Unique Challenges of Shared Decision-Making in Colorectal Surgery for the Elderly Patient.

Older adults require disproportionately more operations and suffer more complications than their younger counterparts, making a thoughtful approach to shared decision-making imperative. This article reviews key considerations in the decision to pursue operative intervention for older adults. First, we explore the "Fix It" model and the importance of acknowledging trade-offs, with a specific focus on quality of life. This is followed by a discussion of how surgical recovery affects physical function, bowel function, and cognition. We describe strategies for shared decision-making in the older p

Preoperative anemia, transfusions and survival in colorectal cancer surgery-A retrospective study.

Perioperative anemia and blood transfusions are frequent and both are associated with a worse outcome in patients undergoing colorectal cancer surgery. To assess if treatment of anemia by transfusions can improve outcome, we conducted the present retrospective analysis.Of all patients who underwent elective colorectal cancer surgery at the University Hospital of Innsbruck between 2010 and 2019, 654 patients could be included in the study. The impact of preoperative anemia and transfusion on 1‑year survival was assessed using Kaplan-Meier analysis, univariate and multivariate Cox regression and

Preoperative Prediction of Sarcopenia in Patients Scheduled for Gastric and Colorectal Cancer Surgery.

INTRODUCTION: Sarcopenia negatively impacts surgical outcomes in gastrointestinal cancer patients, yet practical preoperative screening tools are lacking. The CRP/ALB ratio, a novel biomarker of systemic inflammation and nutritional status, may enhance sarcopenia prediction but remains underexplored in surgical oncology. This study aims to identify the predictors for preoperative sarcopenia prediction in gastric and colorectal cancer patients. METHODS: This retrospective study analyzed 145 patients undergoing curative surgery (2019-2021). Sarcopenia was defined by sex-specific CT-measured L3

Laparoscopic surgery for colorectal cancer in a patient with intestinal malrotation: a case report.

Intestinal malrotation (IM), a rare congenital anomaly, poses significant surgical challenges when coexisting with colorectal cancer-an association reported in fewer than 60 cases globally, predominantly in Japan. This study presents the first documented case in China of ascending colon cancer with nonrotation IM, successfully managed via laparoscopic right hemicolectomy with D2 lymphadenectomy. A 79-year-old female presented with abdominal pain, weight loss, and melena. Colonoscopy revealed an ascending colon mass, confirmed as adenocarcinoma. Preoperative contrast-enhanced CT demonstrated le

[Perioperative and Medium-Term Outcome of Bridge to Surgery for Obstructive Colorectal Cancer].

Bridge to surgery(BTS), in which a self-expandable metallic stent(SEMS)is implanted for obstructive colorectal cancer and surgery is performed after decompression, is now widely practiced in Japan. We reviewed the outcomes of 31 patients with Stage Ⅱ or Ⅲ colorectal cancer who underwent radical resection after SEMS implantation at our hospital between January 2015 and December 2020. The mean age of the patients was 70.3 years; 17 were men and 14 were women. The median interval time from SEMS implantation to surgery was 23 days, and all patients underwent laparoscopic surgery. Four patients exp

Repeat cytoreductive surgery with HIPEC for colorectal peritoneal metastases: a systematic review.

BACKGROUND: Colorectal peritoneal metastases (CRPM) are present in 10-20% of patients at the time of their initial cancer diagnosis, and affects over 20% of those who develop colorectal cancer recurrence. Cytoreductive surgery (CRS) with HIPEC is firmly established as the optimal surgical treatment, but there is very little known about the benefit of repeat or iterative CRS. The aim of this review is to provide a systematic evaluation of the perioperative complications, survival outcomes and quality of life in patients undergoing repeat CRS with HIPEC for CRPM. METHODS: A systematic review of

Assessment of cognitive function after surgery for colorectal cancer-a scoping review.

OBJECTIVE: Colorectal cancer is primarily treated with surgery. Major surgery and older age are risk factors associated with postoperative decline in cognitive function. In clinical research, a wide range of instruments have been used to assess cognitive function. There are no clear criteria for the measurement of postoperative cognitive dysfunction. This scoping review aimed to map how and when cognitive function has been assessed after surgery for colorectal cancer and the reported incidence of postoperative cognitive decline. DESIGN: Systematic scoping review following the JBI approach. D

Weekday Surgery Associated With Short-Term Outcomes in Patients With Colorectal Cancers.

INTRODUCTION: The "weekday effect" on elective surgery remains controversial. We aimed to examine the association between the day of surgery and short-term outcomes after elective surgery for stage I-III colorectal cancer (CRC). METHODS: We performed a multicenter retrospective analysis of 2574 patients who underwent primary colorectal resection for CRC between January 2017 and December 2019 at 15 institutions belonging to the Hiroshima Surgical Study Group of Clinical Oncology. Patients were divided into two groups according to the day of surgery: Friday and non-Friday (Monday to Thursday).

Fertility Concerns Related to Surgery for Colorectal Cancer: An Under-Discussed Topic.

As the incidence of colorectal cancer (CRC) increases among younger adults, the need for discussions regarding treatment-related infertility is growing. The negative impacts of gonadotoxic chemotherapy and pelvic radiation are well documented, but the role that surgical intervention for CRC plays in infertility is less clear. Additionally, treatment-related infertility counseling occurs infrequently. This review provides an overview of the connection between abdominal and pelvic surgery on male and female infertility and elucidates the role of surgeons in counseling to alleviate psychological

Implementation of music in the perioperative standard care of colorectal surgery (IMPROVE study).

AIM: Patients undergoing surgery experience perioperative anxiety and pain. Music has been shown to reduce perioperative anxiety, pain and medication requirement. This study assessed the feasibility and effectiveness of implementing a perioperative music intervention. METHOD: A prospective pre- and post-implementation pilot study was conducted to assess adherence to the intervention and the initial effect of music on postoperative pain scores (Numerical Rating Scale, 0-10) compared to a control group. Secondary outcomes encompassed pain scores throughout hospital admission, anxiety levels, me

Prophylactic ureteral stent in colorectal surgery: a meta-analysis and systematic review.

PURPOSE: Accidental ureteric injury during colorectal surgery is a rare but dreadful event. It is associated with a higher risk of urinary tract infection (UTI) and acute kidney injury (AKI). Prophylactic placement of double J stents could improve ureteral identification and decrease the chance of accidental ureteral injury. METHODS: We searched MEDLINE, Cochrane, Central Register of Clinical Trials, and Web of Science for studies published until March 2024. Odds ratios (ORs) with 95% confidence intervals (CIs) were pooled with a random-effects model. Statistical significance was defined as p

Mesenteric Artery Stenosis is a Risk Factor for Anastomotic Leakage in Colorectal Surgery.

OBJECTIVE: Anastomotic leakage (AL) after colorectal surgery leads to substantial morbidity and mortality rates. Theoretically, compromised blood flow caused by mesenteric artery (MA) stenosis may create suboptimal healing conditions at the anastomotic site, increasing susceptibility to AL. The association between MA stenosis on pre-operative computed tomography (CT) scan and AL in patients undergoing colorectal surgery was investigated. METHODS: A multicentre, retrospective, frequency matched, nested 1:5 case control study was conducted in the Netherlands. For each patient with AL, five cont

Predictive factors of non-completion of cytoreductive surgery in colorectal peritoneal metastasis.

INTRODUCTION: Cytoreductive surgery (CRS) ± hyperthermic intraperitoneal chemotherapy (HIPEC) is the only potentially curative treatment that can improve the survival prognosis for patients with peritoneal metastasis (PM) of colorectal origin. The main independent prognostic factors are extent of disease, as measured by the Peritoneal Cancer Index (PCI), and completion of CRS (CC-0 or R1). Despite thorough preoperative work-up for selection of surgical candidates, 20%-25 % of CRS procedures are stopped after exploration during laparotomy. These patients undergo "open-and-close" procedures asso

Current Status and Future of Robotic Surgery for Colorectal Cancer-An English Version.

Robotic surgical systems were created in part to solve several constraints of laparoscopic surgery and offer technical advantages. With a substantial body of evidence that demonstrates its efficacy in the treatment of rectal cancer, robotic surgery will soon become another conventional treatment. However, further investigations and randomized trials focusing on primary endpoints are needed to establish some advantages for robot-assisted colon surgery. Da Vinci Single-SiteⓇ and SPⓇ platforms were developed to overcome the shortcomings of single-port laparoscopic surgery. Despite the currently i

Endoluminal vacuum-assisted therapy as a treatment for anastomotic leakage in colorectal surgery.

Anastomotic leakage (AL) has a wide range of clinical features ranging from radiological only findings to peritonitis and sepsis with multiorgan failure. An early diagnosis of AL is essential in order to establish the most appropriate treatment for this complication. Despite AL continues to be a dreadful compli-cation after colorectal surgery, there has been no consensus on its management. However, based on patient's presentation and timing of the AL, there has been a gradual shift to a more conservative management, keeping surgery as the last option Reoperation for sepsis control is rarely ne

Predictors That Identify Complications Such As Anastomotic Leak in Colorectal Surgery: A Systematic Review.

Leakage of the anastomotic site is considered to be one of the most serious complications after colon and rectal surgery. It is associated with increased mortality, morbidity, and longer hospital stays. This systematic review examines the need for blood markers such as C-reactive protein (CRP), procalcitonin (PCT), albumin, and various other molecular markers that assist in their propensity to diagnose anastomotic leakage (AL) early after surgery. Utilizing PubMed and Google Scholar as resources and including the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guide
