[Ganglioneuroma derived from inferior trunk of the brachial plexus and first intercostal nerve].

A 62-years-old Japanese male, who had mediastinal tumor at the left thoracic inlet, was admitted to our hospital to receive surgical treatment. The tumor behind the left subclavian artery was guessed to be neurogenic benign tumor, though the involvement of the brachial plexus was unclear. We approached the tumor by means of left hemi-collar skin incision, resulting in performing safe operation with directly looking at the tumor that communicated with 1st intercostal nerve and inferior trunk of the left brachial plexus. Pathological diagnosis of the resected tumor was ganglioneuroma. Cervical a

MR neurography. MR imaging of peripheral nerves.

Recent advances in MR imaging coupled with specially designed phased-array surface coils are revolutionizing imaging of the peripheral nervous system. Direct visualization of normal-sized major peripheral nerves within the body is now possible. This article describes the appearance of normal peripheral nerves together with imaging characteristics of various types of nerve pathology including traumatic injury, compressive syndromes, and neural tumors. Imaging of the brachial plexus, lumbosacral plexus, carpal tunnel, cubital tunnel, and cervical nerves is illustrated and discussed. MR neurograp

Kikuchi-Fujimoto disease complicated with brachial plexus neuritis in a young woman: a case report and review of the literature.

BACKGROUND: Kikuchi-Fujimoto disease (KFD) is a rare necrotizing lymphadenitis. Brachial plexus neuritis is an exceptionally rare complication of KFD. CASE PRESENTATION: A 31-year-old Asian female was diagnosed with KFD via excisional biopsy of a posterior cervical lymph node. Four days after the biopsy, pain and disability occurred in her left arm and shoulder. While Magnetic Resonance Imaging (MRI) suspected compression of the brachial plexus nerves, the ultrasound indicated inflammatory changes consistent with neuritis. Diagnosis was challenging; however, distinguishing between mechanical

The Role of Ultrasound in Venous Thoracic Outlet Syndrome: Lesson Based on a Case Report.

Thoracic outlet syndrome is a rare neurovascular disease, usually caused by compression of subclavian vessels and/or brachial plexus by a cervical rib or hypertrophic scalene muscles. When the subclavian artery is compressed, it can cause perfusion deficiency with cyanosis and greater fatigue of the upper limb; if the subclavian vein is compressed, it can cause venous congestion with swelling and pain in the upper limb. When compression affects the brachial plexus, it can cause tingling or paresthesia of the upper limb. It can be mono or bilateral, congenital, or acquired. The diagnosis of vas

Arterial embolism from anatomical variation at the thoracic outlet.

Thoracic outlet syndrome (TOS) represents a constellation of symptoms arising from the compression of the neurovascular bundle as it exits the thorax. We report a unique case of multiple rare anatomical anomalies resulting in TOS manifested by distal arterial embolism. These anomalies include the combination of: (1) a unilateral right cervical rib, Gruber's type II (Gruber 1869), (2) nonunion of the first thoracic rib, (3) abnormal fibrous insertions of the anterior scalene muscle onto the epineurium of the brachial plexus and adventitia of the subclavian artery, (4) anterior position of the b

Neurogenic thoracic outlet syndrome secondary to an unilateral cervical rib in a poodle mix.

A 2-year-old spayed female poodle mix was evaluated for persistent intermittent right thoracic limb lameness and pain refractory to oral analgesic and anti-inflammatory medications. Orthopedic and neurologic physical examinations localized discomfort to the right shoulder and cervical region. Orthogonal right shoulder radiographs did not detect any abnormalities. Magnetic resonance imaging of the right brachial plexus revealed an anomalous bone structure at the level of the right thoracic inlet. Orthogonal cervical radiographs confirmed this finding. Surgical exploration of the right thoracic

Transnasal odontoid resection: is there an anatomic explanation for differing swallowing outcomes?

OBJECT: Swallowing dysfunction is common following transoral (TO) odontoidectomy. Preliminary experience with newer endoscopic transnasal (TN) approaches suggests that dysphagia may be reduced with this alternative. However, the reasons for this are unclear. The authors hypothesized that the TN approach results in less disruption of the pharyngeal plexus and anatomical structures associated with swallowing. The authors investigate the histological and gross surgical anatomical relationship between pharyngeal plexus innervation of the upper aerodigestive tract and the surgical approaches used (

Evaluation of traumatic cervical nerve root injuries by intraoperative evoked potentials.

OBJECTIVE: To evaluate intraoperative evoked potentials as a diagnostic tool in traumatic brachial plexus injuries. METHODS: Thirteen patients with traumatic brachial plexus injuries were investigated by intradural nerve root inspection (n = 28 roots) via cervical hemilaminectomy to assess or rule out nerve root avulsion from the spinal cord. Two to 8 weeks later, evoked potentials from neck and scalp were recorded after direct electrical nerve root stimulation close to the vertebral foramen during operative brachial plexus repair via an anterior (supraclavicular and infraclavicular) approach

[Method for studying spinal evoked potentials].

The authors describe a new method of examing evoked spinal potentials. Derivation is by means of unipolar needle electrodes which are ventrolaterally introduced into the cervical disk space as far as the posterior longitudinal ligament. Detection of a spinal potential following the stimulation of brachial and crural nerves allows to exclude the possibility of complete transverse lesion of the spinal cord or severing of the peripheral nerve or plexus.

Zero echo time MRI for detecting osseous anomalies in neurogenic thoracic outlet syndrome.

OBJECTIVE: To evaluate the additive value of a zero echo time (ZTE) sequence to standard brachial plexus MR neurography (MRN) for detecting and characterizing osseous anomalies in patients with suspected neurogenic thoracic outlet syndrome (nTOS). MATERIALS AND METHODS: In this retrospective, case-control study (January 2022-March 2025), unilateral brachial plexus MRN examinations with ZTE and reference imaging (radiographs and/or CT) were included. Two board-certified radiologists determined case status and osseous anomaly subtype (cervical rib, rib synostosis, or elongated C7 transverse pro

Neuroimaging in Leprosy: A Case Series Exploring Domains Beyond Peripheral Nerves.

Background and objective Leprosy, an infectious disease caused by Mycobacterium leprae, induces irreversible damage, necessitating early detection. The disease's neurotropism, which extends to the peripheral nerves, is well recognized. In recent years, there has been an acknowledgment of its expanding horizon beyond this traditional boundary. This study aims to explore the involvement of atypical sites such as the central nervous system, brachial plexus, and spinal nerve root ganglion through dedicated neuroimaging via magnetic resonance imaging (MRI) across diverse spectra of leprosy. Methods

Microsurgical anatomy of dorsal root entry zone of brachial plexus.

The purpose of this study was to describe microsurgical anatomy of the dorsal root entry zone (DREZ) and provide an anatomic basis for the approach of DREZ lesion in treating radiculopathy of brachial plexus avulsion injuries. We studied 100 dorsal cervical roots and DREZ/posterior horn complexes in 20 adult cadavers. At each root level the following data were recorded: widths of laminectomy, numbers of posterior rootlets, angle of the inferior rootlets with the spinal cord, and distance from posterior median sulcus to posterolateral sulcus. On cross-sectional plane, the length and width of do

Functioning transferred free muscle innervated by part of the vascularized ulnar nerve connecting the contralateral cervical seventh root to themedian nerve: case report.

BACKGROUND: The limited nerve sources available for the reconstruction and restoration of upper extremity function is the biggest obstacle in the treatment of brachial plexus injury (BPI). We used part of a transplanted vascularized ulnar nerve as a motor source of a free muscle graft. CASE PRESENTATION: A 21-year-old man with a left total brachial plexus injury had received surgical intercostal nerve transfer to the musculocutaneous nerve and a spinal accessory nerve transfer to the suprascapular nerve in another hospital previously. He received transplantation of a free vascularized gracili

[Angiographically occult high cervical dural arteriovenous malformation secondary to intracranial hypertension: a case report].

We have recently encountered a case of high cervical arteriovenous malformation which caused subarachnoid hemorrhage. A 54-year-old male was admitted to our hospital due to sudden onset of severe headache. Neurological examination revealed right sixth nerve palsy which suggested elevated intracranial pressure. A CT scan showed subarachnoid hemorrhage predominantly in the posterior fossa, massively ventral to the brain stem, with reflux into the fourth ventricle. No abnormal vascular lesions were found in the first cerebral angiography which included four vessels and bilateral thyrocervical tru
