Species confusion in the Australian inflorescence-living genus Pseudanaphothrips (Thysanoptera, Thripidae).

An attempt is made to resolve the confused identity of species in the Australian endemic genus, Pseudanaphothrips, with an illustrated key p … ele is considered a new synonym of Pseudanaphothrips frankstoni (Steele). Apart from the historical confusion due to type specimens being poorly slide-mounted, and descriptions that were not diagnostic, biological problems limit the reliability of taxonomic conclusions. Adults appear to disperse before reaching mature colouration, often landing on non-host …

Parasomnia, Acute Confusion, and Transient Amnesia Temporally Associated With Ashwagandha Ingestion.

Ashwagandha (Withania somnifera) is a widely used adaptogenic supplement promoted for stress reduction and general wellness. Although generally well tolerated, rare but clinically significant adverse events have been documented. We report a case of acute neurotoxicity in a 40-year-old man temporally associated with ashwagandha ingestion. The patient presented with prolonged sleep, abnormal sleep behaviours, acute confusion, and retrograde amnesia. Comprehensive diagnostic evaluation excluded alternative causes. Discontinuation of ashwagandha, along with supportive management, resulted in compl…

Sodium, potassium intake, and all-cause mortality: confusion and new findings.

BACKGROUND: The World Health Organization (WHO) has established recommended daily intakes for sodium and potassium. However, there is currently some controversy regarding the association between sodium intake, potassium intake, the sodium-to-potassium ratio, and overall mortality. To assess the correlations between sodium intake, potassium intake, the sodium-to-potassium ratio, and overall mortality, as well as the potential differences in sodium and potassium intake thresholds among different population groups, we analyzed data from NHANES 2003-2018. METHODS: NHANES is an observational cohor

Avoidance, confusion or solitude? Modelling how noise pollution affects whale migration.

Many baleen whales are renowned for their acoustic communication. Under pristine conditions, this communication can plausibly occur across hundreds of kilometres. Frequent vocalisations may allow a dispersed migrating group to maintain contact, and therefore benefit from improved navigation via the "wisdom of the crowd". Human activities have considerably inflated ocean noise levels. Here we develop a data-driven mathematical model to investigate how ambient noise levels may inhibit whale migration. Mathematical models allow us to simultaneously simulate collective whale migration behaviour, a

An older man with progressive short-term memory loss and confusion.

A 72-year-old man with progressive memory loss and confusion presented to a neurology clinic for evaluation. He initially had difficulty remembering names and misplaced objects; however, his memory deficits had progressed, and more recently he had numerous car accidents and difficulty managing his own medications and finances. Cognitive testing revealed significant memory deficits reflecting moderate-stage dementia, and his brain MRI showed several cortical microbleeds and an area of siderosis consistent with the diagnosis of cerebral amyloid angiopathy (CAA). This case report provides an over…

Intrahepatic biliary cystadenoma: Confusion, experience, and lessons learned from our center.

BACKGROUND: Intrahepatic biliary cystadenoma (IBC) is a rare benign cystic tumor of the liver. So far, it has not been comprehensively understood, which causes incorrect diagnosis, treatment confusion, and even inappropriate treatment. Here, we reviewed clinical data of IBC patients in our center, shared our experiences and lessons learned, and improved the level of diagnosis and treatment. METHODS: The clinical data of 10 patients with pathologically diagnosed IBC, admitted to the Department of Hepatobiliary Surgery of the Affiliated Hospital of Guangdong Medical University from January, 200…

Source Memory in Korsakoff Syndrome: Disentangling the Mechanisms of Temporal Confusion.

BACKGROUND: Korsakoff syndrome (KS), most frequently resulting from alcohol dependence (ALC), is characterized by severe anterograde amnesia. It has been suggested that these deficits may extend to other memory components, and notably source memory deficits involved in the disorientation and temporal confusion frequently observed in KS. However, the extent of this source memory impairment in KS and its usefulness for the differential diagnosis between ALC and KS remain unexplored. METHODS: Nineteen patients with KS were compared with 19 alcohol-dependent individuals and 19 controls in a sourc…

Obstructive sleep apnoea syndrome increases source-confusion errors: A pilot study.

We explored external source monitoring (i.e., discrimination between memories of two externally derived sources) in patients with obstructive sleep apnoea syndrome (OSAS). Our specific aim was to ascertain whether, relative to controls, patients exhibit more source-confusion errors when there are similarities between two external memory sources. We recruited 22 patients with OSAS and 22 controls matched for sex, age, and education. The experimental procedure we used came in three phases. First, participants viewed a target film. Second, they were shown a mixed set of photographs, some taken fr…

Treatment of vitamin B12 deficiency-methylcobalamine? Cyancobalamine? Hydroxocobalamin?-clearing the confusion.

Vitamin B12 (cyancobalamin, Cbl) has two active co-enzyme forms, methylcobalamin (MeCbl) and adenosylcobalamin (AdCbl). There has been a paradigm shift in the treatment of vitamin B12 deficiency such that MeCbl is being extensively used and promoted. This is despite the fact that both MeCbl and AdCbl are essential and have distinct metabolic fates and functions. MeCbl is primarily involved along with folate in hematopiesis and development of the brain during childhood. Whereas deficiency of AdCbl disturbs the carbohydrate, fat and amino-acid metabolism, and hence interferes with the formation

Confusion, caring and tuberculosis diagnostic delay in Cape Town, South Africa.

OBJECTIVE: To explore the ways in which provider and patient behaviours interact to exacerbate diagnostic delay in Cape Town, South Africa. DESIGN: Eight focus group discussions were conducted in four urban communities, all with high tuberculosis (TB) prevalence, including two with high human immunodeficiency virus co-prevalence. Groups were stratified by sex, ethnicity and TB status. Findings were elicited inductively from the dataset using a combination of grounded theory and thematic analysis. RESULTS: Diagnostic delay was caused by delays in care seeking, provider failure to diagnose TB

Possible confusion between primary hypersomnia and adult attention-deficit/hyperactivity disorder.

We explored the possibility of diagnostic confusion between hypersomnias of central origin (narcolepsy and idiopathic hypersomnia, IH) and the adult form of attention-deficit/hyperactivity disorder (ADHD). We included 67 patients with narcolepsy, 7 with IH and 61 with ADHD. All patients completed the Epworth Sleepiness Scale and the ADHD Rating Scale. We found that 18.9% of the hypersomnia patients fulfilled the self-reported criteria for ADHD in adulthood, compared with 77% of the ADHD patients. A score > or =12 on the Epworth Sleepiness Scale (usually regarded to indicate excessive daytime s…

Periodic febrile confusion as a presentation of complex partial status epilepticus.

A 75-year-old woman was evaluated for recurrent episodes of fever she experienced periodically every 4-5 weeks over the last 12 months, lasting 2-3 days each. The fever was associated with continuous complex partial seizures, paralleled the seizure activity and returned to normal after the seizures had ceased. The ictal EEG recordings showed rhythmic bitemporal 3-4 Hz activity; the interictal recordings showed a spike and wave discharge over the right fronto-temporal region. Carbamazepine effectively controlled both the seizures and the fever; the latter was presumed to be an inherent manifest

[Right neglect with hemiasomatognosia, mental confusion, apraxia and agraphia without aphasia].

A 74 year-old woman was admitted with a right hemiplegia resulting from a left infarct that had totally destroyed the territory supplied by the middle and anterior cerebral arteries. The patient presented with anosognosia and hemiasomatognosia and negligence of the right half-field. She was aprosodic, not aphasic, and there was a severe apraxia and total agraphia. These neuropsychological disorders are discussed in relation to the contradictory data relative to the manual lateralization of the patient. Findings in this case of a "crossed apraxia" show that management of language and of the pro

Granulomatous pleuritis caused by Francisella tularensis: possible confusion with tuberculous pleuritis.

Granulomatous pleuritis, diagnosed by pleural biopsy, is rarely caused by disease processes other than tuberculosis. We present a case of granulomatous pleuritis caused by Francisella tularensis that occurred in a 55-yr-old male sheep shearer with an exudative, lymphocytic, pleural effusion. Confirmation of the diagnosis was made by elevated serum agglutination titers against F. tularensis and detection of the organism in a pleural biopsy specimen by the glucose oxidase immunoenzyme technique. We conclude that F. tularensis, not previously described as a cause of granulomatous pleuritis, must

Tinea cruris: diagnostic confusion due to isolation of Candida albicans alone.

The diagnostic importance of the isolation of Candida albicans from a skin lesion is often uncertain. In a 68-year-old man from whose lesions only C. albicans was originally isolated Trichophyton rubrum and Epidermophyton floccosum were also isolated when the growth of the yeast was inhibited in a selective medium. The use of this selective medium, casamino acids erythritol albumin agar, ensures the proper interpretation of the significance of the presence of C. albicans in skin lesions.
