Impact of the COVID-19 pandemic on Clostridioides difficile infection in a tertiary healthcare institution in Belgium.

OBJECTIVES: Clostridioides difficile infection (CDI) causes the greatest number of healthcare-associated infectious diarrhoea. CDIs are transmitted by direct and indirect patient-to-patient contact and risk increases with the use of antibiotics. Since early 2020, the COVID-19 pandemic has affected healthcare systems in many ways including substantial changes in hygiene behaviour. The aim of this study was to assess whether CDI incidence differed during the COVID-19 pandemic compared to a year before. METHODS: All tests for suspected CDI cases were recorded for a hospital in Brussels, Belgium.…

Retrospective Study of the Epidemiology of Clostridioides difficile Infection in the Neurosurgery Department of a Tertiary Hospital in China.

BACKGROUND: Although the epidemiology of Clostridioides difficile is important, few studies examining transmission of C. difficile have been reported, especially in wards with low detection rates, such as neurosurgery departments. PURPOSE: This retrospective study investigated the epidemiology of C. difficile infection in a neurosurgery department over a 24-month period, particularly examining the transmission of C. difficile using whole-genome sequencing (WGS). METHODS: Clostridioides difficile strains were isolated and identified from fecal samples of neurosurgical patients. Toxigenic stra…

Prevalence, Risk Factors, and Sequelae of Asymptomatic Clostridioides difficile Colonization in Children with Cystic Fibrosis.

Patients with CF (pwCF) have high antibiotic use and an altered intestinal microbiome, known risk factors for infection with Clostridioides difficile. However, in adults with CF, C. difficile infection (CDI) is uncommon and asymptomatic colonization with C. difficile occurs frequently, for reasons that remain unclear. We investigated the rate, risk factors, and sequelae of asymptomatic C. difficile colonization in children with CF (cwCF). We identified that 32% of cwCF were colonized with C. difficile without acute gastrointestinal symptoms. Higher BMI and exposure to specific antibiotic class…

The WalRK Two-Component System Is Essential for Proper Cell Envelope Biogenesis in Clostridioides difficile.

The WalR-WalK two-component regulatory system (TCS) is found in all Firmicutes, in which it regulates the expression of multiple genes required for remodeling the cell envelope during growth and division. Unlike most TCSs, WalRK is essential for viability, so it has attracted interest as a potential antibiotic target. In this study, we used overexpression of WalR and CRISPR interference to investigate the Wal system of Clostridioides difficile, a major cause of hospital-associated diarrhea in high-income countries. We confirmed that the wal operon is essential and identified morphological defe…

Clostridioides difficile in Food-Producing Animals in Romania: First Study on the Prevalence and Antimicrobial Resistance.

At present, the epidemiology of the gastrointestinal disease caused by Clostridioides difficile (C. difficile) is starting to be slowly elucidated internationally, although information about the bacteria in the food supply chain is insufficient and, in many countries, even absent. The study was conducted in order to investigate the prevalence of C. difficile isolated from animal feces, as well as to determine the antimicrobial susceptibility of such isolates. The presence of antibiotic resistance determinants has also been evaluated. Overall, a total of 24 (12.5%) C. difficile isolates were re…

Implementation of a Clostridioides difficile sentinel surveillance system in Germany: First insights for 2019-2021.

OBJECTIVES: Clostridioides difficile is a major cause of nosocomial diarrhea. Several "hypervirulent" lineages such as ribotype 027 (RT027) and RT078 are of high epidemiological importance, leading to outbreaks and more severe courses of disease. An active surveillance system targeting molecular epidemiology and corresponding antimicrobial resistance has not been established in Germany. METHODS: Since October 2019, University Hospitals throughout Germany collected by two dates every year (1st April and October, respectively) their first ten unselected samples being tested positive for C. diff…

Is Increased BMI a Risk Factor for Developing Severe Clostridioides Difficile Infection? A Retrospective Study.

BACKGROUND: Obesity is associated with a relative increase in bacterial phyla like firmicutes, which helps in the colonization of Clostridioides Difficile. HYPOTHESIS: Individuals with increased BMI (greater than 25) are more susceptible to severe Clostridioides Difficile infection (CDI). METHODS: Data was collected by retrospective chart query. Severe CDI was defined as a white blood cell count of more than 15,000 (x 109 cells/L) or serum creatinine levels greater than 1.5 mg/dL. To examine the association between the primary outcome (severe CDI) and BMI, the factors of age, gender, albumin…

Acquisition of a gene cluster in Clostridioides difficile PCR ribotype 023 strains enables xylitol utilization.

Hundreds of ribotypes of the gastrointestinal pathogen Clostridioides difficile have emerged over the last three decades, yet the factors driving their emergence are poorly understood. Recently, there has been an increase in infections caused by PCR ribotype 023 (RT023) strains in Europe. We profiled the growth of seven RT023 strains in 190 unique carbon sources and found they were able to grow in xylitol, a sugar alcohol used as a food additive in humans and animals. Other ribotypes of C. difficile tested (n = 19) displayed little to no growth in 0.5% xylitol and were growth-inhibited in high…

Pangenome-GWAS analysis of Clostridioides difficile resistant traits from healthcare- and non-healthcare-associated settings.

Clostridioides difficile (C. difficile) is a gastrointestinal pathogen that causes healthcare-associated infections with numerous mechanisms to acquire and confer antimicrobial resistance (AMR) and rapidly adapt to novel environments. These environments extend beyond healthcare, and encompass non-healthcare-related settings, where antimicrobials engender distinct selection pressures. Employing a dataset of 3870 whole-genome sequences of C. difficile from healthcare and non-healthcare environments, this study investigates the selection pressures associated with each setting and its impact on th…

Novel surveillance design exploiting network analysis for efficient, rapid detection of emerging Clostridioides difficile strains.

Whole genome sequencing is being introduced in England to support national surveillance of key hospital-acquired pathogens, starting with Clostridioides difficile, to facilitate novel strain identification and enable timely interventions. However sequencing capacity is limited. Symptomatic and asymptomatic patients attending multiple hospitals can act as inter-facility transmission vectors; here we therefore identify the empirical network of shared patients from analysing national admission data and simulate spread of a hypothetical novel strain. Algorithmically optimising detection, incorpora…

Development of Peptide Glucosyltransferase Inhibitors With Comprehensive Coverage Across Clostridioides difficile Toxin B Sub-Types.

Clostridioides difficile infection presents an escalating clinical challenge due to the proliferation of hypervirulent and antibiotic-resistant strains. The primary symptoms of disease, namely colitis and diarrhea, are induced by the release of two toxins: TcdA and TcdB. Targeting these toxins with peptide inhibitors provides an attractive therapeutic strategy that can be used alone or synergistically with standard antibiotic treatments to alleviate severe symptoms and reduce the risk of resistance development. In this study, we present the rational discovery and optimization of potent TcdB pe…

Virtual Screening and Multistage Computational Profiling of Small-Molecule Inhibitors Targeting Clostridioides difficile Toxin B.

Clostridioides difficile infection (CDI), a leading cause of antibiotic-associated diarrhea, is driven by the virulence factor toxin B (TcdB), whose receptor-binding interfaces (RBIs) exhibit evolutionary divergence into α and β subtypes with distinct host-receptor specificities. Leveraging this insight, we developed a multistage computational pipeline to repurpose FDA-approved drugs as pan-RBI, pan-subtype TcdB inhibitors. Virtual screening of 10,027 compounds against AlphaFold3-predicted TcdB1-8 structures, integrated with molecular docking (AutoDock Vina/LeDock), MM/GBSA binding free energy…

Clostridioides difficile co-infection worsens prognosis in inflammatory bowel disease in patients with cytomegalovirus colitis.

BACKGROUND: Cytomegalovirus (CMV) colitis and Clostridioides difficile infection (CDI) are both linked to disease exacerbation and poor prognosis in patients with inflammatory bowel disease (IBD). Nonetheless, the effect of co-infection on clinical outcomes in individuals with IBD remains underexplored. This retrospective study was designed to assess the clinical outcomes and determine predictors of co-infection with CMV and CDI in individuals with IBD. METHODS: This analysis involved hospitalized patients with IBD and confirmed CMV colitis (based on intestinal CMV immunohistochemical stainin…

A case of community-acquired Clostridioides difficile infection causing intussusception, severe pneumonia, and severe hypokalemia.

BACKGROUND: Clostridioides difficile infection is associated with antibiotic use and manifests as diarrhea; however, emerging cases of fulminant diarrhea caused by binary toxin-producing C. difficile unrelated to prior antibiotic exposure have been reported. Although fulminant colitis caused by C. difficile has been documented, instances of intussusception remain scarce. Here, we present a case of adult intussusception with severe hypokalemia and pneumonia resulting from a community-acquired C. difficile infection in Japan. CASE PRESENTATION: An 82-year-old male presented with dizziness, prog…

Clostridioides difficile dynamic electronic order panel, an effective automated intervention to reduce inappropriate inpatient ordering.

BACKGROUND: Ordering Clostridioides difficile diagnostics without appropriate clinical indications can result in inappropriate antibiotic prescribing and misdiagnosis of hospital onset C. difficile infection. Manual processes such as provider review of order appropriateness may detract from other infection control or antibiotic stewardship activities. METHODS: We developed an evidence-based clinical algorithm that defined appropriateness criteria for testing for C. difficile infection. We then implemented an electronic medical record-based order-entry tool that utilized discrete branches with…
