Diet and health outcomes in vulnerable populations.

Good nutritional health is essential to physical and cognitive function, prevention or management of chronic health conditions, and prevention of disability. Poor nutritional health describes dietary intakes of inadequate amounts of energy, protein, and individual or multiple micronutrients. Homebound older adults are considered the most vulnerable for poor nutritional health; many are poor and experience persistent food insufficiency. This chapter discusses the importance of dietary intakes, especially for homebound older adults, and presents resource-related factors that influence food choic…

Monitoring of pesticide residues in vegetarian diet.

Samples (28) of complete vegetarian diet consumed from morning till night i.e. tea, milk, breakfast, lunch, snacks, dinner, sweet dish etc. were collected from homes, hostels and hotels periodically from Hisar and analysed for detecting the residues of organochlorine, synthetic pyrethriod, organophosphate and carbamate insecticides. The estimation was carried out by using multi-residue analytical technique employing gas chromatograph (GC)-electron capture detector and GC-nitrogen phosphorous detector systems equipped with capillary columns. The whole diet sample was macerated in a mixer grinde…

Diet mixing time affects nursery pig performance.

The objective of this study was to determine the effects of mixing time (mixer efficiency) of diets containing several low-inclusion ingredients (crystalline AA, ZnO, a medication, and vitamin and trace mineral premixes) on growth performance of nursery pigs. In a pilot study, mixing efficiency of a 1,360-kg capacity, horizontal ribbon mixer was evaluated with salt of different particle sizes (440, 730, 2,000, and 3,000 microm). Sample preparation was evaluated by analyzing diet samples as collected (unground) or by grinding the entire sample to approximately 400 microm in particle size (groun…

Tuberous sclerosis complex and the ketogenic diet.

PURPOSE: Tuberous sclerosis complex (TSC) is a condition that is frequently associated with intractable, early-onset epilepsy, and often is first seen as infantile spasms. If medications fail and no clear epileptogenic tuber is identified, nonpharmacologic therapies are often attempted. The use of the ketogenic diet specifically for children with TSC and epilepsy has not been previously described. METHODS: A chart review was performed of patients with TSC treated with the ketogenic diet over a 5-year period at Johns Hopkins Hospital and Massachusetts General Hospital. RESULTS: Twelve childre…

[Kwashiorkor after an exclusion diet for eczema].

BACKGROUND: Nutritional disorders observed in children with eczema may be aggravated by inappropriate diet, sometimes with dramatic effects. CASE REPORT: A child aged 3 years 7 months presented a state of severe malnutrition with edema (kwashiorkor). The child had chronic eczema and had been given a highly restrictive diet based on a principle of exclusion without medical advise. Cure was achieved without sequelae with a normal diet. DISCUSSION: This case illustrates the fragility of nutritional status in children with chronic widespread skin disease due to the risk of cutaneous loss of prot…

Periodontal disease and diet in domestic pets.

Periodontal disease is the most common oral condition seen in domestic pets. In addition to the discomfort caused in the affected animal, there is strong circumstantial evidence to show that a focus of infection in the mouth may cause disease of distant organs. Consequently, prevention of periodontal disease is of paramount importance for the general health and well-being of pets. The presence of plaque on the tooth surfaces is the primary cause of periodontal disease. However, the mechanisms by which disease develops are by no means fully understood. Dietary texture does have an effect on the…

Treating phenylketonuria by a phenylalanine-free diet.

The phenylalanine-free diet is needed for the treatment of phenylketonuria. Phenylketonuria is an inherited metabolic condition in which there is a deficiency of the enzyme phenylalanine hydroxylase. Lack of this enzyme means the body cannot metabolise the essential amino acid phenylalanine, which then builds up in the blood and causes mental retardation and other abnormalities. Retardation can be prevented if phenylketonuria is diagnosed in the first three weeks of infancy and dietary treatment started straightaway. There is a universal screening test in the UK (the Guthrie test). Heel-prick …

Multiple sclerosis: sunlight, diet, immunology and aetiology.

Multiple sclerosis is more common in temperate latitudes, and migration studies indicate the involvement of an aetiological environmental factor in pre-pubertal life; the precise nature of this factor has never been identified and may perhaps have been overlooked. Here we suggest a simple explanation for the latitude gradient of multiple sclerosis, i.e. that it can be explained by the immunosuppressant effects of sunlight mediated via suppression of the secretion of the immunostimulatory neurohormone melatonin from the pineal gland. The effects of dietary fatty acids in multiple sclerosis also…

Low nickel diet: an open, prospective trial.

BACKGROUND: Nickel-sensitive patients may experience persistent dermatitis even if they avoid cutaneous contact with nickel-plated items. OBJECTIVE: The purpose of the study was to determine whether reduced nickel intake in food reduces the activity of dermatitis in selected nickel-sensitive persons. METHODS: Ninety nickel-sensitive patients who had a flare of dermatitis after oral challenge with 2.5 mg of nickel but had no reaction to a placebo were instructed to adhere to a low-nickel diet. RESULTS: Fifty-eight of the 90 patients benefited in the short term from the diet, whereas 15 other…

The diet and cancer hypothesis: current trends.

The current decade has witnessed an increasing interest in the diet-cancer issue as a central one for public health. Notwithstanding a substantial amount of epidemiological investigations, firm evidence of carcinogenicity exists only for alcoholic beverages with respect to cancers at several sites, and for aflatoxin with respect to liver cancer; also, the relation is established between diet related excess of energy intake, as translated into obesity, and cancer of endometrium and gallbladder. For a number of other dietary factors the evidence for a causal or protective role still remains at a…

Diet and lifestyle in hypertension: changing perspectives.

It is now recognized that dietary and other lifestyle or environmental factors are critical for the phenotypic expression of a genetic predisposition to blood pressure (BP) elevation. These environmental factors influence the entire frequency distribution of BPs in any given population, and hence affect the prevalence of hypertension using whatever arbitrary cutoff point is chosen to categorize patients in this way. "Dose-response" relationships with BP have been demonstrated with body fat, alcohol consumption, sodium intake, vegetarian vs. meat-related diets, and physical activity. Possible r…

Parents' understanding of coeliac disease and diet.

Parents of 50 children with coeliac disease were questioned about their knowledge of the disorder and the child's dietary compliance. Twenty of 50 parents admitted that compliance was less than strict. This correlated with poor knowledge of the disease which in turn was related to the family's social status. Membership of the Coeliac Society (23 of 50) correlated with both parental understanding and dietary compliance. Greater emphasis on dietary compliance and education of parents and patients is required.

Refsum's disease: management by diet and plasmapheresis.

A case of Refsum's disease treated by serial plasma exchanges together with a moderate low phytanate diet is reported. Serial plasma exchanges determined a rapid significant clinical improvement (neuropathy and cerebellar ataxia) that allowed immediate return to full-time employment. The initial improvement could be maintained by intermittent serial plasmapheresis despite partial failure of the initially introduced low phytanate diet bringing 20 mg phytanic acid daily. A new dietary regimen bringing 10 mg phytanic acid was later introduced that was well tolerated. No liquid formula was used. T…
