Congenital dilatation of Stensen's duct in siblings.

Congenital dilatation of the Stensen's duct is an extremely rare disease, which may have a hereditary background. However, there are no reports about the familial occurrence of congenital Stensen's duct dilatation. Herein, we present the first case of congenital dilatation of Stensen's duct in siblings. Therefore, we suggest that congenital dilatation of the Stensen's duct is an extremely rare hereditary disorder.

[A case of severe bronchial stenosis caused by a vascular ring in an elderly woman].

We report a case of bilateral severe bronchial stenosis which was caused by compression by ascending and descending portions of the aorta. Asthma-like symptoms developed 9 years previously and dilation of the ascending aorta was noted 2 years previously. Bronchial obstruction was suspected because of diminishing breath sounds in the left hemithorax. Investigations suggested existence of a vascular ring consisting of the left aortic arch, right descending aorta and possibly the right ligamentum arteriosum. The patient fell in shock during bronchoscopy. Emergency implantations of bronchial stent

[A case of diverticular form type of congenital choledochal dilatation with anomalous arrangement of pancreaticobiliary duct].

We report a case of diverticular form type of congenital choledochal dilatation with anomalous arrangement of pancreaticobiliary duct and bile duct stone. The patient was a 63-year-old woman with the chief complaint of epigastralgia. Abdominal CT showed low density area suggesting cystic lesion at the region of pancreatic head. MRCP and ERCP revealed dilatation of the common bile duct in the multiple diverticular form with anomalous arrangement of pancreaticobiliary duct. A 2.0cm sized stone was also recognized in the dilated common bile duct. The patient underwent resection of gall bladder an

Current treatment options for corneal ectasia.

PURPOSE OF REVIEW: The approach to the management of various forms of corneal ectasia is changing, with the advent of new surgical and nonsurgical options. The purpose of this review is to summarize and evaluate relevant studies on new treatments for keratoconus, postrefractive surgery keratectasia, and peripheral ectatic corneal disorders. RECENT FINDINGS: Various alternatives to corneal transplantation for the management of keratoconus aim to enhance corneal rigidity by means of nonsurgical collagen cross-linking, or with the use of intrastromal corneal ring segments, and studies suggest th

Successful surgical repair of an elderly patient with tetralogy of Fallot: a case report.

We report on a rare case of a 61-year-old female patient with tetralogy of Fallot who had a late radical repair. The patient had no previous surgical interventions, and her postoperative course was uneventful. One year after the repair, the patient is doing well and has no symptoms of cardiac failure. Surgical treatment greatly benefited our patient by improving her functional status and extending her life expectancy.

Mammary duct ectasia in children presenting bloody nipple discharge: a case in a pubertal girl.

Mammary duct ectasia occurs rarely in childhood. The authors report on the case of a pubertal girl who was operated on for duct ectasia with bloody nipple discharge. Duct ectasia is regarded as a primary lesion; it is considered to be a cause of bloody secretion, and it has a mechanism similar to that of mammary duct papilloma.

Recurrent pancreatitis in a child with pancreas divisum. Endoscopic therapy of a Santorinicele.

Pancreas divisum is a rare congenital anomaly of the pancreatic ducts that has been implicated in pancreatitis. In addition, the finding of a Santorinicele, which is a cystic dilatation of the dorsal duct, suggests that there is an obstruction associated with a congenital or acquired weakness of the mucosa. We used an endoscopic technique to treat a child with recurrent pancreatitis who was found to have pancreas divisum and a large Santorinicele.

Unusual presentation of esophageal communicating duplication in a child.

Communicating esophageal duplication and true congenital esophageal diverticulum are entities rarely encountered during childhood. The authors report an unusual case of midesophageal communicating duplication with secondary upper esophageal obstruction.

[Congenital cystic dilatation of the intrahepatic bile ducts (Caroli disease)].

In two men and three women aged 64-89 years on necropsy a localized form of congenital cystic dilatation of the intrahepatic bile ducts (Caroli's disease) was found affecting the left and/or one of the segments of the right liver lobe. There was diffuse fibrosis with striking reduction of the left lobe where the dilated bile ducts were macroscopically apparent only in two subjects with concurrent occlusion of the extrahepatic bile ducts by stones and carcinoma of the head of the pancreas. Segmental dilatation of the bile ducts of the right lobe was not associated with structural changes of the

Producing experimental cholangiectasis in dogs by the stripping method.

Despite the general belief that cholangiectasis is caused by biliary stenosis, clinical cases of cholangiectasis exist which do not seem to have biliary stenosis. The purpose of this research was to produce cholangiectasis models without stenosis, and it was found that stripping the surrounding supportive tissues from the extrahepatic bile duct resulted in cholangiectasis without biliary stenosis. The maximum diameter and intraluminal pressure of the bile duct were specifically examined in 24 mongrel dogs before and 4 weeks after stripping. The mean diameter of the bile duct was 2.44 mm before

Prophylactic colectomy in Ehlers-Danlos syndrome with colonic ectasia.

An adolescent boy with clinical features of Ehlers-Danlos syndrome developed ectasia of the colon leading to intractable constipation and clinically imminent colonic perforation. Total colectomy with ileorectal anastomosis in two stages was performed eliminating not only his intractable constipation but also the risk of potentially lethal perforation. This case illustrates an approach that may reduce the high mortality rate reported in patients with Ehlers-Danlos syndrome and colonic perforation.

[2 cases of the diffuse form of Caroli disease].

Report of two cases. The authors report two observations in what concern the diffused form of the Caroli's disease, one pure, the other mixed associating the congenital dilatation of the intra-hepatic bileducts with lesion of congenital hepatic fibrosis. They emphasize the efficiency of the echographia and computed tomography in diagnosing Caroli's disease. The surgical treatment of these diffused forms often complicated by acute recurrent cholangitis used with some success various methods of biliary drainage. But the radical treatment seems to be obliged to turn itself more likely toward live
