Turner syndrome (TS) is a common genetic disorder in females with high incidence of ascending aortic dilatation and even dissection occurring as early as in the second decade. Known risk factors (RF) are bicuspid aortic valves (BAV), coarctation of the aorta (CoA), and arterial hypertension. Since 10% of dissections occur in patients without RF, an intrinsic aortic wall abnormality has been postulated. This study aimed to investigate the elasticity of the ascending aorta as a surrogate marker of aortic wall texture. Forty-six pediatric patients with genetically proven TS were prospectively exa

Six patients with a large aneurysm of the ascending aorta involving the root of the aorta and severe aortic valve insufficiency owing to marked annular dilatation were treated by replacement of the ascending aorta and aortic valve with a composite unit. It was also necessary to transpose the origin of the coronary arteries with the use of saphenous vein grafts. In 3 patients, the aneurysm was due to a chronic dissection. In one patient, the aneurysm extended beyond the arch of the aorta. There were no operative deaths. One patient died suddenly of pulmonary emboli 11 months after surgery. The

Aortic dissection (AD) is a life-threatening tear of the vascular tissue with creation of a false lumen. To explore the mechanism underlying this tissue tear, this study investigated the delamination strength of AD model rats and the histological composition of the aorta at various stages of AD development. SD rats were administrated beta-amino propionitrile for 0 (Control), 3 (Pre-dissection), and 6 (Dissection) weeks. The thoracic aorta was harvested at 10-11 weeks of age. The Dissection group exclusively showed AD at the ascending aorta. The delamination strength, a force that separates the

Aortic dissection complicating percutaneous transluminal coronary angioplasty is rare. We report the case of a 45-year-old man who after right coronary artery angioplasty with stenting, dissected that vessel to involve the aorta to the bifurcation. Surgical repair with Gelatin-Resorcinol-Formaldehyde (GRF) glue as opposed to prosthetic graft replacement of the ascending aorta was successful. The use of GRF glue is effective in the surgical treatment of aortic dissection after coronary angioplasty.

OBJECTIVE: To explore the safety and feasibility of endograft repair in refractory aortic dissection and dissecting aneurysm.
METHODS: The clinical data of 13 refractory cases of aortic dissection and dissecting aneurysm, 11 males and 2 females, aged 52 (38-82), out of the 68 consecutive patients with aortic dissection and dissecting aneurysm who underwent endograft repair from Jan 2001 to Oct 2004, were analyzed. Nine of the 13 cases were diagnoses as with aortic dissection and dissecting aneurysm of Stanford type A, 3 of which had tears in the ascending aorta (DeBakey type I), 3 had tears i

AIMS: Dissection and rupture of the ascending aorta are life-threatening conditions resulting in 80% mortality. Ascending aortic replacement in patients presenting with thoracic aortic aneurysm (TAA) is determined by metric measurement. However, a significant number of dissections occur outside of the parameters suggested by the current guidelines. We investigate the correlation among altered haemodynamic condition, oxidative stress, and vascular smooth muscle cell (VSMC) phenotype in controlling tissue homoeostasis.
METHODS AND RESULTS: We demonstrate using finite element analysis (FEA) base

BACKGROUND: The mechanism involved in retrograde type A aortic dissection (RTAD) remains unclear, while research through morphological studies is limited. Therefore, this study aimed to compare the aortic geometric features between RTAD and type B aortic dissection (TBAD) and to identify specific anatomical predictors of RTAD.
METHODS: A total of 60 patients diagnosed with acute aortic dissection, with the primary entry tear located in the descending aorta, were included based on aortic computed tomography angiography (CTA) performed at our center between November 2019 and November 2023. Amon

BACKGROUND: Many dissections seem to also have a retrograde component. The aim of the study was to evaluate different sites of primary entry tears and the propagation of the dissecting membrane, antegrade and retrograde, in an experimental model of acute type B aortic dissection.
METHODS: The entire thoracic aortic aorta including the supraaortic branches was harvested from 26 adult pigs. An intimal tear of 15 mm was created by contralateral incisions sites 20 mm downstream the origin of the left subclavian artery. In 13 cases the dissection was created at the concavity and in 13 cases at the

Acute aortic dissection is a medical emergency requiring prompt diagnosis and intervention. Mortality in untreated Stanford type A dissection increases approximately 2.6% per hour during the first 24 h and may reach 47%-55% within 24-48 h. In critically ill patients, point-of-care ultrasound has emerged as a valuable bedside tool, particularly when conventional imaging is delayed or contraindicated. We describe the case of a 43-year-old man with Marfan syndrome, prior ascending aortic replacement with a Dacron graft (without aortic valve replacement), intellectual disability, psychiatric disor

BACKGROUND: Management of the enlarged, chronically dissected aorta after previous repair of acute ascending aortic dissection or after a previous cardiac operation may present a formidable technical challenge and the optimal method of management is not clearly established.
METHODS: Twenty-one patients with chronic type A aortic dissection (mean age 57 years) underwent resection of the ascending aorta, the aortic arch, and varying segments of the descending thoracic aorta. Single-stage replacement with perfusion of the aortic arch first to minimize the duration of brain ischemia and a bilater

BACKGROUND: Adolescents and young adults with Marfan-syndrome, Ehlers-Danlos-syndrome and with coarctation of the aorta are at risk of developing dissecting aortic aneurysms. The rupture of an aortic aneurysm has a poor prognosis.
PATIENTS: We report on two adolescents with Ullrich-Turner-syndrome (UTS) who died from dissection of an aneurysm of the aorta.
RESULTS: Including these two case reports we reviewed the literature concerning congenital heart disease and development of aortic aneurysms in UTS. Patients with UTS are at increased risk to develop an aortic aneurysm. Beside the basic ch

It is known that bicuspid aortic valve with dilatation of the ascending aorta is one of risk factors of the aortic dissection. A case of acute aortic dissection (DeBakey type-II) associated with bicuspid aortic valve who underwent successfully operation 8 hours after onset is reported. This patient went into cardiogenic shock because of cardiac tamponade and aortic valve regurgitation immediately after onset. Aortic valve and the ascending aorta were replaced using composite graft (#23 SJM prosthetic aortic valve and 26 mm woven Dacron vascular graft) combined with coronary artery reconstructi

The vast database of the Yale Center for Thoracic Aortic Disease--which includes information on 3000 patients with thoracic aortic aneurysm or dissection, with 9000 catalogued images and 9000 patient-years of follow-up--has, over the last decade, permitted multiple glimpses into the "playbook" of this virulent disease. Understanding the precise behavioral features of thoracic aortic aneurysm and dissection permits us more effectively to combat this disease. In this monograph, we will first review certain fundamentals--in terms of anatomy, nomenclature, imaging, diagnosis, medical, surgical, an

Considerable surgical progress of treating aortic dissection has been achieved during the past decade. The emergency indication for acute dissection of the ascending aorta (type A according to the Stanford classification) is unquestioned while surgical treatment for acute dissection of the descending aorta (type B dissection) is mainly reserved for complicated cases. The major complication of acute operations--fatal hemorrhage from the suture line and secondary multi-organ failure--have been successfully reduced by a progress of cardiopulmonary bypass techniques, the introduction of cold cardi

After recovering from severe COVID-19 infection, 2 women presented with chest pain. Computed tomographic angiography suggested acute ascending aortic dissection. At operation in both patients, the ascending aorta was encased in dense fibrous tissue, within which were focal collections of mononuclear cells, including many plasma cells. There was no entry tear or dissection. Such findings we have not encountered previously, and PubMed search of "periaortic fibrosis and COVID-19" yielded no similar cases or possible relation.

The proper surgical strategy for retrograde type A aortic dissection (RTAD) is still controversial, and some studies have reported the efficacy of frozen elephant trunk and thoracic endovascular aortic repair (TEVAR). A 68-year-old man was diagnosed with acute type A aortic dissection using enhanced computed tomography. The false lumen at the arch and ascending aorta was thrombosed, and the primary entry was placed in the descending aorta. In addition, there were malperfusions of the right renal artery and both iliac arteries. We performed TEVAR using the right femoral artery combined with the

A 25-year-old woman with Marfan syndrome in the 37th week of pregnancy was referred for acute chest pain and dyspnea. An emergency Cesarean section was performed because of fetal distress. Intraoperative echocardiography at the end of the Cesarean section showed dilatation of the aortic root and dissection of the ascending aorta. The patient underwent replacement of the aortic root and the ascending aorta on the following day because of uterine bleeding. The postoperative course was uneventful for the mother and her baby.

Bicuspid aortic valve (BAV) is the most frequent congenital heart disease, with an incidence of approximately 1%. It can be silent and associated with normal valve function. However, a series of complications, even catastrophic, may occur with time: valve incompetence, valve stenosis by dystrophic calcification, infective endocarditis, progressive dilatation of the ascending aorta, aortic dissection, sudden death. The problem of BAV is not just about the number of semilunar cusps, but also the aortic wall. Severe noninflammatory degenerative changes (elastic fiber fragmentation, smooth muscle

An 87-year-old female in shock was referred to our hospital. Emergency chest computed tomography and echocardiography showed a dissecting aneurysm of the ascending aorta and remarkable epicardial effusion. She was diagnosed as having rupture of Stanford type A acute aortic dissection with cardiac tamponade and an emergency operation was performed. While anesthesia was administered, cardiac arrest occurred and the chest was opened. We managed to establish an extra-corporeal circulation and performed graft replacement of the ascending aorta with GRF glue. The patient followed a satisfactory post
