BACKGROUND: The clitorophallus, or glans, is a critical structure in sexual development and plays an important role in how gender is conceptualized across the life span. This can be seen in both the evaluation and treatment of intersex individuals and the use of gender-affirming masculinizing therapies to help those born with a clitoris (small clitorophallus with separate urethra) enlarge or alter the function of that structure.

BACKGROUND: Labia minora reduction has become part of the treatment spectrum offered by most plastic surgeons. INTRODUCTION: We present a refinement to our original technique in MtF gender reassignment surgery. Our goal was to construct a neoclitoris, which is wet and covered with urethral neoprepuce. Since 1995 more than 300 transgender MtF patients have been operated at our institution. Clitoral hypertrophy is usually seen as a congenital malformation, specifically during the stages of hormonal expressions in the disorders of sexual development. Acquired clitoral hypertrophy is a relatively rare condition. Clitoris can be the site of neoplastic lesions. Clitoral choristoma has not been reported so far in the literature.

A female French bulldog was presented with an enlarged clitoris. Abdominal surgery revealed a normal uterus and gonads resembling testes. Histologically, the gonads contained seminiferous tubules. The karyotype was XX, and the SRY gene was not detected. A diagnosis of XX male, SRY negative disorder of sexual development was made.

A preterm female infant was diagnosed with congenital adrenal hyperplasia after the first month of life. Electrolyte abnormalities and prominent clitoris were originally attributed to the prematurity of the infant. Congenital adrenal hyperplasia was confirmed when the abnormalities persisted. In 246 patients with vulvar tumors of the Landesfrauenklinik Magdeburg from 1972 to 1982 we could find 5 women (2%) with tumors involving the clitoris. Two of the patients suffered from a carcinoma. The three other patients had benign lesions, one a hidradenoma, one a retention cyst and one a myxofibroma.

The author elaborated a method of creation of an artificial vagina in patients with false male hermaphroditism during puberty from the skin of the removed clitoris, with the correction of external genitalia in the female direction. This method is harmless, the operative technique is simple. Positive anatomical and cosmetic results were obtained in two patients with the mentioned pathology.

For transgender women, genital adjustment surgery involves removal of the natal reproductive organs and creation of a neovagina, vulva and clitoris.
