BACKGROUND: Ectopic intracranial schwannomas (those that do not arise from a named cranial nerve) are rare. They account for <2% of surgically resected central nervous system schwannomas.
CASE DESCRIPTION: We report the case of a 14-year-old boy presenting with a left conductive hearing loss and temporal bone deformity. No facial or cranial nerve deficits were present. Cross-sectional imaging demonstrated a large expansile extra-axial temporal bone mass, extending into and distorting the middle cranial fossa. At surgical resection the tumor was functionally and anatomically distinct from the

Although schwannoma may arise from any true cranial nerve, it is well known that the acoustic and trigeminal nerves are frequent origins. Schwannomas of the facial nerve are rare. In this communication, two cases of facial nerve schwannoma arose from the horizontal portion are reported. Case 1. A 44 year-old male was admitted to our clinic complaining of left hearing loss and facial asymmetry. About three years prior to the admission, he first noticed left hearing disturbance which was gradually deteriorated and was transiently accompanied by left tinnitus during the progression. Then muscle s

PURPOSE: Craniosynostostic syndromes are due to multisuture synostoses and affect the entire craniofacial skeleton. This study analyzed the facial complex and airways to quantify the relationship between insufficient facial growth, airways obstruction, and the sutural pattern of the splanchnocranium and cranial fossae.
METHODS: Preoperative high-resolution CT images in 19 infants with syndromic craniosynostosis were quantitatively analyzed. Because all children showed involvement of minor sutures/synchondroses coursing in the posterior cranial fossa, they were divided into three groups accord

The orbitozygomatic approach provides wide, multidirectional access to the anterior and middle cranial fossae, as well as to the upper third of the posterior fossa and clivus. The authors describe technical details of the surgical approach as it has evolved over 3.5 years of experience in 83 consecutive cases. This modified technique eliminates the need for bone reconstruction of the orbital walls to prevent enophthalmos and minimizes the risk of injury to the frontal branch of the facial nerve. At a follow-up evaluation after a period averaging 14 months, all patients were pleased with the co

Tumors in the posterior fossa can be situated either dorsal and lateral, ventral and medial, or occupying both regions in relation to the cranial nerves, with the latter position being especially challenging. In an effort to organize neurovascular complexes contained within, anatomically based triangles have been proposed to serve as guiding landmarks for locating critical neurovascular structures. The objectives of this study were to: (1) provide a review of historical anatomically based vascular-centric triangles of the posterior fossa based on respective neurovascular complexes; (2) introdu

A 61-year-old housewife had complained of unilateral facial pain and had been treated as prolonged trigeminal neuralgia by a dentist. When she came to our clinic, the III-X cranial nerves of the right side were involved. A base view of the skull indicated destructive lesions of the right side middle cranial fossa, and a diagnosis of Garcin syndrome was made. Neurosurgical operation, X-ray radiation and chemotherapy improved to some extent the neurological condition temporarily. Postmortem examination showed an epipharyngeal rhabdomyosarcoma occupying the maxillary sinus, and extradural space o

Urothelial cell carcinoma (UCC) of the bladder infrequently metastasizes to the central nervous system (CNS). The incidence worldwide is approximately 1%. The cerebral hemispheres of the anterior and middle cranial fossa are the most common sites of CNS spread, and usually, multiple metastatic lesions are present. Infrequently, metastasis presents as a single solitary metastatic malignancy to the posterior fossa. Here, we present the case of a patient with bladder UCC who presented with a single solitary metastatic malignancy to the cerebellum. The authors discuss the signs, symptoms, mechanis

A 60-year-old, right-handed man suffered from mild word finding difficulties and kanji agraphia. Brain computed tomography revealed left temporal lobe hemorrhage, and cerebral angiogram revealed multiple dural arteriovenous fistulas (d-AVFs) in the left anterior cranial fossa and middle cranial fossa. Surgical shunt ablation was performed, and the lesions were obliterated completely. Analysis of the kanji agraphia during neuropsychological rehabilitation showed similar symptoms to those of the left posterior inferior temporal lobe lesion. Re-evaluation of magnetic resonance imaging showed mult

BACKGROUND: The microscopic transciliary SupraOrbital keyhole (mtSO) approach has been used for a wide variety of anterior and middle fossa pathologies, including aneurysms, meningiomas, craniopharyngiomas, and other skull-base tumors. Several clinical series have proven its efficacy and safety, but detailed anatomical demarcations of the anterior and middle cranial base exposure are lacking. Our aim was to define the surgical limitations of the mtSO approach to the ipsilateral and contralateral anterior and middle skull base.
METHODS: Five cadaveric specimens (10 sides) were studied with ima

BACKGROUND: Classically used for treatment of orbital lesions, the lateral orbitotomy with cantholysis can be combined with a temporal craniectomy for lesions involving the middle cranial fossa.
OBJECTIVE: To present a single-center experience with the lateral orbitotomy approach for lesions involving the middle fossa.
METUODS: Twenty-five patients underwent lateral orbitotomies from April 2012 to July 2015. Excluding patients with solely intraorbital pathologies, 13 patients’ clinical and radiographic records were retrospectively reviewed.
RESULTS: Signs/symptoms in the 13 patients (ages 2

Spontaneous cerebrospinal fluid (CSF) otorrhea is an uncommon but increasingly recognized condition, particularly in older adults. It most often results from temporal bone defects, such as tegmen tympani dehiscence, which establish abnormal communication between the subarachnoid space and the middle ear or mastoid air cells. Clinical manifestations of CSF otorrhea may range from subtle conductive hearing loss and middle ear effusion to life-threatening recurrent meningitis. High-resolution computed tomography (HRCT) is essential for diagnosis and surgical planning. We are presenting a 66-year-

Patients who have had middle-ear or mastoid surgery are at an increased risk of developing cerebrospinal fluid (CSF) otorrhoea. The CSF leak is usually from defects in the tegmen or posterior cranial fossa. We present a patient with CSF otorrhoea following a modified radical mastoidectomy seven years ago. There was an unusual communication between the internal auditory meatus (IAM) and the middle ear. Radiologic imaging like the MRI is useful in identifying the site of leak.
