STUDY OBJECTIVE: To evaluate an alternative monitoring protocol without day 4 β-human chorionic gonadotropin (β-hCG) measurement for predicting the need for a repeated methotrexate (MTX) dose in patients undergoing single-dose MTX therapy for ectopic pregnancy (EP).
DESIGN: Single-center retrospective study (Canadian Task Force classification II-3).
SETTING: University-affiliated hospital.
PATIENTS: Included in the study were 184 EP patients treated with MTX between January 2009 and December 2016.

OBJECTIVE: The finding of persistent low-level human chorionic gonadotropin (hCG) with or without a preceding pregnancy event presents a rare but clinically important challenge and a therapeutic dilemma. These are patients with "real" hCG shown by the positive test in both serum and urine or by specialized testing. Human chorionic Gonadotropin (HCG) therapy has been used beneficially in men with subfertility especially after varicocele surgery. Very few side effects have been observed in male patients. Herein, we report a case of pulmonary embolism in a male patient following the use of HCG. The patient was subsequently found to have type IV hyperlipoprotenemia. The etiology of the thrombo-embolic episode is conjectural, but this case emphasizes the need for stricter criteria of patient selection and screening for conditions that predispose to thrombo-embolism before starting HCG therapy.

We recently described the generation of mouse monoclonal antibodies directed against hCG using a synthetic peptide immunogen: the carboxy-terminal peptide (CTP) of beta-hCG. Using a sensitive measuring device, 3-day hCG administration (Pregnyl; 1500 IU daily) was shown to temporarily increase ulnar growth velocity from prepubertal (0.40 +/- 0.35 mm/3 weeks to pubertal values (1.1 +/- 0.64 mm/3 weeks) in 10 boys with delayed puberty. Plasma prolactin, estradiol, progesterone, and testosterone, but not HCG-beta levels, were higher in a patient who developed the ovarian hyperstimulation syndrome while undergoing ovulation induction with human gonadotropins than in two other women who also became pregnant after similar treatment without complications. These results suggest that hyperprolactinema, in association with elevated ovarian steroid levels, may be factors in the pathogenesis of this disorder.
