Most ulcers of the lower limbs are caused by existing chronic venous insufficiency. Later on, true social and professional problems will arise, with serious economic and psychological consequences not only for the patient himself, but for the community as well, such as huge medical costs--hence the importance of prevention and treatment, which must in no case be purely symptomatic. The ligation of the arch and of the perforating veins and stripping of the affected vein are part of the classical management of varices. These procedures can may prove to be virtually impossible in case of chronic

We report a 16-year experience with full-thickness grafts to resurface defects created by releasing contractures of Dupuytren's disease. This technique was used in 68 patients with Dupuytren's contracture from 1970 to 1985. Follow-up of 36 hands of 24 patients averaged 3.9 years postoperatively. There was no recurrent disease in the palms and digits that were covered with the full-thickness grafts. The incidence of extension outside the grafts was 8%. The area of full-thickness grafting covered most of the width of the palm, an extension of Gonzalez's technique, which was presented in 1970.

Fifty-eight patients suffering from intractable pain in the trigeminal distribution due to neoplasms of the intracranial or facial regions underwent controlled percutaneous coagulation of the Gasserian ganglion (trigeminal thermorhizotomy). Of them, 35 were evaluated at varying time intervals from the intervention (mean: 7 months). While pain was completely relieved in 71 per cent of cases at discharge, the follow-up examination was consistent with 49 per cent of analgesias. The rationale of percutaneous thermocoagulation in malignant facial pain is given and the pertinent literature summarize

Successful reconstruction of the cheek following excision for cutaneous malignancy requires careful consideration of defect location, size, and depth in relation to the anatomic properties of the affected cheek unit. Various reconstructive options are available to the surgeon, ranging from simple excisions to complex cervicofacial advancements to meet the needs for functional and aesthetically pleasing reconstructive outcomes. The surgeon must prevent distortion of mobile structures, such as the eyelid, nose, and lips; respect aesthetic subunits; and avoid blunting natural creases. This discus

OBJECTIVE: To investigate a method of auricular reconstruction by soft tissue expansion techniques without skin grafting.
METHODS: Two tissue expanders (50 ml and 70 ml) were implanted under the skin of mastoid in 15 patients with grade II or III microtia. One big expander (100 ml) were implanted under the skin of mastoid in 13 patients with grade I microtia and 3 patients with grade II or III microtia. After skin expansion, the expanders were taken out. The autologous rib cartilage or Medpor scaffolds were implanted. The superior expanded skin flap was used to cover the frontal surface and t

A new triaradiate skin incision is presented as an approach to the wrist. Centred over Lister's tubercle, it consists of one longitudinal proximal and two oblique distal rays, making three 120 degrees skin flaps. These provide an excellent triangular operating field to the distal radius, the radiocarpal joint and the carpus itself, allowing for most procedures in this region. There have been no complications in 32 patients, and the cosmetic appearance has been excellent in most of them.

The surgical treatment of pressure sores in paraplegic patients currently consists of myocutaneous flaps, direct suture and cover with split thickness skin grafts. These surgical methods of closing defects in pressure bearing areas do not normally provide any sensory function unless the skin flaps have intact sensibility or have been re-innervated in a first stage operation. The aim of the present project is to study the feasibility of re-innervating the gluteal region directly by mobilizing intercostal nerves, extending their reach by nerve grafts and neurotization of the required area withou

The presence of traumatic soft-tissue defects in the extremity indicates serious damage that may compromise the systems of motion, circulation or sensibility and therefore jeopardize functional rehabilitation. This overview highlights the significance and the various causes of soft-tissue defects, of which several types may be distinguished. Principles for the selection of various flap procedures are outlined, in accordance with the need for elevation and early motion therapy. The requirements placed on flap tissue are described, such as surface characteristics, ability to restore sensibility,

Free tissue transfer is quite an important and attractive procedure for reconstruction of an injured limb, especially an upper limb. A simple skin flap can be transferred to close an open wound of the limb, but a sensory flap or a special flap such as a nail-skin, tendon-skin, or muscle-skin flap is a much better donor for reconstructing the function of the upper limb. The muscle or muscle-skin transfer procedure should be highly valued as a means of restoration of motor function in those patients in whom all other measures have totally failed in the past. Sensory flap transfer from the foot i

Hypertrophic and keloid scars present significant challenges in surgical recovery and perioperative care, especially in patients undergoing reconstructive procedures or burn treatment. These scars can cause pain, pruritus, and psychosocial distress. There is a higher burden in individuals with skin of color, a term commonly used to describe those with Fitzpatrick skin types IV to VI, due to both increased incidence and limitations of early detection. Traditional treatment approaches often overlook the complex nature of wound healing and the influence of perioperative inflammation and pain cont

Xeroderma pigmentosum is a rare hereditary autosomal recessive genodermatosis. At present, there are many treatment options for xeroderma pigmentosum, covering medical/procedural, surgical and combined modalities. However, the quality of these interventions has not been assessed. Our study aimed to perform a systematic review of the literature regarding the treatment of xeroderma pigmentosum. Multiple medical databases were accessed with the Medical Subject Headings terms; "xeroderma pigmentosum," "therapeutics" and "surgical procedures, operative" from January 2000 to April 2019, including ar

INTRODUCTION: Surgical fires are a rare event that still occur at a significant rate and can result in severe injury and death. Surgical fires are fueled by vapor from alcohol-based skin preparations in the presence of increased oxygen concentration and a spark from an energy device. Carbon dioxide (CO2) is used to extinguish electrical fires, and we sought to evaluate its effect on fire creation in the operating room. We hypothesize that CO2 delivered by the energy device will decrease the frequency of surgical fires fueled by alcohol-based skin preparations.
METHODS: An ex vivo model with 1

BACKGROUND AND OBJECTIVE: Elephantiasis nostras verrucosa (ENV) is a disfiguring skin condition that is difficult to treat. Existing treatment modalities serve to improve cosmesis or treat symptoms. Herein, we report a case of ENV with lymphocutaneous fistula successfully treated with ablative carbon dioxide laser.
STUDY DESIGN/PATIENTS AND METHODS: A 57-year-old woman with biopsy-proven ENV with lymphocutaneous fistula was treated with ablative carbon dioxide laser to the symptomatic area of her right thigh in 3 treatment sessions over 6 months.
RESULTS: The patient had resolution of lympho
