the pulmonary trunk were associated with earlier death (1.4 years), more frequent cardiomegaly ( …

This joint work has studied the cardiomyopathies occurring in hereditary neuro-muscular disorders (270 cases). The Duchenne type of disorder (74 cases) was responsible for asystole (4 cases), for cardiomegaly, and especially for abnormalities of the ECG (59 cases)--Q waves and large R waves in V1 and V6. The cardiomyopathy was of the hypokinetic type, with histological evidence of degeneration of the myocardial fibres. Dystrophia myotonica of Steinart (23 cases) caused conductive disorders (17 cases) which were either atrioventricular or intra-ventricular or both. Studies of the His pathway co…

Ankylosing spondylitis is one of the subgroup of diseases called "seronegative spondyloarthropathy". Frequently, it affects the vertebral colon and sacroiliac joint primarily and affects the peripheral joints less often. This chronic, inflammatory and rheumatic disease can also affect the extraarticular regions of the body. The extraarticular affections can be ophthalmologic, cardiac, pulmonary or neurologic. The cardiac affection can be 2-10% in all patients. Cardiac complications such as left ventricular dysfunction, aortitis, aortic regurgitation, pericarditis and cardiomegaly are reviewed.

Chagas' Disease is common in Central and South America. As the number of immigrants increase in the United States and North America the incidence of Chagas' Disease is increasing. This is a report of chronic Chagas' Disease diagnosed on the Gulf Coast. A review of the recent literature suggests seropositivity in the United States to be approximately 350,000 people with approximately 100,000 cases of chronic Chagas' Disease. The triad of cardiomegaly, megaesophagus, and mental disease are common symptoms. A case report, literature review, epidemiology, and diagnostic studies are presented.

An incomplete atrioventricular block of grade III was detected by electrocardiography in a 3.5-year-old boy one week before his sudden death. On autopsy, cardiomegaly involving a 47% increase in heart weight and a moderate adaptational endocardial fibroelastosis were noted, as was a nodular tumor sized 6 X 3 X 1.5 mm at the atrioventricular node. The tumor was concluded to be a microcystic mesothelioma that had been destructive to the node and had begun infiltrating the cardiac muscle.

In a 50-year-old man presenting with dyspnoea and palpitations, cardiomegaly, incomplete right bundle branch block and bursts of ventricular tachycardia, Two-Dimensional Echocardiography revealed an impressive enlargement of the right ventricle, particulary in the outflow tract. Arrhythmogenic right ventricular dysplasia was suggested and confirmed by right ventricular angiography and electrophysiologic study. We emphasize the role of Two-Dimensional Echocardiography in the appropriate planning of cardiac catheterization for a definitive diagnosis of arrhythmogenic right ventricular dysplasia.

BACKGROUND: Cardiomegaly detected on chest radiography (CXR) is frequently encountered in clinical practice, even in asymptomatic individuals. However, its diagnostic value for identifying underlying abnormal cardiac structures in this population remains uncertain. This study investigated the predictors for the structural echocardiographic abnormalities among asymptomatic individuals with increased cardiothoracic ratio detected on chest radiography.Methodology.This is a cross-sectional study of asymptomatic adults who underwent routine health check-ups with increased cardiothoracic ratio (CTR)…

This study hypothesized that sympathetic … s were altered in dogs with MVD before clinical signs were observed and prior to the development of cardiomegaly. This is the first HRV study in untreated dogs with MVD. Investigating ANS imbalances preceding subclinical left-sided cardiomegaly in dogs with MVD should be considered in future age-matched, longitudinal studies to determine the potential benefits …

Background: Patients with chronic kidney disease (CKD) are associated with high prevalence rates of proteinuria, vascular calcification and cardiomegaly. In this study, we investigated relationships among proteinuria, aortic arch calcification (AoAC) and cardio-thoracic ratio (CTR) in patients with CKD stage 3A-5. In addition, we investigated correlations among proteinuria and decline in renal function, overall and cardiovascular (CV) mortality. Methods: We enrolled 482 pre-dialysis patients with CKD stage 3A-5, and determined AoAC and CTR using chest radiography at enrollment. The patients we…

Hereditary Hemorrhagic Telangiectasia (HHT), commonly known as Osler-Weber-Rendu disease, is an autosomal dominant multisystemic vascular disease associated with approximately 70% of cases of pulmonary arteriovenous malformations (PAVMs). Prenatal cases of PAVMs typically present with pulmonary vein dilatation on ultrasonography. This study presents a prenatal diagnosis of PAVMs with enlarged right pulmonary vein, cardiomegaly, cystic-appearing areas in the right lung and subsequent confirmation of Osler-Weber-Rendu syndrome using autopsy and whole exom sequencing.

An 8-yr-old male meerkat (Suricata suricatta) presented with sudden lethargy. A globular-shaped heart associated with pleural effusion on chest thoracic radiographs was consistent with congestive heart failure, and echocardiography was performed under general anesthesia. It revealed an Ebstein anomaly, with mild pericardial effusion and marked right heart enlargement. The animal was treated with imidapril chlorydrate. After 4 mo of treatment, thoracic radiographs still showed right-sided cardiomegaly; however, the animal appeared clinically normal.

Systolic murmurs were detected in 22 (61%) of the 36 children with sickle cell anaemia (SCA) who completed the study. Cardiomegaly was detected in 14 (39%). Mean values of left and right ventricular dimensions were higher in SCA than in controls (p < 0.05). Left atrial chambers and aortic root dimensions followed the same pattern. The dilated cardiac chambers in SCA were not associated with any abnormality in systolic or diastolic left ventricular function nor with significant pulmonary hypertension.

Intracardiac rhabdomyosarcoma was diagnosed in a 14-month-old Golden Retriever referred for evaluation of syncope. The dog was weak, with irregular heart rate, pulse deficit, heart murmur, and hepatomegaly. Thoracic radiography revealed generalized cardiomegaly, and electrocardiography revealed sinus rhythm with multiform ventricular premature complexes and intermittent ventricular tachycardia. Two-dimensional echocardiography revealed extensive soft-tissue density masses in both ventricles. An intracardiac neoplasm seen at postmortem examination was identified histologically as rhabdomyosarco…

In 1980 a large series of patients with Lyme carditis was first described. The most frequent symptom was atrioventricular block, more unusual were other arrhythmias, nonspecific ECG alterations, cardiomegaly and pericarditis. We report the case of a 51-year-old man with Lyme carditis which presented as atrial fibrillation and atrioventricular block 2 degrees. Gallium-67 scan was negative. The arrhythmias ceased during therapy with doxycycline. The clinical findings, diagnosis and therapy of Lyme carditis are discussed.

A 7-week-old infant with a 1-week history of a SARS-CoV2 respiratory infection presented with tachypnea. Cardiomegaly was noted on chest roentgenogram. Echocardiogram showed a large pericardial effusion, with tamponade physiology and a large pericardial mass. Serosanguinous fluid was noted on pericardiocentesis. Cardiac magnetic resonance imaging revealed a well-circumscribed vascular mass suggestive of a pericardial hemangioma. Serial echocardiograms showed reaccumulation of the pericardial effusion. The mass was surgically removed. A feeding vessel from the right coronary artery was ligated.

Murine typhus is a rare condition caused by the gram-negative bacterium Rickettsia typhi which classically presents with the triad of fever, rash, and headache. Herein we report a rare presentation of murine typhus in an adult who presented predominantly with fever and gastrointestinal symptoms. Initial imaging showed cardiomegaly and hepatosplenomegaly, which led to further workup revealing rickettsial disease. Although this illness is considered a rare diagnosis in the emergency department, every person with a pet that might have fleas is susceptible to it.

Traumatic renal arteriovenous fistula involving the inferior vena cava (IVC) are exceptionally rare, but if left untreated can have devastating clinical consequences, including development of renovascular hypertension, cardiomegaly, and congestive heart failure. We report a rare, pediatric case of a renal-caval arteriovenous fistula that developed after a gunshot wound to the abdomen and its subsequent treatment with endovascular means. We review our case and the world literature on the evaluation and management of trauma-related renal-caval arteriovenous fistulae.
