3D Compressed Convolutional Neural Network Differentiates Neuromyelitis Optical Spectrum Disorders From Multiple Sclerosis Using Automated White Matter Hyperintensities Segmentations.

BACKGROUND: Magnetic resonance imaging (MRI) has a wide range of applications in medical imaging. Recently, studies based on deep learning algorithms have demonstrated powerful processing capabilities for medical imaging data. Previous studies have mostly focused on common diseases that usually have large scales of datasets and centralized the lesions in the brain. In this paper, we used deep learning models to process MRI images to differentiate the rare neuromyelitis optical spectrum disorder (NMOSD) from multiple sclerosis (MS) automatically, which are characterized by scattered and overlap

Managing venous stasis disease and ulcers.

Venous leg ulcers are arguably the most common type of venous ulcers seen in clinical practice. Compression therapy is the essential intervention in venous leg ulcer treatment, but coexisting arterial vascular insufficiency must be excluded before compression is initiated. No single topical dressing has been shown to be superior for all wounds. Venous leg ulcers are chronic and often difficult to heal, with only 40% to 70% healing after 6 months of treatment. Surgical procedures to reduce venous hypertension do not accelerate healing of a chronic ulcer, but trials suggest a decreased rate of f

Internal loads in the human tibia during gait.

BACKGROUND: Internal loads in long bones are of special interest, for testing and optimization of current implant designs for fracture treatment. Inverse dynamic musculoskeletal models were successfully used to determine the muscle forces in the lower extremities, however the internal loads expressed as forces and moments along the human tibia during gait have not been reported. METHODS: A musculoskeletal model of the lower extremities, developed and provided under public domain from the AnyBody research group (www.anybody.aau.dk), was modified to determine the three-dimensional internal load

A breakthrough in lumbar surgery: ipsilateral interlaminar endoscopy and its impact on spinal stability and patient outcomes.

Lumbar stenosis (LS) often leads to debilitating symptoms due to nerve compression in the spinal canal. As this condition becomes increasingly prevalent among the elderly, innovative surgical techniques are crucial. This letter examines a recent advancement introduced by Marco Aurélio Moscatelli et al., who have developed an ipsilateral interlaminar endoscopic approach for decompression at the L5-S1 level. Their study, involving 30 patients with degenerative stenosis, reports significant improvements in quality-of-life metrics, including the Oswestry Disability Index (ODI) and visual analog sc

SFE-AFCE-SFMN 2022 consensus on the management of thyroid nodules: Synthesis and algorithms.

The SFE-AFCE-SFMN 2022 consensus deals with the management of thyroid nodules, a condition that is a frequent reason for consultation in endocrinology. In more than 90% of cases, patients are euthyroid with benign and non-progressive nodules that do not warrant specific treatment. The clinician's objective is to detect malignant thyroid nodules at risk of recurrence and death, toxic nodules responsible for hyperthyroidism or compressive nodules warranting treatment. The diagnosis and treatment of thyroid nodules requires close collaboration between endocrinologists, nuclear medicine physicians

Non-operative management of pneumoperitoneum following cardiopulmonary resuscitation.

Spontaneous pneumoperitoneum in a patient with a tracheostomy tube following cardiopulmonary resuscitation is exceedingly rare, with little experimental nor observational data to guide evidence-based management. We present the case of a 75-year-old woman with a tracheostomy tube who developed pneumoperitoneum following CPR. The patient experienced pulseless electrical activity arrest requiring nine rounds of chest compressions to return to spontaneous circulation. Computerized tomography demonstrated pneumothoraces, subcutaneous emphysema and extensive intraperitoneal, extraperitoneal and retr

Consensus and Equipose in the Management of Military Trainee Femoral Neck Stress Fractures: A Survey of Military Surgeons.

BACKGROUND: Femoral neck stress fractures (FNSFs) are a unique injury pattern not commonly treated in the civilian trauma population; however, it is particularly high with military trainees engaged in basic combat training. To date, no study has surveyed a population of military orthopedic surgeons on treatment preferences for military service members (SMs) with FNSF. QUESTIONS: We aim to evaluate the extent of clinical equipoise that exists in the management of these injuries, hypothesizing that there would be consensus in the factors dictating surgical and non-surgical intervention for FNSF

Comparing interventions used to treat venous leg ulcers - A network meta-analysis.

IntroductionVenous leg ulcers (VLUs) account for up to 80% of chronic lower limb ulcers. The management of these ulcers is complex and can involve multiple interventions, ranging from compression, to pharmacotherapy, surgical and endovenous procedures. Despite these, VLUs are associated with prolonged healing times and high recurrence rates. They therefore pose a significant burden to both patients and the healthcare system, costing the National Health Service over £3 billion annually. Previous studies have largely compared interventions with standard of care rather than against each other. Du

Lumbar radiculopathy secondary to primary spinal dural diffuse large B-cell lymphoma.

Lymphoma of a dural genesis is a distinct variant of primary central nervous system lymphoma and is rare. It putatively has a more benign clinical course. Cranial primary dural lymphoma is more often marginal zone B-cell lymphoma, whereas spinal primary dural lymphoma is most commonly diffuse large B-cell lymphoma. We report a male patient who presented with subacute progressive radiculopathy due to a compressive infiltrative lumbosacral spinal lesion. This was determined to be primary dural diffuse large B-cell lymphoma. The radiology, therapeutic considerations and differentiating biological

A Near Miss of a Retropharyngeal Abscess with MRSA in a 5-Week-Old Boy Due to an Unusual Presentation.

A retropharyngeal abscess (RPA) in early childhood is not uncommon due to at-risk lymph nodes in this deep neck space and is typified by fever, odynophagia, and a constellation of respiratory manifestations. However, RPA is exceedingly rare in the neonatal subpopulation and not part of the usual differential diagnosis algorithm in this age range. Herein, we present a unique case of a previously healthy 5-week-old male infant with protracted "congestion" and difficulty in oral feeding, whose clinical course is confounded by intermittent, positional bradycardia and subsequent apnea. He was event

Detection of Mild Traumatic Brain Injury via Topological Graph Embedding and 2D Convolutional Neural Networks.

Early diagnosis of mild traumatic brain injury (mTBI) is challenging, yet significantly important in order to grant the patients with timely treatment and mitigating the risks of possible long-term psychiatric and neurological disorders. To tackle this problem, in this paper, we develop an mTBI detection framework based on graph embedding features combined with convolutional neural networks (CNN). Cortical activity in transgenic calcium reporter mice expressing Thy1-GCaMP6s is recorded in two sessions, prior to and after inducing injury. Functional networks are then constructed for recordings

Management - spinal metastases.

Due to a worldwide increase of cancer incidence and a longer life expectancy of patients with metastatic cancer, a rise in the incidence of symptomatic vertebral metastases has been observed. Metastatic spinal disease is one of the most dreaded complications of cancer as it is not only associated with severe pain, but also with paralysis, sensory loss, sexual dysfunction, urinary and fecal incontinency when the neurologic elements are compressed. Rapid diagnosis and treatment have been shown to improve both the quality and length of remaining life. This chapter on vertebral metastases with epi

The "Code Drugs in Cardiac Arrest"--the use of cardioactive medications in cardiac arrest resuscitation.

In October of 2010, the American Heart Association (AHA) published the 2010 Guidelines on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care. These guidelines place significant emphasis on 5 major areas of therapy in patient with cardiac arrest, including immediate recognition and activation of the emergency response team, effective chest compressions, rapid defibrillation, effective advanced life support (ALS), and integrated postresuscitation care. "Effective ALS" includes the placement of an advanced airway, establishment of parenteral access, and the administration of cardioac

[Truncus coeliacus stenosis in duodenopancreatectomy].

INTRODUCTION: Patients with celiac axis stenosis are asymptomatic due to the rich collateral blood supply through superior mesenteric artery. Ligating and dividing gastroduodenal artery during pancreatoduodenectomy can cause ischemic threat especially to liver, less frequently stomach and spleen, or failure of anastomoses. CASE REPORT: The authors present a case of 27-year-old female who underwent duodenopancreatectomy for pseudopapillary tumour of the head of pancreas. Celiac axis stenosis was found peroperatively and proven during angiography. Although an attempt of endovascular dilatation

Diagnosis and treatment of deep-vein thrombosis.

Deep-vein thrombosis (DVT) is a common condition that can lead to complications such as postphlebitic syndrome, pulmonary embolism and death. The approach to the diagnosis of DVT has evolved over the years. Currently an algorithm strategy combining pretest probability, D-dimer testing and compression ultrasound imaging allows for safe and convenient investigation of suspected lower-extremity thrombosis. Patients with low pretest probability and a negative D-dimer test result can have proximal DVT excluded without the need for diagnostic imaging. The mainstay of treatment of DVT is anticoagulat

Cyanoacrylate adhesive closure for varicose veins: An Indian expert consensus using a modified Delphi method.

Cyanoacrylate adhesive closure (CAC) is a non-thermal, nontumescent option for treating saphenous reflux. This study aimed to develop a formal, evidence-based expert consensus on the use of CAC for the management of varicose veins in India. A modified Delphi consensus process was conducted. A panel of 12 experienced Indian interventional radiologists was convened. Consensus was defined as ≥80% agreement, achieved through anonymized voting and iterative discussions. All 12 panelists (100% participation) achieved a unanimous consensus (100% agreement) with the finalization of 29 consensus statem

Cardiac Magnetic Resonance Imaging with Myocardial Strain Assessment Correlates with Cardiopulmonary Exercise Testing in Patients with Pectus Excavatum.

Objectives: To evaluate correlations between cardiac magnetic resonance imaging (cMRI) at rest including strain imaging and variables derived from quantitative cardiopulmonary exercise testing using a treadmill in patients with pectus excavatum. Methods: We retrospectively correlated the results of cMRI and cardiopulmonary exercise testing in 17 patients with pectus excavatum, in whom both examinations were performed during their pre-operative clinical evaluation. In addition to cardiac volumetry, we assessed the strain rates of both ventricles using a feature-tracking algorithm of a piece of

"Idiopathic" Shoulder Pain and Dysfunction from Carpal Tunnel Syndrome and Cubital Tunnel Syndrome.

UNLABELLED: As a referral center for chronic pain, we see many patients with "idiopathic" shoulder pain and limited range of motion. The combination of mild or subclinical carpal tunnel syndrome and cubital tunnel syndrome may be an underrecognized etiology of symptoms in such patients. Here, we report our treatment algorithm and results for such patients. METHODS: Of patients with a chief complaint of shoulder pain, we identified 56 consecutive patients who had pain or tingling with median nerve compression at the proximal wrist crease and positive Tinel's around the cubital tunnel. They wer
