Cluster Headache and Migraine Shared and Unique Insights: Neurophysiological Implications, Neuroimaging, and Biomarkers: A Comprehensive Review.

Migraine headache (MH) and cluster headache (CH) are debilitating primary headache disorders that impose a significant global burden. While they share certain clinical features, such as unilateral pain and autonomic dysfunction, their underlying pathophysiological mechanisms remain distinct. Advances in the understanding of neurophysiological features, such as neuroimaging and biomarker research, have provided critical insights into both their overlapping and divergent characteristics. Neurophysiological research has revealed differences in nociceptive processing, cortical excitability, and se…

Exploration of Prolonged Remission and the Natural Course of Cluster Headache: An Interview-Based Cohort Study.

BACKGROUND AND OBJECTIVES: The aim of this study was to gain rare insight into prolonged cluster headache (CH) remission by (1) identifying patterns and factors associated with and (2) phenotypical changes before prolonged remission. The results can help patients better understand their disease course and uncover mechanisms behind spontaneous remission. METHODS: In this cross-sectional cohort study, all participants with a history of (probable) CH from the Leiden University Medical Center cohort were invited to complete a screening survey. Participants in prolonged remission were invited for …

Cluster headache polygenetic risk and known functional variants of CYP3A4 are not associated with treatment response.

BACKGROUND AND PURPOSE: The response to cluster headache treatments has a high interindividual variation. To date, treatment response has only been assessed by a candidate gene approach and no investigations into metabolic pathways have been performed. Our aim was to investigate the association between the polygenetic risk of cluster headache and treatment response to first-line cluster headache treatments as well as known functional variants of CYP3A4 and the response to verapamil. Further, it was aimed to replicate previous single nucleotide polymorphisms found to be associated with treatmen…

Can Craniometry Play a Role in Cluster Headache Diagnosis? A Pilot Exploratory TC-3D Based Study.

BACKGROUND: Since the pioneering reports of the so-called leonine face in cluster headache (CH) patients, cranial and facial features of these patients have been poorly investigated with conflicting results. We aimed to investigate whether abnormalities in craniometric measurements could characterize male CH patients and represent reliable and reproducible diagnostic biomarkers able to identify CH patients. METHODS: Brain CT images were recorded between 2018 and 2020 in 24 male patients with CH and in 24 matched healthy controls (HC). Then, craniometric measurements were obtained, and logisti…

[Progression of cluster headache to Raeder's syndrome with marked response to corticosteroid therapy: a case report].

A 42-year-old man complained of severe left orbital pain for 7 months. The diagnosis of cluster headache was made on the basis of diagnostic criteria formed by the International Headache Society. Sumatriptan was effective in relieving pain to a certain degree, but the frequency of the occurrence of pain gradually increased. Subsequently, he presented sensory disturbances in the left trigeminal nerve, and was admitted to our hospital. On admission, his neurological examination revealed left miosis and paresthesia in the first branch of the left trigeminal nerve. Neither anhidrosis nor ptosis wa…

The Comprehensive Effect of Depression, Anxiety, and Headache on Pain Intensity and Painkiller Use in Patients with Headache Analyzed by Unsupervised Clustering Using Machine Learning.

Background/Objectives: Patients with headache experience depression, anxiety, and reduced quality of life, which are individually associated with pain intensity and painkiller use, but their comprehensive combined effect remains unclear. Methods: Comprehensive patient groups were formed based on unsupervised clustering using machine learning algorithms, and their associations were analyzed via ordinary least square regression. K-means and t-distributed stochastic neighbor embedding (t-SNE) combined with hierarchical density-based spatial clustering of applications with noise (HDBSCAN) were app

Some questions provoked by a chronic headache (with mixed migraine and cluster headache features) in a woman with a pineal cyst. Answers from a literature review.

The main known function of the pineal gland in humans is the production of melatonin. Benign cysts of the gland have been related to headache, although the mechanism of production of this assumed clinical manifestation has not been clearly determined, due to the lack of large prospective studies. The question is complicated by the fact that pineal cysts are frequently found on brain magnetic resonance imaging. Much has been published about the possible role of benign pineal cysts in the pathophisiology of headaches and the potential of melatonin in headache therapy, as well as in other disorde

Elevated cytokine levels in the central nervous system of cluster headache patients in bout and in remission.

BACKGROUND: Cluster headache is characterized by activation of the trigeminovascular pathway with subsequent pain signalling in the meningeal vessels, and inflammation has been suggested to play a role in the pathophysiology. To further investigate inflammation in cluster headache, inflammatory markers were analysed in patients with cluster headache and controls. METHODS: We performed a case-control study, collecting cerebrospinal fluid and serum samples from healthy controls, cluster headache patients in remission, active bout, and during an attack to cover the dynamic range of the cluster h…

The influence of the pineal gland on migraine and cluster headaches and effects of treatment with picoTesla magnetic fields.

For over half a century the generally accepted views on the pathogenesis of migraine were based on the theories of Harold Wolff implicating changes in cerebral vascular tone in the development of migraine. Recent studies, which are based on Leao's concept of spreading depression, favor primary neuronal injury with secondary involvement of the cerebral circulation. In contrast to migraine, the pathogenesis of cluster headache (CH) remains entirely elusive. Both migraine and CH are cyclical disorders which are characterised by spontaneous exacerbations and remissions, seasonal variability of sym…

Efficacy and safety of 121 injections of the greater occipital nerve in episodic and chronic cluster headache.

INTRODUCTION: Infiltration of the greater occipital nerve (GON) with local anaesthetics and corticosteroids is a treatment option for cluster headache. METHODS: We retrospectively analysed the efficacy and safety of 121 GON injections in 60 patients with episodic or chronic cluster headache over a period of 4 years. RESULTS: Almost 80% of the infiltrations were at least partially effective (reduction of attack frequency, duration or severity) and 45% resulted in a complete response (no further attacks). The effect was maintained for 3.5 weeks on average in chronic cluster headache. In episod…

Sumatriptan. A reappraisal of its pharmacology and therapeutic efficacy in the acute treatment of migraine and cluster headache.

Sumatriptan is a potent and selective agonist at a vascular serotonin1 (5-hydroxytryptamine1; 5-HT1) receptor subtype (similar to 5-HT1D) and is used in acute treatment of migraine and cluster headache. Following administration of sumatriptan 100mg orally, relief of migraine headache (at 2 hours) was achieved in 50 to 67% of patients compared with 10 to 31% with placebo in controlled clinical trials. In a comparative study, oral administration of sumatriptan 100mg consistently achieved significantly greater response rates than a fixed combination of ergotamine 2mg plus caffeine 200mg during 3 …

Real-world utilization of acute treatments for cluster headache: Insights from the French National Open Health Data.

BACKGROUND: The utilization of open health databases offers a unique opportunity to analyze the dispensing of healthcare products within real-world populations. A recent study by Van Obberghen et al. (2025) was the first to leverage the French national database to examine consumption volumes for cluster headache (CH) treatments, with a specific focus on the sex ratio. Nonetheless, further investigation into additional variables - such as seasonality and geographical distribution - is warranted, given the well-established role of chronobiology in the pathophysiology of CH. OBJECTIVES: The prim…
