This is a case report of a young woman, who after a successful septorhinoplasty procedure, sustained repeated nasal trauma with a subsequent diagnosis of multiple sclerosis (MS) at a large tertiary hospital in Riyadh, Saudi Arabia. A 24-year-old woman with a history of childhood trauma presented with difficulty in breathing and dissatisfaction with her nasal appearance. After a successful and uneventful septorhinoplasty, she required numerous hospital admissions due to multiple episodes of blunt nasal trauma, culminating in clear nasal discharge and neurological symptoms, including dizziness,

INTRODUCTION: Psoriasis is an inflammatory skin disease that is characterized by T-cell-mediated hyperproliferation of the keratinocytes. It develops through immune-mediated mechanisms and is defined as an immune-mediated inflammatory disease. The inner ear is susceptible to inflammatory attacks, and vertigo and dizziness can occur as a complication. There is little information about psoriasis and the vestibular system.
OBJECTIVE: This study aimed to investigate the cervical vestibular-evoked myogenic potential (cVEMP) results of psoriasis patients and the effect of psoriasis on the vestibula

The diagnosis of bilateral vestibulopathy (BV) is typically established based on bilateral semicircular canal dysfunction. The degree to which both otolith organs-the saccule and utricle-are also impaired in BV is not well-established, particularly with respect to the etiology and severity of BV. The aim of this study was to evaluate semicircular canal, saccular and utricular function in patients with BV due to aminoglycoside ototoxicity and bilateral Menière's disease, and with different severities of BV. Caloric and head impulse testing were used as measures of canal function. Cervical vesti

BACKGROUND: A small number of reports have described subarachnoid hemorrhage resulting from a ruptured aneurysm embedded within a prolactinoma. To the best of our knowledge, no reports have described an embedded carotid cavernous fistula. We report a patient with carotid cavernous fistula secondary to a ruptured internal carotid artery aneurysm embedded within a prolactinoma.
CASE DESCRIPTION: A 61-year-old woman was referred to our hospital with sudden headache, vomiting, and dizziness. Magnetic resonance imaging demonstrated a small acute subdural hematoma, recurrent prolactinoma, and left

PURPOSE: Despite its effectiveness in other surgeries, studies on continuous epidural block in upper-extremity surgery are rare because of technical difficulties and potential complications. This study compared postoperative analgesic efficacy and safety of ultrasound-guided continuous interscalene brachial plexus block (UCISB) and fluoroscopy-guided targeted continuous cervical epidural block (FCCEB) in arthroscopic rotator cuff repair (ARCR).
METHODS: Seventy-five patients were randomly and equally assigned to groups FCCEB (0.2%), UCISB75 (0.75%), and UCISB20 (0.2%) according to the initial

BACKGROUND: Benign paroxysmal positional vertigo is one of the commonest peripheral vestibular causes of vertigo. The particle repositioning manoeuvre (Epley's manoeuvre) has become the 'gold standard' treatment for this disorder. Benign paroxysmal positional vertigo can affect any age group but is commoner in older patients. Cervical spine problems (e.g. spondylosis and disc prolapse) are commoner in this age group. Epley's manoeuvre necessitates passive neck movements. Such movements may not be wise in patients at risk of cervical spine fracture.
PATIENTS AND METHODS: This study included 40

BACKGROUND: There is increasing recognition of connective tissue disorders and their influence on disease in the general population. A conserved clinical phenotype involving connective tissue disorders and idiopathic intracranial hypertension (IIH) and associated cerebral venous outflow disorders (CVD) has not been previously described.
METHODS: A single-institution retrospective review of a prospectively maintained database of patients with connective tissue disorders and CVD was performed.
RESULTS: A total of 86 patients were identified. The majority of these patients carried a diagnosis o

BACKGROUND: Patients with hypermobile Ehlers-Danlos Syndrome (hEDS) are at risk of developing craniocervical instability (CCI). Iatrogenic causes of CCI in hEDS are underreported. We describe a case series of hEDS patients who developed CCI from routine neck hyperextension during common procedures (e.g., intubation for anesthesia, dental work). METHODS: We conducted a retrospective case series of 8 adults (≥ 18 years old) with hEDS (2017 diagnostic criteria) who had no preoperative CCI symptoms but developed new CCI symptoms after a surgical procedure that persisted for at least 5 years. Eagle syndrome is a symptomatic disorder associated with elongation or abnormal orientation of the styloid process and/or ossification of the stylohyoid complex. Clinical manifestations depend on the spatial relationship between the styloid complex and adjacent neural, muscular, fascial, and vascular structures. We describe an ultrasound-guided peristyloid hydrodissection technique using 5% dextrose in water (D5W) that integrates preprocedural computed tomography (CT), sequential landmark-based sonographic localization, live-dual imaging, Doppler vascular mapping, and orthogonal short- and lon

Neuromyelitis optica spectrum disorder is an autoimmune demyelinating disease that usually targets the spinal cord and optic nerves, though occasionally other central nervous system structures are involved. There are variants of neuromyelitis optica spectrum disorder that can mimic metabolic or osmotic demyelinating diseases on imaging. A 54-year-old female with a history of hypertension and Roux-en-Y bypass presented with numbness, weakness, dizziness, and blurred vision and concurrent severe hyponatremia (serum sodium 115 mmol/L). OBJECTIVE: Middle ear surgery is frequently associated with significant postoperative pain, nausea, and vomiting. Such issues have prompted clinicians to adopt regional nerve blocks, such as the superficial cervical plexus block (SCPB) and the great auricular nerve block (GANB). Our aim was to evaluate the efficacy of regional nerve blocks in adult patients undergoing different middle ear surgeries.

BACKGROUND: Breast cancer has become the second most common cancer after lung cancer. Patients may present with skin manifestations at the time of initial diagnosis, while erysipel-like carcinoma typically appears later, following initial treatment. This delay increases the risk of misdiagnosis.
CASE PRESENTATION: The patient was a 51-year-old female. A modified radical mastectomy for left breast carcinoma (pT2N3M0, stage IIIC; tumor size 4.6 cm × 4.5 cm × 1.6 cm, 14/21 axillary lymph nodes involved), HER2-positive type, was performed on April 21, 2021. We present a case of middle-aged woman whose health problems began 3 months after a registered tick bite in endemic area of Lyme borreliosis. First symptoms included fatigue, chills, cervical lymphadenopathy, neck pain and stiffness. Patient was afebrile. Lyme disease was excluded due to lack of erythema migrans and negative enzyme immunoassay test results for anti-Borrelia antibodies. During the next few months, her condition was getting worse and symptoms were accompanied with brain fog, dizziness, palpitations, irregular menstrual cycles, insomnia, panic attacks, headaches, and muscle aches

INTRODUCTION: While the cochleotoxicity of cisplatin has been well investigated, less is known about the effects of platinum-based chemotherapy on the vestibular system. In particular, there is a lack of prospective studies using modern laboratory vestibular testing that examine the effects of cisplatin on the semicircular canals and on the otolith organs. The aim of the present study was, therefore, to investigate the vestibulotoxic effect of cisplatin in patients with head and neck tumors who are undergoing chemoradiation.
METHODS: Forty-five patients undergoing cisplatin-based chemoradiati
