Enterocolic lymphocytic phlebitis: statistical analysis of histology features in viable and ischemic bowel.

PURPOSE: Health coaching is effective for chronic disease self-management in the primary care safety-net setting, but little is known about the persistence of its benefits. We conducted an observational study evaluating the maintenance of improved cardiovascular risk factors following a health coaching intervention. METHODS: We performed a naturalistic follow-up to the Health Coaching in Primary Care Study, a 12-month randomized controlled trial (RCT) comparing health coaching to usual care for patients with uncontrolled diabetes, hypertension, or hyperlipidemia. Participants were followed up

Smooth muscle degeneration of the mesenteric and branching veins causing ischemic enteritis: a case report.

BACKGROUND: Ischemic bowel injuries are generally caused by arteriosclerosis, thromboembolism, or vasculitis. Ischemic enteritis is less common than ischemic colitis because of the rich collateral arteries of the small intestine. In the present case, smooth muscle degeneration of the mesenteric to the submucosal veins caused ischemic enteritis and small bowel obstruction. CASE PRESENTATION: An 85-year-old woman with recurrent enteritis eventually developed small bowel obstruction. We performed laparoscopic partial resection of the small intestine. The pathological findings revealed smooth mus

Acute ischaemic colitis; a case report.

UNLABELLED: Acute ischaemic colitis (AIC) is being increasingly recognised as an uncommon cause of abdominal pain associated with fresh bleeding per rectum. It is paramount to maintain a high index of suspicion and adopt appropriate management strategies to avoid complications and inappropriate interventions. In this paper, we describe a case of AIC and review literature pertinent to the management of this condition. KEYWORDS: Ischaemic colitis, acute abdomen, management.

[Report of 2 unusual cases of abdominal pain].

We present two cases of rare abdominal pathologies, for which the initial diagnosis appeared simple but turned out to be exceptional pathologies. The first was compatible with simple lithiasic cholecystitis with choledocian obstacle, but was diagnosed as an idiopathic multifocal retroperitoneal fibrosis, also known as Ormond's disease. The second patient presented as an acute appendicitis. After a surgical exploration of the lower right quadrant, which did not allow a diagnosis, a median laparotomy was performed, leading to the diagnosis of ileo-caecal ischemic necrosis caused by lymphocytic p

Antibiotic-associated diarrhea and Clostridium difficile colitis: an update.

C. difficile colitis ranges from mild diarrhea to life-threatening "toxic" illness with fever, severe diarrhea, and abdominal pain. A colitis, frequently with a pseudomembrone, is the characteristic finding on sigmoidoscopy and is caused by one of more toxins elaborated by the organism Clostridium difficile. The clinical syndrome is not specific and can be mimicked by other colonic infections, inflammatory bowel disease, radiation colitis, or ischemic colitis. The diagnosis should be suspected in any patient who develops diarrhea during antibiotic therapy or within 6-8 weeks of treatment. Diag

[Behcet's disease and toxic megacolon].

The gastrointestinal symptoms of Behçet's disease are ancillary manifestations of this disorder reflected principally in the form of diarrhea, abdominal pain, meteorism, nausea, and loss of appetite. If radiological changes can be detected they generally appear as dilatation of the small intestine or ulceration at different levels of the digestive tract. In our patient the intestinal symptoms started with dilation of the ileum and then toxic megacolon developed. At later follow-up examinations the radiological picture resembled Crohn's disease and ischemic colitis of the entire organ. It has b

Biopsy diagnosis of colitis: possibilities and pitfalls.

The histopathologic diagnosis of inflammation is common in colorectal biopsies but is of limited value, if not further specified. We reviewed 280 endoscopic colorectal biopsy specimens for nonneoplastic disease from 100 consecutive patients in order to assess (a) the frequency of inflammation in excess of the physiologic infiltrate, (b) the frequency with which the cause of the inflammation could be specified, and (c) the interobserver variability in diagnosing inflammation. Based on the reviewers' impression, each case was classified into one of three categories: (I) normal or nonspecific cha

Right-sided ulcerative colitis.

This report describes a case of right-sided ulcerative colitis in which multiple shallow ulcers and erosion with symmetric luminal stenosis were distributed segmentally from the ascending colon to the cecum, with a skip lesion composed of superficial erosions in the right half of the transverse colon. Both the rectum and the left colon were spared at the time of onset. Biopsies taken from the lesions showed non-specific inflammation, while those from the rectum and sigmoid colon showed no abnormal findings. A 5-year follow-up study was made based on radiography and endoscopy. Other inflammator

Non-invasive diagnosis of acute ischemic bowel disease detected as hepatic portal venous gas during intra-aortic balloon percutaneous counterpulsation.

The authors report a very unusual case of non-invasive diagnosis of acute ischemic bowel disease detected as hepatic portal venous gas during intra-aortic balloon percutaneous counterpulsation. A 64-year-old man with acute ST-elevation myocardial infarction complicated by cardiogenic shock was treated with percutaneous angioplasty and intra-aortic balloon percutaneous counterpulsation. The post-procedural period was complicated by severe abdominal pain. Abdominal computed tomography revealed hepatic portal venous gas. Multiple kidney and splenic ischemic areas were also identified. Colonoscopy

CT evaluation of the colon: inflammatory disease.

Computed tomography (CT) is valuable for detection and characterization of many inflammatory conditions of the colon. At CT, a dilated, thickened appendix is suggestive of appendicitis. A 1-4-cm, oval, fatty pericolic lesion with surrounding mesenteric inflammation is diagnostic of epiploic appendagitis. The key to distinguishing diverticulitis from other inflammatory conditions of the colon is the presence of diverticula in the involved segment. In typhlitis, CT demonstrates cecal distention and circumferential thickening of the cecal wall, which may have low attenuation secondary to edema. I

Histopathologic study of the rectum in 1,438 consecutive rectal specimens in a single Japanese hospital: I. benign lesions.

The author investigated histopathology of 1,438 consecutive rectal specimens in the last 10 years of our pathology laboratory in Japan. A computer review of pathologic reports was done. Observations of pathologic slides were performed, when appropriate. The rectal specimens were composed of 1,022 benign lesions and 416 malignant lesions. The 1,022 benign lesions were composed non-specific proctitis (n=460, 45%), adenoma (n=248, 24%), ulcerative colitis (n=98, 10%), hyperplastic polyp (n=54, 5%), carcinoma in adenoma (n=40, 4%), rectal ulcer (n=37, 4%), serrated adenoma (n=24, 2%), hyperplastic

Chronic ischemic colonic lesion caused by phlebosclerosis with calcification.

We report here two patients in both of whom ileus finally developed and chronic ischemic colonic lesion caused by phlebosclerosis with calcification was diagnosed. We believe that phlebosclerosis is an old-fashioned diagnostic term, but appropriate here because of the remarkable calcification of the sclerotic walls of the colic veins that seems to be one of the most important hallmarks of this previously unrecognized, "new" condition. Phlebosclerosis could be a novel type of ischemic colitis because of its unique clinical presentations, although the pathogenesis of this unusual venopathy remai

Histopathological profile of benign colorectal diseases in Al- Madinah region of Saudi Arabia.

BACKGROUND: Patients with long-standing inflammatory bowel disease (IBD) have an increased risk of developing colorectal cancer (CRC). Colon cancer risk in IBD increases with longer duration and greater anatomic extent of colitis, the presence of primary sclerosing cholangitis, family history of CRC and degree of inflammation of the bowel. This study aimed to characterize the histopathological pattern of benign colorectal diseases among Saudi patients and to highlight age and gender variations of lesions as base line data for future studies to investigate the link between benign / IBD and colo

Fecaloma in Chronic Opioid-induced Constipation: A Fatal Case of Stercoral Colitis.

Stercoral colitis (SC) is a rare and potentially lethal condition characterized by severe colonic inflammation resulting from fecal impaction. This disorder is more prevalent among the elderly and vulnerable groups, especially those on chronic opioids. Our case study involves a 56-year-old male with a history of chronic opioid use who faced an acute colonic obstruction due to longstanding constipation. Despite receiving surgical treatment, the patient dies from ischemic bowel complications and multiorgan failure. The case accentuates the complexities of diagnosing and treating stercoral coliti

Impact of Previous Open Abdominal Surgery on Open Abdominal Aortic Repair: A Study from the NSQIP Database.

BACKGROUND: While endovascular aneurysm repair has become a first-line strategy in many centers, open surgical repair (OSR) of abdominal aortic aneurysms (AAAs) is still the best option for certain patients. A significant number of patients who are offered OSR for AAA have been previously submitted to other open abdominal surgeries (PAS). It is unclear, however, how this may impact their outcomes. The purpose of this study was to determine if there is an association between PAS and outcomes of OSR of AAA. METHODS: This is a retrospective cohort study based on clinical data from the American C

Stercoral Colitis: A Diagnostic Challenge and Therapeutic Approach in an Elderly Patient With Chronic Constipation.

Stercoral colitis, although rare, remains a significant cause of acute colonic inflammation. It is characterized by the presence of fecaloma leading to fecal impaction and subsequent mucosal injury, ultimately resulting in colonic wall inflammation. This condition primarily affects elderly patients with chronic constipation and is associated with significant morbidity and mortality if not recognized and managed promptly. Given its rarity and varied presentation, stercoral colitis often poses a diagnostic challenge. The clinical manifestations can mimic other colonic pathologies, such as divert

Metastatic Colonic Adenocarcinoma Mimicking Acute Bowel Perforation: Hiccups and Vomiting as Unusual Initial Symptoms: A Case Report.

INTRODUCTION: Colorectal cancer (CRC), the third most prevalent malignancy globally, can present with complications such as bleeding, obstruction, and, less commonly, perforation. These complications are associated with significant morbidity and mortality, demanding timely recognition and intervention. Unusual initial symptoms can obscure the clinical picture, delaying diagnosis, and treatment. CASE PRESENTATION: We report a 50-year-old male with a history of rectosigmoid adenocarcinoma treated with surgery and chemoradiotherapy, presenting with atypical symptoms of intractable hiccups, water

Enterocolic lymphocytic phlebitis with lymphocytic colitis, lymphocytic appendicitis, and lymphocytic enteritis.

We describe a 53-year-old man with a history of diarrhea temporally related to the use of flutamide. He developed an acute abdomen, and presented with an ileocecal intussusception due to an edematous ischemic cecum. The ischemia was due to enterocolic lymphocytic phlebitis (ELP), with numerous associated thrombi. The phlebitis involved not only the ischemic area but also the grossly unaffected areas, including the entire right colon, terminal ileum, and appendix. All layers of the bowel wall were involved. Mesenteric veins were also prominently affected, but the arteries were spared. This rare

Two cases of acute abdomen after an intravitreal injection of bevacizumab.

We report on a patient with ischemic colitis and another with paralytic ileus, both of whom experienced an acute abdomen after intravitreal injection of bevacizumab (IVB). Case 1 was a 78-year-old woman. Her medical history included surgery for colon carcinoma 10 years earlier. The patient developed acute severe abdominal pain and nausea the day after IVB for retinal vein occlusion with macular edema, and massive lower gastrointestinal bleeding occurred. Ischemic colitis was diagnosed. Case 2 was a 64-year-old man who presented with neovascular glaucoma with proliferative diabetic retinopathy.
